I had applied for family pension through Empire Industries Ltd the employer of my wife Vijayalakshmi Rajan ( deceased on 11/6/2009) , on 25/10/1952. the Ref is MH/BAN/516/6404.
Form 10 D alongwith all annexures have been sent. I understand through a telephonic communication that the pension has been sanctioned . But the pension is yet to be conveyed and credited to my account.

I would like to submit that I had opted for VRS in the year 2003 from Indian Telephone Industires Ltd., and was relieved from the services of the company w.e.f. 30.11.2003. At that time I was less than 50 years in age and was not eligible for monthly pension. Hence, Employee Pension Scheme 1995 Certificate vide No: KN/KRP/4392 was issued in my favour. It was mentioned that this certificate is to be surrendered...

A/C No.[protected] CR No.[protected] Monthly Pension Not Received since January 2012.
I have not received my monthly pension since the month of January 2012 against my above mentioned account.
In this regards, I have personally handed over my Life Certificate of Axis Bank on 9th April 2012, but till date I have not received the payment.
Please look into this matter and arrange to send my monthly...
