Dear Sir, I filed my claim for Rs 23521/0 yourself was pleased to pass it only for rs 20921/- after deducting a sum of Rs 2600/-. As per the policy in the case of hospitalization a sum of Rs 500/- is payable to the attendant. I have been admitted in the hospital for 4 days, i.e 9.6.15 to 12.6.15. As per the terms of the policy we are entitled for a sum of Rs 2000/- as attendant allowance. Keeping in view of the above, I request you to reconsider my claim form and pay me the balance amount, or provide me the reason why you have deducted other wise I have to file the case in the consumer court. Thanks Poonam Jain [protected]

I, the undersigned, mr. Karan pratapsinh bhosle, (Mobile - [protected]) i purchased a tata dumper (Tipper) l. P. K. 2518, registration no mh08h2528 on 19 / 10 / 2013. I got the complete insurance of this vehicle covered by oriental insurance co. Ltd. Zadgaon branch, ratnagiri. This vehicle was stolen on 18 / 04 / 2014 from padve, tal. Rajapur, dist ratnagiri (Maharashtra) . The incident occurred within less than six months from the date of purchase. We filed a written complaint (F. I. R.) about the theft in the morning of 19 / 04 / 2014 at the sagari police station, nate and also intimated oriental insurance co. Ltd. And bank of maharashtra (Financier) by telephone and also by e - mail. In addition to that i personally visited the office oriental insurance co. Ltd. And also informed them in person on 21 / 04 / 2015. At that time we were given a list of documents required for the filing of the insurance claim by oriental insurance co. Ltd. I started to submit the documents as per the list given to me. I kept all the papers ready except the ' a ' summary (A permission from the rajapur court to stop the investigation) in august 2014. Then i submitted all the documents to oriental insurance co. Ltd. In september 2014 and asked them to check if all the documents required for passing of the claim are furnished by me. Hounouring my request all the documents were checked. To pass the claim i was informed that the ‘ a ‘ summary (Order to stop the inquiry given by the court) in very essential & only on the submission of this document the said claim could be passed. Accordingly i started to follow up for getting the courts order but unfortunately i could not get the court order to stop the investigation (‘ a ‘ summary) for the next four to five months. But during this period (From the date of the theft, about nine to ten months i. E. April 2014 to march 2015) i was unable to pay the emi’s of the bank loan. Since the source of income was stolen i faced a heavy financial crisis and since i could not pay the loan emi’s even the bank started to demand the repayment of the loan amount. Under these circumstances i kept on following up with oriental insurance co. Ltd. And again submitted the set of documents except ‘ a ‘ summary (On 16 / 02 / 2105 and 10 / 03 / 2015) requesting them to settle the said claim. On both the occasions the set of documents were received by them demanding the court’s order to be submitted. Other than this i was not told to furnish any more documents on the above mentioned dates (On 16 / 02 / 2105 and 10 / 03 / 2015) . Later when i submitted the rajapur court’s order dated 30 / 03 / 2015 to stop the investigation along with the set of documents submitted earlier on 20 / 04 / 2015 to oriental insurance co. Ltd. Office and again requested them to settle the claim at the earliest. All the documents were again collected at this time & i was assured that the claim would be cleared shortly. No further action was taken on my claim documents for the next fifteen to twenty days. When i inquired about the said claim status to the concerned office & the branch manager of the insurance company i was told to furnish the license extract (Document) of the driver who was on the stolen vehicle. Since the lapse of 13 months (One year and one month since the vehicle was stolen) from the date of the theft i am not in contact with the then driver mr. Kasim ansari. Still i tried to get the extract from the hazaribaug, zarkhand rto office who informed me that the extract was not possible to be furnished in this manner. When i informed the oriental insurance co. Ltd. About the same they informed me that they will try to arrange the extract from the hazaribaug rto office and till date are been informed that still they are trying to arrange the extract for so many days. Hence frustrated i again went to the office of oriental insurance co. Ltd. Again for the follow up of the claim on 21 / 05 / 2015 and also informed them that i would be forced to approach the consumer court through a letter on which they informed that the extracts for the vehicle & the drivers license is essential without which the claim cannot be processed and if i approach the consumer court i will only be responsible for the delay or the outcome of the same. Submission of the original documents and the extract of the vehicle was possible for me which i had already submitted but i was only informed or demanded about the driver’s license & the extract till 11 / 05 / 2015.

This entire process is almost now completing 14 months. The vehicle was the only source of income for me & my family, which being stolen has created a very big financial crisis in my life. In addition to this the bank from which the vehicle loan was taken is continously following up with me for the repayment of the loan which was taken to purchase the vehicle.

In case of accidental insurance claim it is understandable to get the driver’s license along with its extract. But in case of theft of a parked vehicle is it really necessary for the driver’s license extract? If it is so important then why is it not informed to the claimant on the initial day itself?

All above events only conclude that the claimant is purposefully tortured mentally, physically and financially. Instead of asking for all the papers at one time why deliberately one paper is asked after the earlier submission and next paper asked after the one more submission just to pass time. In addition after the document is submitted the next flaw is only pointed out after 15 to 20 days and the delay in passing the claim is deliberately done by the insurance company. Hence i humbly request the concerned authorities to take a note of all above and take necessary action.

Today it almost 15 months. Please help to settle this issue. My mobile no. [protected]. My email id is [protected]@gmail.com

Dear Sir, Ref ID: CLAIM NO. [protected] / MEMBER ID:[protected] My name is Kamlesh Parekh having above policy, before 2 months i had filed reimbursement for Rs.32916/- as i was hospitalized from 17/04/2015 to 25/04/2015 at Gujarat Cancer & Research institute, Civil hospital, Ahmedabad for surgery affected by carcinoma. Yesterday i have recd a mail-letter from agent as stated that it was non payable - rejected, reason is clause 4.8 chewing of tobacco, betel nut as a habitant etc., pls. see below my reply to you for approval of said claim as genuine reason: My self wanted Raksha TPA to understand the background of my case that clearly indicates that i am not a user of tobacco however doctor with some misunderstandings mentioned tobacco usage in the consulting letter which we have cleared as untrue in every stage of the procedures. I would want to mention that i was just used simple pan normal ocassionaly (which is stated unharmful). With reference to clause no.4.8, you had rejected my mediclaim which do not apply to my case. Moreover, If tobacco did cause Ca, why there are so many people dead or alive who never had Ca after the use of tobacco? Whether tobacco is the only cause for Ca ? Why Ca is seen in people with no history o[censored]se of tobacco? In other words the other causes of Ca such as a 'Sharp tooth', Bad oral hygiene, heriditary factors and food items which can cause a cancer so much so the cause of cancer has not been fully established as yet. There are only some associations that's all. Tobacco chewing is not the only cause of cancer. Squamous cell Ca of tongue has been seen even in patients who never used any form of tobacco. What medical reasearch says is that there is association between any form of cancer and the use of tobacco so much so that a cause and effect relationship can be attributed to the use of tobacco and cancer. Coming to the law point, The date of operation is between 17/04/2015 and 25/04/2015. Now what is the contention of your responsibility, whether it says that there was no cancer at the time of issuing the policy and it appeared only after that date? I mean to say that cancer could have been in the initial stage even at the time of issuing the policy which no one can ascertain now by any stretch of imagination. What does the insurance company say about the smoking habit of its insured at the time of issuing the policy? Did it gave the policy to a person who is predisposed to cancer? Even in such situation the onus of proving the clause " use of tobacco has lead to this cancer in this particular person" squarely lies with the company which is highly impossible for them. In addition to, you can never reject 100% amount without consent of me as you can also welcome to my home for any verification, documents, meetings if any that you required further, my self is also and advocate of high court, pls. look into the matter seriously and process my claim asap otherwise i can also file serious case against your company for this reimbursement, compensation of harassment and 18 % interest value. Summarily, pls. look into this matter and do the needful as I had submitted only Rs. 32916 /- and it is not big amount for this type of disease, it is very very genuine amount to be passed.

Hi Team I have submitted a medical claim for my father’s Mr. Sutinder Pal Singh in Ivy Hospital Mohali. The claim number is 10756206. This was submitted on[protected] in Mediassist Mohali office. The bill of my father’s treatment is Rs 885133. I have claimed this amount in 2 policies: Plocy Num - [protected]P[protected] = Rs 214791 Plicy Num – 602200/46/14/[protected]_F = Rs 168176 But still as the bill is of quite big amount, I have put the remaining amount Rs 502166 for claim in the current policy number – 231290/48/2015/267. This claim has been submitted on[protected], but till now I have not received any further communication on this. On calling the Mediassist office in Noida I have come to know the previous claim number 10756206 is cancelled and new claim number 10766875 will be used to track this claim. The policy owner is my mother Mrs Inder Mohan Kaur, who is ex employee of Punjab National Bank. Oriental has a tie up with PNB and this policy was bought from the PNB branch. Still nobody is ready to respond to our queries. Request you to take the prompt action on this claim, and kindly pay the sum as requested. Other 2 claims have not taken more than a month to settle this and I am not sure why is it taking so long to settle this by your end. It has been 6 months and still no action has been happened on this, Please call me on the below number in case of any clarifications. Thanks in advance. Regards Simrat Singh [protected]

            Oriental insurance policy no;’ policy no : 131102 / 48 / 2015 / 4043

With reference to the above subject, i would like to file an official complaint regarding mediassist & oriental insurance co’ with respect to the below matter.

My uncle, sumanbhai g. Bhodia, a 73 year old, senior citizen, was feeling uncomfortable due to urine retention on sunday (14 - june’15) since 11 o’clock in the morning. Despite several remedies which didn’t work, we finally consulted a doctor at around 9. 45 pm as his pain was becoming unbearable – since his bladder was full. After detailed examination; doctor advised us to get admitted to a hospital for urine catheter. So without a delay, we immediately gathered his mediclaim related documents & called surana hospital, if they assist cashless treatment, since they agreed we admitted him at around 10. 30 pm.

By this time, his condition was getting worst – you can imagine the pain in his abdomen since his gallbladder was full from morning but he can’t urinate. This may have increased his blood pressure too – isn’t it normal to a person who is unable to urinate since morning to night – won’t be there some disturbance in his internal body structure? This can happen to anyone – including you & me. Hence the attending doctor mentioned complaints on admission as: urine retention; breathlessness with bp, severe pain in abdomen & general weakness & they started the treatment. Please note, diabetes, blood pressure, hypertension etc has neither been diagonised for him, nor he ever took any medicines for the same. Trust me, once the catheter was inserted; he was completely relieved & things became absolutely normal including the bp. However doctors need to find a reason behind urine retention in order to cure the same; hence they started medication as per the attending / visiting doctors; advised reports, sonography’s etc which may be a part of their treatment. Please note, neither the patient nor their relatives are ever informed about what reports / medications are been done / given; this happens everywhere.

On the other hand, tpa desk at the hospital was doing their task for getting an approval for cashless. However they request for last 5 years mediclaim policies copy. Since the uncle himself was admitted, it was difficult for us to search previous years copies
Hence i contacted the oritental insurance company & happened to speak with mr. Milind (Sr. Officer) to check if he can arrange copies of previous years which they normally have & can arrange very easily. However mr. Milind without even listening completely, wanted to divert the query to tpa – mediassist. He just kept on saying “you contact ms. Vaishali from mediassist, she will help you. . ” it clearly seems he is not interested to assist customers, then why is he working there? With this sort of attitude of simply diverting queries to another department without even listening or understanding whether the other dept’ could help the client or not.

            My question to oriental insurance:
            • how could ms. Vaishali from tpa arrange for previous years policy copies?
            • why is customers query, not even be heard completely?
            • why do we have officers like mr. Milind working for oriental if customer - service & satisfaction is not his motto?
            • does senior citizens, take policies for this harassment by not rendering assistance when needed?

Then i contacted ms. Vaishali from mediassist tpa. She said she can’t arrange policy copies – as i always knew.

Meanwhile mediassist had some queries which were responded by the hospital.

A day later, we were shocked when we got a know from the hospital that “cashless was rejected by tpa stating it’s a fresh case & the waiting period is 2 years for hypertensions / bp / diabetes etc”

I contacted mediassist toll free no’ – who said it was denied due to above reason & if i need further clarification, i should contact mumbai office – hence i contacted ms. Vaishali again – who said “she has got an email from oriental insurance to deny the cashless / claim since it’s a new policy. ”

My question to her was “what relevance it has for treating urine retention whether a policy is new or old?”
But she still denied stating “since the patient has hypertension, he couldn’t avail treatments for 2 years which is their waiting period. ”
I tried to explain “the bp / hypertension may only be an incidental finding during admission / treatment however patient neither has any past history of it, nor takes any medicines for the same. The reason for admission was only & only urine retention; anyone’s b. P. Could go high if you are unable to urinate for around 12 hours – that too a senior citizen. ”

My uncle is a sr. Citizen, unmarried lives alone, retired from work & with no source of income. The only reason for getting admitted in surana hospital was because it was in the panel of hospital which assists cashless hospitalization, however the purpose seems to be defeated here since on the other hand, hospital was forcing my uncle to pay.

I spoke to them about the same & they put up the query once again with mediassist explaining that “patient was admitted due to urine retention & not hypertension”

Also to my upset none of the sr. Staff were available to speak from mediassist & oriental on friday, saturday & sunday. I made several calls but no response, or switched off. I was only be able to connect to their tollfree number who said they have no decision making power, i need to contact mumbai offices however ms. Vaishali, mr. Milind, mr. Abhimanyu – none of them answered calls.

Answers from mediassist callcentre “its again rejected saying - since hypertension has been diagnosed they won’t be paying a pie till 2 years for any treatment. ”

It clearly means they just want to see the cash with their comfort angle: hypertension (Reason for rejection) & doesn’t even are interested to understand the actual reason.

I then consulted dr. Aniruddh gokhale (Sr. Urologist) , dr. Vivek maste (Urologist) & dr. Sharad oza to check if hypertension can lead to urine retention? & trust me all 3 of them said “hypertension has not relevance to urine retention – it may only be an incidental finding during examination. High bp does occur during long urine rentention but once releived bp goes normal” which actually happened too.

Vaishali / smita from mediassist told me on saturday that they have a panel of in - house doctors who have checked the case & they have rejected the case, she couldn’t do anything in it.

            So my query is:
            • are the above doctors (Dr. Aniruddh gokhale, dr. Vivek maste & dr. Sharad oza) incorrect in their judgement that hypertension cannot be a reason for urine retention?

            • why was the request for cashless hospitalization rejected when the reason for admission (Urine retention) has no relevance to policy being old / new – it can happen to anyone including you & me – even on the day of buying the policy.

            • unlike kidney stone, hernia etc; urine retention isn’t a disease which one can hold for long – in order to get it treated once policy is generated.

            • does mediassist & oriental – jointly mean to say “if you buy a new policy & if you suffer from diabetes / bp / hypertension etc – you can’t claim any illness including accident, fever etc for the first 2 - 3 years (Waiting period) even if they have no relevance to each - other.

Please note, i’ve most of the call recordings which happened with ms. Vaishali, mediassist toll free nos, mr. Milind oriental, dr. Gokhale, dr. Maste & other related calls – hence mediassist won’t be able to deny that they rejected only & only due to hypertension which was incidental finding. Also, i’ve preserved supporting emails.

My uncle ideally enrolled for orirental mediclaim policy around 10 years back & all premiums were on time – however due to mishap from punjab national bank staff – last to last year his policy wasn’t generated & created a gap year. I’ll take that seperately with pnb. He holds a active policy in his current year since ocotober’14 vide policy no : 131102 / 48 / 2015 / 4043. To the best of my knowledge he has never claimed anything from the mediclaim & this was the first instance where we required cashless hospitalization but rejected.

Not only claim was rejected but services of mr. Milind / ms. Vaishali / tollfree no’ were not upto mark despite being into service industry. No timely reply on emails. No availability on phone. No listening to customers problems.

My uncle is still under serious mental trauma due to financial crisis created by mediassist tpa & oriental insurance co’. He was not discharged from hospital due to non - payment on one hand & tpa rejection claims on other hand without even knowing the root - cause of admission.
He cried every night after that. Who is responsible for the same?

I wish to take this incident to all consumer forum & courts to assist me in getting justice for my uncle. Will also be publishing it on mouthshut, facebook, twitter & other socializing platforms on the internet so that everyone can know about your pathetic services. Your motto seems to be very clear of only making money via premiums but denying claims without proper reason.

I have submitted the claim under the above policy on 26th Dec. 2014. FIVE MONTHS ARE OVER – STILL THEY ARE NOT SETTLING MY CLAIM. Since 1st week of March 2015, I am following with the TPA for settlement for my claim. They always said there is no requirement pending from our side. They are simply waiting for the Divisional Office to take necessary action. Initially they were telling that the DO Clarification for uninterrupted coverage is awaited. Claim will be settled only if the Divisional office gives the clarification regarding uninterrupted continuity of coverage. On 29th April we were told by TPA that the DO clarification for continuation is received but file is closed. So TPA have sent mail to Divisional office to permit TPA to reopen the file. Today more than five months are over from date of submission of claim, I am going from pillar to post but there is no hope of claim settlement. My neighbour who is from New India Assurance Co. Ltd. informed me to approach YOU and thus this letter. He said as per Law within 30 days I am entitled to receive and TPA is liable to settle my claim. If we are late in payment of premium by even ONE Day, They refuse to renew the insurance – if they renew they renew with break - interruption and now when we are asking for claim they are sending us from pillar to post. I demand delayed payment panelty from them as they have failed miserably in dealng with the claim and have harressed us. Everytime we approached them they were waiting for some or the other communication from division. !! Are there No Rules guiding the divisional office to reply within prescribed time ? I request you to please look in to the matter So atleast others will not suffer like me. Thanking you, Sincerely yours, SOHIL SHAH.

I am Hanumanthappa a car insurance holder with Oriental insurance company, I met with an accident on Mar 30 2015 and the same day i intimated to the Oriental hotline number and the insurance with all related documents was claimed on April 22 2015 and the same was confirmed from the insurance office authorized person. File # was provided as 1504/00176 and that's the end of it, till today i.e.June 05 2015 i haven't heard any information regarding this. In-between i had called the Oriental insurance asking for the status of my claim but always told that the concerned person is on leave / we will check and give a call back, i got frustrated and ask them to connect me to manager (Ragavendra) after holding my call for 15 mins he responded saying that i was on leave from past two days and i will look into your file and will do it when the time comes. Please don't entertain this kind of cheap insurance companies.

Dear Sir/Madam I wish to complain about my health insurance policy of policy no:- 272101/48/2016/241 that I purchased on April 2003. Insured's Name:- Mr.Deepak Bhalla Insured's Code:- 4487549 I am complaining because i was told that I will get the amount of my medi claim through insurance company or TPA. I was admitted twice in hospital . Once in april 2015 and then may 2015. Firstly i was told i can claim through cashless facility and need not to submit any amount which was later paid by us only to the hospital. I submitted my file to oriental insurance kalkaji office for my april 2015 claim with all the releveant documents. I was told that i will get the paymet within 2weeks. Which again was never done. My new policy started from 8th may 2015 for which i was told the same like before . It never got the approval from t.p.a as of gap of 8 days in the policy The gap was due to techincal issue in 2012 and was not told to us by the agent. The amount for april 2015 claim was Rs.47, 911. And new cycle policy claim for may 2015 was Rs.1, 00, 000. To resolve this problem I would like you to kindly provide us or the t.p.a break condonation letter. When I first learned of this problem, I contacted Mr.Gulshan Jurani who is our agent and who is of no use. He told us to contact insurance company development officer Mr.K.C.Pandey. I submitted my april 2015 claim file to him and i was assured of geeting tge payment within 2 weeks. Later on we were told that nothing could be done about my problem. I believe that this response is unfair. I would like a written statement explaining your company's position and what you will do about my complaint. I look forward to hearing from you as soon as possible to resolve this problem. If I do not hear from you within 5 days I will file complaints with the appropriate consumer agencies and consider my legal alternatives. I am enclosing copies of my receipt. I may be contacted at the above address and phone number. Sincerely, Dr.Deepak Bhalla Mob:- +91-[protected]

Dear Sir, Policy Number: 261600/48/2014/2598 Policy: PNB-ORIENTAL ROYAL MEDICLAIM POLICY Insured's Name: MRS. ISHWAR DEVI W/O SH. SANTLAL. Claim Number: 10942978 Claim Amount: Rs. 49964 My Mother was hospitalized on 26 Feb 2015 for female gentle prolapse and discharged on 03 March 2015 and I had submitted all the original medical bills and necessary supporting documents to the TPA for mediclaim. After submission, I got the Intimation of the refusal of the Claim citing some policy terms and conditions. This was not a planned surgery as we came to know about this unfortunate disease last month only and doctor advised for immediate surgery. At the time of the policy purchase, no one has told us such terms and conditions, instead we were told that all diseases will be covered. We are having medical cover for such unforeseen medical problems, if such problems are not covered then for what we are paying premium every year. Besides, my mother was insured under the Oriental Insurance family floater Policy No: 261600/48/2013/1599 in FY[protected] as well. After my marriage we decided to buy separate policy for them. I request you to review my mother case and consider it for your kind approval.

My son and I met with an accident on 05-05-15 and was admitted in Ruby Hall Clinir, Pune. The vehicle was sent to the showroon and they asked to submit the document. My son was driving and I was injured major. I was in the hospital and my son took my signature on claim forms. Instead of his name as driver he mentioned my name. At the time of filling the application at showroom there was no one to assist. The papers which were given to me were xerox copy and the information was clear. You can also check the documents. My licence is expired. I contacted Miss. Tanuja who is in your Wakdewadi, Pune office. She says that documents are submitted and we cannot get the claim. My son explained her but she is not co-operating. I have already paid a huge amount at Ruby Hall Clinic and I wont be able to pay money for vehicle. I have also pain money to the vehicle owner to whom I dashed my vehicle. Can someone help me in this difficult situation. Name: Kasim Bijapur Number: [protected] (Soheb) Reg. No: MH14 EJ 8626
