TN/18/103/0207054 dated 15.12.1997.

My present address is 14/1, Alacrity Flats, SPIC Nagar, Vijayanagaram Ist Main Road,

Getting back to your question..

If you need to change your address, change your name or whatever,
Look out for electoral list inclusion drives in your area. Generally they occur a few months before an election.
You need to fill up the pertinent forms and back it up with proofs like ration card and a photo ID card.

All the best for your endeavour. Its not going to be easy. Be prepared for humiliation and a lot of running from pillar to post.
All for what- electing a neta with a criminal record.
