      Annuity No: 064M[protected] as mentioned in your Letter Dt. 03/02/2009 however same is getting featured as 61M[protected] in my HDFC account while crediting annuity from your end through ECS.
      Dear Sir,

      From Jan 2015 onwards the monthly pension amount of Rs. 1941 against my above policy is not being credited in my HDFC bank Account. I assume this may be because of non submission of Life certificate to you or HDFC bank by me.

      I approached HDFC branch of Prahladnagar, Ahmedabad, Gujarat . and they made life certificate. Same is enclosed as original along with this letter.

      You are requested to credit my balance annuity from Feb 2015 onwards in my HDFC account and also update my correspondence address. I am enclosing my Adhaar card copy as a address proof for my correspondence address and my passport copy as a proof of my permanent address.

          I have been issued with policy number aps-nm-219672-l dated 16 sep 1998 with monthly premium of rs 395 which has been paid regularly till 31 dec 2015 from defence salary till 31 mar 2011 and then after retirement at head post office sikar till 31 dec 2015. Now the same is not being deposited by them stating that the same in not registered online. You are requested to get it registered online at the earliest please.

                    3. The acknowledgement of this notice must have been received by the Division/Branch, upon which they have keyed in the "notice received date" into the system. This is nothing but the date on which party received the notice sent to him. They have keyed in 01.12.2015 as notice received date into the system.

                    4. As per plan conditions, after 30 days from "notice received date", i.e. on 07.01.2016, Ulip AFM foreclosed the policy by treating it as 'Complete Withdrawal' case.

                    Thus, whatever happened has happened perfectly as per plan conditions and as per the systems duly in place. No further premiums can be accepted under the policy as it is in 'Complete Withdrawal' stage.

                                I have paid my policy no.[protected] for Rs .6142=00 through net banking on[protected]. But not received receipt till todate to my mail address . please send receipt to my mail address as given here under. [protected]@gmail.com This problem is every time I am facing after paying premiums through net banking. please persue the matter and do the needful to receive premium receipt of every transaction regularly without delay
