Respected Sir/ Madam,

Please refer to your trailing email on the subject matter,
We deeply regret the over-delay in approaching your good self now, you can feel and estimate that the social rituals in respect of the deceased son takes time to finally settle down with a little amount of peace in mind, really very sorry.

We, as a family members, have undergone the situation and circumstances in reality during the period since beginning of the illness of Puneet Verma till his last breath.
In brief, as a matter of fact, last year he suffered from jaundice basically and was under regular treatment, but it was not improving, he was taken to the Institute of Liver and Binary Sciences, Vasant Kunj, New Delhi, for specialised treatment, where he was admitted ONLY in GENERAL WARD, with a view to taking his proper investigation and care. Since he improved a lot, he was discharged in FIVE days only and further advised to attend the OPD in hospital regularly in consultation with Senior Doctor Mr. Vinod Arora, which he continued regularly as per doctor’s advice and took the regular medicines and tests as prescribed from time to time.

It was unfortunate on July 22nd and 23rd, 2023, that he fell sick with fever, he was immediately taken for consultation with the family doctor. At the time of consultation, his medical history regarding jaundice, etc. was explained properly as a precautionary measure to the doctor. All the medicines and diet schedules were followed properly as prescribed. However, the next day he was having complaint of loose motions, but the next morning he was good enough and healthy without any doubt and taking all medicines.

Now, next the problem started when his legs started swelling by noon, and by late evening, instead of any improvement, he was unable to stand and walk, and the swelling was increasing, including on his stomach side. Upon this, we decided to take him to the ILBS Hospital, where he was admitted in EMERGENCY on the night of July 27, 2023.

The doctors on duty in the hospital, after studying the investing reports and medicines he was taking at home, as advised by the family doctor. They informed that the “MEFTAL” medicine had not suited his body and reacted strongly, resulting in the present situation at that time. We were further instructed to be careful strictly not to take “MEFTAL” group medicine in the future. He was shifted to the ICU, and after the primary tests dated: - 28th Jul 2023 (test reports appended for your ready reference in sequence date-wise till his treatment in ILBS), the very first report revealed that merely and solely the MEFTAL tablet had very badly affected both the kidneys and advised to go for dialysis. As a result, one of the kidneys improved and started functioning properly. The next day, when they decided to go for the second dialysis, it was found that the second kidney had automatically improved and started functioning properly. as can be studied from the reports appended for your kind perusal. However, due to such an infection/ reaction, it was told to us that his lungs and liver were also effected badly and since the liver was already weak, we may have to go for his liver transplant, and we should be ready for that, Still, that can only be taken up when his health has improved, then only. However, the reports revealed that his bilirubin was reported as 11.9 and his creatinine as 0.99.

It will be appreciated that the improvement in the overall health of Puneet Verma was as good as that the ICU In-charge Doctor Ms. Rakhi, advised him to be shifted to the lower ward i.e., Semi-ICU (HDU), and keep him under observation for two / three days. The test reports continued and revealed the increase in bilirubin, but doctors advised that the treatment of the liver shall be taken up separately, but let him improve here, with a view to shift him to GENERAL WARD. On 4th Aug, 2023, it was decided to shift him to General Ward by evening and was given breakfast properly around 10:00 AM.

We will not hesitate to emphasise here, which may please be noted carefully and clearly without any prejudice, that the patient was not suffering from any other ailments/side effect / after effects of anything else as presumed by your good self(Please excuse and don’t take it otherwise) and was in reality got all of a sudden ill, and went under critical condition day by day, fully and only due to the reaction of the Meftal as it will be revealed from the further treatment and physical condition and reports of the patient from time to time and appended for your ready reference.

It is brought to your kind perusal and taking note of it that, the situation of the patient all of a sudden changed the scenario when around 13:00 Hours on the 4th Aug, 2023, after 3 hours of taking breakfast, we were called and informed that some amount of blood was coming from his mouth, and he has become serious. As such being shifted once again back to the ICU. It was revealed that his liver was punctured at three places and since Doctor Rakhi will come on Monday i.e. 07th Aug, 2023, we are taping the liver at three places, further course of action shall be decided when ICU In charge will come back. As can be seen from the reports appended, the patient’s condition went on critical to critical, and the infection reached to the uncontrollable level.

Upon, return of the Doctor Mrs. Rakhi (HOD – ICU) on 7th August 2023 and studying of the critical condition of the patient, she insisted upon the family to sign the hospital papers/ undertaking to shift the patient to the VENTILATOR and simultaneously insisted for arranging liver transplant. She was uncomfortable for his this condition, but said at the same time that during ventilator treatment, we will not give any treatment for the liver but, rather, simply keep the patient under High Oxygen Pressure, which frankly speaking not felt safe and useful by the family, so she refused.

Upon this, she forced us to take the patient to some other hospital, ultimately, we had to decide to shift the patient.

Firstly, we, with great efforts, were permitted to be admitted in ICU in AIIMS, New Delhi., but again, it was really very unfortunate that before we could get clearance from ILBS, there was reported a BIG FIRE at AIIMS at noon, so we were denied.

Then, we tried for Dr. Ram Manohar Lohia Hospital, New Delhi, where ICU was not available immediately despite our sincere efforts and had to be adjusted in Emergency then after General Ward. Despite all efforts by the team of expert doctors, the infection was uncontrollable since the night of 7th Aug 2023. When he was admitted in RML Hospital, till he breathed his last on the morning of 9th Aug 2023 around 06: 50 AM. However, ICU availability was made for us around 07:30 AM, but he was no more there. The story ends in this way, all the family members, colleagues & friends were not ready for this shock. He was our only son, his wife, mother, sister and father are under extreme sorrow and suffering from unexplainable pain which can be realised by any one of you.

At this stage and under the critical circumstances the scene moved like an episode on the screen, we feel looted by one and all, we don’t know what investigation your team members have carried out at ILBS Hospital and concluded the claim as CLD ethanol. As a matter of fact, if the above circumstances and reports / Facts are put up before any Board of Officers, they will not be able to reject the claim we hope.

It is our humble request to very kindly review the case and adopt a sympathetic view to approve the claim to the extent possible. Family is now running on pension of father only; we have gone under debt beyond our capacity and have been left in no way to come out of it. We went on depositing the amount with the hospital only on the hope that the claim shall be approved by the insurance, our trust has been hit strongly. Ultimately, we are left to suffer for whole life, which is not sustainable under laws of equal natural justice and nature.

    Forwarding below the communication to cheater agent of PNB Metlife who has misguided me and sold me the policy on the commitment to refund old policy value alongwith brokerage charges within 90 days of policy issuance and letter on switch off the mobile and not responded my several calls and mails to him.
    I have also made so many complaint to PNB Metlife and they know that fraud has been done but no action till date for cancellation as well as against agent.

    You are requested to visit PNB Metlife and arrange cancel my Policy immediately to avoid legal action on you for cheating.

        I am extremely not satisfied with the answer given by PNB Metlife Customer care because they are interested in their business and protecting their cheater brokers and this issue is related with two policies which are mentioned below in trailing mail. PNB Metlife not responded by more 20 mails and physically complained submitted for cancellation on 11th Dec’21 but no response received from him for the last 2 months. Even nobody from their office has shown courtesy to call me and understand the situation with the exception of a Senior Executive i.e Ms. Laxmi call received one month back and she understands the fact but replies through e-mail which was100% negative in favour of company & cheater brokers.

        Synopsis of case are as under for your kind considered and immediate action against cheater agent of PNB Metlife and against the PNB Metlife Company who is not cancelling my policies and protecting fraud agent and innocent persons are losing their hard core money.
        1. I had taken a policy no. 22277866 in July'17 with an annual premium of Rs. 56751/-
        2. Call received from one cheater mentioning working with PNB MetLife naming Ravi Sharma around 10th July'21 and confirmed to me that in your current policy there are huge brokerage charges and do not pay the premium and I will cancel this policy and you will get a new policy without brokerage charges. The mobile no. of this cheater are [protected] & [protected]
        3. He sold me a new policy no. 23873506 on line the no. is dated 15/7/21 of Rs. 55000/- with annual premium.
        4. Mr. Ravi the cheater again called me on 21st Aug'21 and disclosed that if you will take another policy than I will arrange to reimburse bonus due of `1.90 lacs on your policy dated July'17 with an assurance that later on I will cancel this policy direct from my office and will return 100% money after deduction Rs. 1000/- of policy cancellation charges.
        5. He again sold me online a new policy no. 23928793 dated 24/8/21 of Rs 90, 000/- of annual premium
        6. Mr. Ravi the cheater also arranged a con call with PNB MetLife officer in Noida to satisfy me before selling the new policy. Later on I understood that that was a cheating call and they made me fool.
        7. i chased to Mr. Ravi immediately after 75 days for status of refund in the end of Oct'21 and he respond me for next 15-20 days with a status that your refund is in progress and from 1st Dec'21 onwards switched off both no's and then I come to know that he had cheated/misguided/forged with me.
        8. I had tried to talk with both the agents and they neither responded on their provided cell no. in the policy nor give reply of many mails marked to them to help me.
        9. Finally submitted a written complaint dated 11th Dec'21 to cancel both policies in your karkardooma Office in Shahdara, Delhi-110032 but they have not yet replied as I think due to PNB MetLife culture.
        10. I am giving hereunder details of your Brokers who have never talked to me before the policy or after the policy because i had talked only on line with cheater Mr. Ravi Sharma ( I am sure he must discussed with me fake name but as a company you can identify the identity of culprit through the mobile no.'s)
        11. ASC Global Master against policy no. 23928793 dated 24/8/21, Broker code no. 57853668, cell no. [protected] with e-mail id no. [protected]@globemasterimf.in
        12. Shikha Khullar against policy no. 23873506 dated 15/7/21, Broker code no. 60098933, cell no. [protected] with e-mail id no. [protected]@gmail.com
        Till date I have called up so many times to PNB MetLife and Cheater Brokers as per point no. 11 & 12 but no response received.

        Now I want the cancellation of my both policies on the basis of above facts.
        1. What action has been taken till date by the Company against the both cheater brokers who never talked and me and PNB MetLife mentioned their name as an agent of the company in my policies
        2. Who will compensate me for the loss of interest which I am paying till date to my friends from whom I have taken a loan in the anticipation of a refund from the MetLife?
        3. If PNB MetLife is not able to deduct cheater brokers commission and not able to pay me that it is the responsibility of the PNB MetLife to compensate me interest from Aug '21 to till the cancellation of policies.
        4. What is the interest of the company to not cancel the invoice? Are PNB MetLife employees are sharing some part of commission which is getting by the cheater brokers on false commitment and fraud with the innocent customer.
        5. Finally how one man can issue the policy for separate brokers because only the cheater Mr. Ram Sharma had contacted me for policy cancellation, refund, issuance etc?
        Now request you with deep heart to cancel my invoice and refund my money and confirm the action against cheater brokers to avoid escalation to take further action with PNB MetLife and individually with both brokers.
        Please reply with refund orders within a week to avoid further proceeding.

          Hi Team,

          I was admitted to hospital from 11th Aug 2021 to 20th Aug 2021 for Abdominal pain(Gastritis and pylori bacterial infection). After discharge, we are following up with prudent broker for claim reimbursement, but every time representative is rejecting the claims stating different reasons.

          We have provided all the documents, discharge summary, letter from doctor, drug chart to Insurance company.

          Please help.

After giving so many reminders royal sundaram has denied and asked me to submit claim documents to their chennai office. Thus i have submitted documents to chennai office on 08/6/2021.               Dear Sir / Madam,

              My mediclaim claim number is 23922322 vide P.no.[protected]of National Insurance Co. Ltd. I have received the letter dated 03/04/2021 with sanctioned amount as Rs. 90000/- out of Rs. 191030/-.
              * You have deducted Rs. 78830/- as Policy excess / deductible.
              IN this pandeamic situation when my wife in trouble and we tried to other hospital for 3 hours but we
              not get admission in KEM Hospital, Dinanath Mangeshkar Hospital, and Ruby Hall Clinic so
              we have no choice through our Doctor we get admission in Poona Hospital.
              I am not understanding what is this policy excess / deductible is deducted that too 78830/- which is too high.
              You have increased the premiums by more than 50% this year, still being a loyal customer of National since more than 10 years, I have paid that high premiums, and still after taking a policy of Rs. 4 lacs I have been paid claim for less than 50% claimed amount which is not fair at all and not accetable by me.
              Kindly consider the claim once again and pay me the remaining Rs. 78830/-
              .

                  With due respect, I Ajay Dubey resident of Barhalganj of dist Gorakhpur up took a policy of HDFC LIFE SANCHAY PLUS.The employee Ankit kumar Srivastva convinced me for taking an insurence policy and I did also for that 100000/-inr is to be deposit yearly.But He deduct 4500/-inr extra from my ac without my permission whereas they did not tell me before about that.If they notice before I never go for that.From 1st day of my policy i request him to cancel the policy and refund my money and now i have already told him that i will never continue it.I also mailed to the concern department of hdfc life but no further action has been taken place till time.They avoiding the fact.

                    I have purchased this plan 5 month back, recently I have received call regarding this product is not good etc..., and the explained the disadvantage of this product, and the advised me to complaint bima lokal, and i agreed, and suddenly i received call in bima lokpal, and now this point of time i am confused, are they fake calls or what? because the suggested me another policy to recover this amount.

                      Hi Team,

                      This is to bring into your notice that I was trying to reach the icicilombard for taking the Senior Citizen Health Insurance for my mother who is diagnosed with cancer in July'20 and undergoing through the treatment of CA Cervix Cancer 1st stage but all the executives clearly denied that they do not cover pre existing cancer even when this option is clearly available in their imobile application. PFA snapshots for the reference and call recording for their continuos denial of not covering the Gynaecology cancer.

                      Finally, I raised a concern with IRDA with token number-IRDAI Token no.[protected],[protected] then only your(GRO)- grievance rederessal manage...finally replied by saying as below-

                      "Dear Sir,

                      We write this with reference to your concerns registered with IRDAI vide token number[protected].

                      As per our telephonic conversation held today, we would like to confirm you that any pre-existing disease Declared by insured and accepted by our Medical UW team will be covered after certain waiting period. For this particular product available on our I-Mobile application we do not have any PED specific waiting period (30 days initial waiting period is there).

                      Further in regards to your concerns regarding PED ‘Any Gynaecological Breast Cancer’, as this is not a part of permanent exclusion we will cover the same after initial waiting period of 30 days.

                      Also an appropriate feedback has been shared with our concern department for non-occurrence of the same in future. We will strive towards improving our customer service standards to give you a better experience henceforth and we request you to treat this incident as an exceptional occurrence and not as an indication of our service levels.

                      Then as per his confirmation, I finally made a payment of Rs. 65, 500 with details as mentioned below:

                      But after the payment, I have made more then 200 calls to [protected] and to icici lombard customer care and more than 10 reminders over emails but they are not responding to me at all and even not picking up the call and even after providing me the approval, they have cancelled my policy.

                        Sir, i have to policy in the name of vikas gupta and pooja gupta (My wife) of max life insurance 50000 each premium. A person who has cheated me his named is manish aggarwal (M: [protected], [protected]) in august 2016. He called me that he is calling from bima lokpal irda and suggested that he can port my bima policy from max life reliance nippon insurance because the agent which is axis bank will take high amount of commission from my policy. So in his talking way he has told me that don't give the premium or continue your max life. He has given the number of alok mehta of max life ([protected]) and you can talk to him he will support you to and he will merge the both premium amount of max life to reliance. At that time was doing call and sending messages and he gave me trust that in 1 or 2 months it will done. And a lady her name is supriya from reliance ([protected]) call me that in reliance policy there is a clause of port the policy and your amount of premium of max life insurance will merge in reliance policy. So i have signed all the documents of reliance policy. After that i have regularly all them and they gave me promise that process is going on. You have to wait for next premium and after that your max premium will add on in the reliance policy. But still nothing has done. All them three has change there number. It has bee since three years i am continuing the reliance policy. I have called to max life they said that this kind port can't be possible. They said if will not continue or pay the premium the policy will be lapse. Now today a person has called me name showing through true caller which is "siya bhardwaj fraud" m: [protected] (Voice in typical haryanvi language) saying apka paise aa gaya hai policy main. I told him that nothing has been come in the reliance policy. He said "ohhoo abhi tak na aaya paisa, fir ab ke karoge" i said to him i will launch the fir. He disconnect the phone.
                        Please suggest what to do. I have still not make the fir.

                          I want to inform you that o had a medical insurance with bajaj allianz, but when i applied for the cashless system, they denied it. They said that your mother is having a disease before taking your policy but my mother had no disease before. The doctor also wrote clarification on the letterhead, but they still denied. After denning the bajaj allianz, i had also complained yo irdai but i got the same reason from the irdai. Please find the attached related documents for your reference.

                              मैं जीतन सिंह पुत्र विंध्याचल सिंह ग्राम तरकुलही पोस्ट समदारखुर्द जिला गोरखपुर का मूल निवासी हूं! मैंने किसान क्रेडिट कार्ड से लोन लिया हूं! मेरी फसल बर्बाद हो गई थी! मैंने यूनिवर्सल sompo जनरल इंश्योरेंस कंपनी लिमिटेड से फसल बीमा कराया था !मैंने यूनिवर्सल sompo जनरल इंश्योरेंस कंपनी में 13/03/20 को कंप्लेंट किया था जिसका मुझे फसल बीमा का कंप्लेंट यूनीक इआईडी मिला जो है UP21601225 ! अभी फसल बीमा कंपनी ने कोई कार्रवाई नहीं की मेरी फसल बर्बाद हो गई थी मुझे कोई मुआवजा नहीं मिला और मैंने कंपनी में बहुत से इमेल किया हूं मैंने कंपनी में बहुत सारी मेल किया हूं मगर कंपनी की तरफ से कोई जवाब नहीं आया और मैंने मेरा केसीसी एसबीआई से है मेरा केसीसी अकाउंट ही मेरा इंसुरेंस पेपर है कोई भी मेरा क्रॉप इंश्योरेंस फसल बीमा को बैंक में जाकर देख सकता है मेरी फसल बर्बाद हो गई है मुझे मुआवजा का मुझे बहुत आवश्यकता है! दिनांक 19/04/20 की रात में ओला के साथ साथ बारिश हुई जिससे पहले की बारिश में बची हुई फसल को भी नुकसान हुआ किसान क्या करें! कंपनी वाले केसीसी का इंश्योरेंस का पैसा काट लेते हैं लेकिन जब मुआवजे की बात आती है तो नहीं देते हैं!
                              अतः आपसे विनम्र निवेदन है कि मेरे केसीसी की फसल बीमा का मुआवजा दिलवाने की कृपा करें!

They have promised to give cashless but rejected after that told me about reimbursement but now rejected that also by saying its a pre existing disease but that is totally wrong as i have all the proofs that it is sudden onset jst 45 days before this disease...                                     1) this is to inform you that jaipur branch hdfc agent of hdfc standard life insurance company and its authorised agent fraudulently sold 4 life insurance policies to my retired father in law, shri ashok narayan bhise, (Retired from jaipur dairy, as casual worker, on 30/06/2016 with no other income source) with heavy annual premium totalling to rs 3, 24, 748/- for next 10 yrs. Or 15 yrs.

                                    2) the agent gave a misrepresentation that these are personal pension policies, with only one time premium payment and like fixed deposit, policy-holder will get back his money along with interest, after three years.

                                    3) on the basis this misrepresentation of one time premium policies, the agent got the policy forms, which were in english, (My father in law is 8th std pass and doesn’t know english well) signed by him and acquired blank cheques from him and thus fraudulently sold 4 hdfc life policies of hdfc standard life insurance company ltd, to him.

                                    4) when after three years, my father in law went to hdfc standard life insurance company’s jaipur office, the branch people told him that these are annual premium policies with term of 10 yrs. Or 15 yrs. And are lapsed due to non-payment of further annual premiums. He was shocked as the agent has told him that all the policies he purchased in 2016, were one time premium policies.

                                    5) the basic question is on what basis, the hdfc standard life insurance company along with its authorised agent sold these polices of heavy annual premium totalling to rs 324748/- payable for next 10 yrs. Or 15 yrs., to my retired father in law, who doesn’t have any other source of income?

                                    6) is it so that hdfc standard life insurance company, keeps on issuing the policies without properly informing the client about future premium payment or verifying future earning capacity of its client?

                                    7) the details of policies are as follows-

                                        Dear Sir/Madam,

                                        When I tried to pay for my yearly premium on 16th or 17th of Feb 2019, I realized that payment is already done through my Credit card account. I called HDFC ERGO Customer care twice or thrice, Customer care representatives confirmed me twice that Payment is done through 'agency' on 5th Feb and not through Auto-pay/ Standing instruction kind of. May please refer the audio calls of Feb 2019 17th/18th.

                                        HDFC Bank (The credit card Bank) still did not confirm that there was any auto-debit or how the payment is done, without any transaction (Consent) and even without any information. HDFC Bank requested me to ask the same query to HDFC ERGO through email.

                                        Requested HDFC ERGO the same query through email. HDFC ERGO did not respond.

                                        I had to send a reminder to HDFC ERGO to get a response. They confirmed this time that the payment is done through Auto-Pay which is completely different from their previous response. I am a sensitive and sincere customer. I did not do any act till now which can be valued as my consent or in agreement to go for an yearly debit. In general practice too for my utility and credit card bills I never go for Auto-debits and in this case the Premium amount is negotiable (check the last 10 years record).

                                        There is no sms sent by HDFC ERGO prior or after the payment. They have sent the Insurance policy certificate on 10th Feb through email.

                                        Moreover, In cases of ECS payment I know and Auto debits (I know from my friends) that there are communications from the Beneficiary banks/Merchants just before the ECS monthly debits and also there are communications of Receipt particularly in cases of Auto Debit.

                                        It is a clear case of the Insurance company going for an illegal transaction having encrypted my card details during 2018. Contrarily HDFC ERGO Representative Vaibhab has tried to contest me rudely and threatened whether I should continue the policy or not (Most unwanted question now)

                                        Request for necessary remedy and necessary action.

                                          Sir,

                                          I have purchased a policy "smart assure"from dhfl pramerica and paid my first installment in feb, 2018. After going through the policy documents and inquiring with the company about the terms of the policy i came to know that the terms explained to me for the policy by the concern agent was mis-communicated and mis-guided. Now i an getting call for the concern for making payment for the 2nd installment, i asked them to refund my premium amount as the benefits explained to me was not anywhere mentioned in the policy doc and even the company has not confirmed for the same.
                                          The concerned replied me that the premium paid by you of an amount of rs 52, 251/- is not refundable, it means my premium amount is lost just because of mis-communication and mis-guiding from the agent of the company. Please help me out from this, i want my hard earn many refunded back to me.
