Dear sir i need to change delivery address in ESIC Form as i am from Hyderabad need to delivered in local Hyderabad office as it is mentioned as Vijayawada office please change as i said my ESIC NO: [protected]

Sir, on 19 apl 2016 seriously injured, (First time) went esi eranakulam north hospital with my esi card. From counter they reject me by following reason.
1. For to get op ticket register temporary id card required.
2. Before coming any other esi hospital you have to register it nearest esi dispensary. (Actualy my place is kannur dist it is far away from eranakulam)

I have a complaint regarding to my esi claim. I have submitted the required documents to the esi office located at irinjalakuda on 26.02.2016. Becaue i have not received the claim. I got there and made an enquiry about it, and found out that the documents i have submitted has been forwarded to the thrissur branch had been misplaced by the authority people at thrissur . They have lost the documents and not informed me to resubmit it. After then i have resubmitted the same documents on 16.03.2016 since now i have not received the payment. I have made an enquiry call the thrissur branch office to this number[protected] several times and they have not received the call even one time. Then i visited the branch office for further details on 08.04.2016 i had an very bad experience from the branch office and found out that they have done nothing for the claiming process. The lady there who talked with me took a waste paper and wrote my phone number and details and ask me to leave. As i asked the lady her name and about why the documents is not forwarded and why you have not recieved the call?. The lady replied that the phone was complaint and angrily said that she has no interest in replying me and insulted me in front of everyone. As a person i felt very ashamed to visit a government office with such a response.
Till now i have not received any response from the esi office thrissur . I have been trying to contact them from 09.04.2016

With regret to inform you I am not received payment against I am submitted bill for Rs.8262/75 & another one Rs.9500 approx for reimbursement against my self treatment' on 27.11.13 at esic lahurabir, Varanasi, U.P. .I am visited so many times at lahurabir office with staff handling claim but every time he will reply wait and wait and asking not in there hand.
This is Misjudge with me.

Shri maan ji main ESI service se bhut dukhi hoon ..mera last 1 year six months se ESI fund salary se deduct ho rha hai...but jab bhi main iska labh uthane ke liye ESI Dispensary jata hoon toh woh mujhe beizat krke bhga dete hain..mere pass e-pehchan card hai but abhi permagnent nhi bna hai...Recently aaj hi Nabha ki Dispensary se beizat ho kr aya hoon..Please help me..

My wife is a cardiac patient in SCI MT IN TRIVANDTRUM, she's need cath lab study, its expenditure is 15000 RS.there MRD section gave estimate certificate for adavance from esi.but my esi branch kottarakkara not give the advance.what i can do?

I applied for esic exam. and i choose my state district as exam centre. But I got exam centre in other state. What is this tell me? Girl candidate and pwd candidates are getting their home centre but the boys are getting centre out of state.this is the real inequality esic is doing against boys candidates.please help us ..

Sir, i applied for the post o[censored]dc in the latest recruitment of employee's state insurance corporation. They have allotted me examination centre bells institute of management technology, knowledge city, mehli, , shimla, himachal pradesh which is minimum 250 km from my home city patiala. This is not possible for being a unmarried girl to give exam from 2-6 pm and reach patiala same day because there is no bus service from shimla to patiala after 2 pm . It's very unjustice for me both mentally and financially . Sir, i have allready requested them to change my exam centre to chandigarh or mohali which i gave my preference while filling form. But i have got no response so far. My exam is on 18 march 2016.
Sir please look into the matter and bring me justice . Otherwise i will be compelled to drop my exam.

In Berhampur town one esic dispensary in kamapalli, but esic dispensary is not working properly, Doctor's are not coming properly in medical time & not giving medicine properly, so we have faced many problems, the medicine's are damaged, so pls immediately action, it is our request,

Myself Hemant Singh sisodiya. I have submitted my medical bills at the Udaipur dispensary on 3/12/2015. I have not still have any claim ID and any info as well. I don't know is it in the process or not and nobody can answer satisfied. I have only received a receipt that i have attached.

i want apply for my refund. i tried 5 or more time, but there is no any person relating refund proedure.

there is nobody at their seats/offices at esic dispensary.

fer mugko emergency mai brain operation private hospital mai karna pada jab mai2 month k baad esi hospital gaya to mera claim nahi liya or bola aap bena transfer k dusere hospital mai tritment karya is liya case file nahi kiya mai aaj bi sabhi document likar hospital k chhaker kat raha hu plese help[protected]

No proper care of patients in esic hospitals. which deases is remove from medicines they forced for operate. never listen patients problems.

Dear Sir, The complaint is against one of the staff of ESI Dispensary, Palarivattom.P.O, Ernakulam Dist., Kerala State. For availing information regarding the bank account to be started for getting Maternity Benefits for my wife, I contacted the inquiry number [protected]. First day he told me that he is outside and will reply tomorrow. Next day also he told me the same thing. The third day when I contacted he told that today is Sunday and contact him next day. Like that he made me beg 5 days. Today morning when I contacted him he shouted at me that 'how many time I need to say to you that you should come and enquire it in the office'. Then I asked him that then why couldnt he tell this earlier, then he uttered some bad words and disconnected the call. I contacted him again to know his name but he is not willing to reveal his name. The only thing I know about him is the contact number [protected]. Dear Sir, I believe that its my right to information. I wonder why these Govt. Servants behave so rudely to ordinary citizens, since they are the representatives of the GOVT. Hope you will treat this complaint with much importance and close this issue at the earliest. Hi Team, I am Vimal Kumar Thakur. some issu facing your side Last 2year.some reason his last my company name is Verex. and my esic No.[protected] not received this card. and now left this company.now my current company Ams Comtel in this company my Esic no:[protected].also not received my company.my HR told vist nearist Esic Office, so finaly visit your office G-118, sector-09, noida, 201301but there looked this office not mentained any Recoards and your Empolyee Name.Ahmad told me go to the other esic office.sector-57, noida, there some lady staff told agin visit sector-09, noida office this cardthis place.both address visit but not reciving cardkindly help.
