I have lost my PAN card in New Delhi while traveling in METRO dated[protected], so kindly register a complaint against it so that no one can miss use that. Besides it I would request you to issue me a duplicate copy of that to the below mentioned new address.

I have requested for Surname correction in PAN CARD with all supported documents.. i have applied through local agent he said we are working but still no there is no response
i have applied on 21/5/2016 its more then month.. please check and let me know if anything is missed out from my side.Please call me [protected]

Kanahiya mahto s/o- lt durga mahto of mishra para, po-budbud, ps-budbud, dist-burdwan, state- west bengal.
He is no any tax submitted & his income is more. He is a illegal buisness man.He also supply illegal gas cylnder.
