Policy no: 10358178, date of commencement: 09.02.2018, proposer: anupam burman, life to be assured: boudhayayan burman (D. O. B – 23.05.1978), premium: rs 160, 000.00

Please go through the mail as i am under severe depression and yours company representative have damaged my life peace stability

This is to inform you that i mr anupam burman 69years old and above senior citizen had done a policy with your esteemed organization on the month of february 2018 with an amount of rs 1, 60, 000.00 by lending money from others and being a widower i have to deposit my late wife gold ornament in muthoot finance for the adequate money asked by yours company representative and the agent where my policy was put under is “soumick sarkar - direct agent with a agent code 60188721, email id: [protected]@gmail.com”.

These all money had been given as i had received a call from an unknown number claiming he is calling from insurance service provider where i had been asked that if i had any lapsed policy/any problem, they would help me out to recover the money with minimum requirement being fulfilled by me and according to that they had taken a appointment in my residential address where i lived. After he came i had told them i had a lapsed policy of met life and kotak life (Might be i had not informed earlier) which i had already made a complain against them to the company but to his confident i was made to understand by him that if i submit a total premium of rs 1lakh 60thousand once only i would receive the total money by 6 months with 200% confidence as per the norms of irdai. Added to that they had asked me for the documents of my son as my age would not permit as i am above the age to be life assured. I had told him as my son doesn’t live with me it would be not possible for him to sign the documents as well to give the documents but sumit jaiswal the agent had provided me his mail id as ([protected]@gmail.com) and had asked me to tell my son to send the documents down there and about the signature of my son it could be done later either the scan copy sign of pancard signature would be put there which i later found had been forged by them. On this mentioned fact i am once more attaching the signature verification form and identity proof with self attested signature put into it along with bank stamped and sign put into this.

It is really frustrating for a 69 years senior citizen who is a diabetic patient to deal with such people. On top of that i would like to inform you that when the policy was done with forged signature of my son, who was not in west bengal as mentioned earlier. I had personally called the agent and till today no action was taken.

They are violating the irda norms for life insurance coverage by your authorized advisor "soumick sarkar - direct agent with a agent code 60188721, email id: [protected]@gmail.com fraudulently had made them life insured for the disputed policies (Insurance contract document) when the life insured were not himself were in town at the time of doing the policy. From the signature of my son it clearly depicts every details had been forged including the signature.

So, request you considering the above mentioned fact pls. Return/refund my hard earned money as soon as possible as this is a fraud done by your authorized agent “soumick sarkar - direct agent with a agent code 60188721, email id: [protected]@gmail.com" with a very senior citizen of india, otherwise i will put these forgery episode in front of" insurance ombudsman” for proper justice. Added to that sir please note that the above mentioned complains i have put was much way back on the month of 9th may 2018 where the complain was acknowledged on the same day and on 18th may an additional requirement was asked for but that time as i was seriously ill i had mailed the same to yours customer service and one of aviva sales team as mentioned to me is” ayanangshu samajadar” email id:ayanangshu. [protected]@avivaindia.com who had personally requested me on behalf on sumit jaiswal to withdraw the complain and they would refund my money on medical ground which is true, but on the other hand this sumit jaiswal had given me a written declaration from pnb met life and kotak life insurance with a logo and stamped put into this that my money will get refunded and approval had came for this which is attached with this letter.

Also to my more disappointment i had mailed several times and had received mail from your team aviva on the date as 18th may for additional requirement the on 19th july mailed to your sales team name as ayanangshu samajadar received mailed agin on 20th july acknowledgement to be patient as they are treating my request with priority then again on 25th july i had mailed mr ayanangshu samajadar to treat this with utmost important and 10th august i had personally mailed your customer service for the time required as:this is reference to your mail received on 25th july 2018 where you have asked for : bank attested signatures in attach format along with self attested scan copy of valid id proof of life assured (Boudhyayan burman). On this above requirement as my son stays out of town most of the time i have asked for the required documents from him which he will be sending by 10working days and as i am medically not well for this i was not being able to send the required documents for the same.

And on 9th september lastly i have send all required documents needed by your aviva team in a maild id :[protected]@avivaindia.com and on very next day i. E. 10th september 2018 i have again received a same acknowledgment mail that they will treat my case with priority and i would also receive a call from your redressal team name as sonu sharma probably but to my sorrows i haven’t received any call regarding the issue despite of that i had received again a rejection mail dated on 19th september 2018.

Here is a list of reasons why the advice i received when i was sold endowment policies in the name of my wife and myself have been with bad advice and huge mis-selling.

The endowment policies were not suitable for me.

The sales representative did not properly establish the reason why i am putting money on this plan

No fact finding was completed during the sales process and therefore the sales representative gave a huge mis commitment of the plan which we found to be false later

The representative named as sumit jaiswal who had mis-sold my other company policies also where i have complain recently gave me a call that my previous invested policies would get merged into a new fund with different company name as aviva life insurance and accordingly he had bought all letter found to be a false documentation but to be honest at first sight i was convinced as he had stamped the documentation with yours company name and other company where i use to have policies were also stamped by him. He said he would help me out to recover the money with minimum requirement being fulfilled by me and according to that he had asked me to provide around 4lakh amount so that he can merged this policies with my previous company. After he came i had been told that a approval had came regarding my merging of policies for which i had been provided a approval copy with irdai stamped and kotak life stamped on other letter along with pnb met life approval letter on it and to his confident i was made to understand by him that if i submit a total premium of rs 3lakh 85 thousand to aviva life and bharti axa after being negotiating the amount from 4lakh i would receive the total money by 6 months with 200% confidence as per the norms of irdai.

Other options for repaying the policies, such as a withdrawal procedure were not discussed with me.

The representative said these policies would definitely pay off within the stipulated time given to me and gave me a vague figure i would get back.

The representative said there would be a lump sum payable in addition at the end of the 6th month which later got extended to 1year and did not explain there was a risk the endowment would not meet the target amount.

The representative did not discuss in full the funds my endowment was to be invested in or when actually i can withdraw my money back in full.

The representative did not explain the endowment was a long-term policy and that i would need to be able to keep up payments over the whole term.

I already had an endowment plan with previous company which was mis-sold to me as said above by the same representative and the adviser told me to cash it in and sold me a new policy by merging the same with the previous company and told me as insurance terminology as portability.

Also he had taken my documents of aviva life documents along with other life documents invested by him assuring that for my peace of mind he is taking these and if possible he will correct all the discrepancy and would returned it back as soon as possible. Because i had insisted him to cancel all the policies as it was within the free looks period.

Also he had shown and given me a copy of preapproved cheque of around rs 7, 08, 000.00 which i will getting from aviva life as per merging norms of irdai

For me it would be merelly impossible to run this plan for long term as i have lot of others liablities to look into and i am widower senior citizen of around 70years with no fixed income to run this plan as i worked as contractual basis in amul dairy
