Policy issued by forging my signature

Dear Sir/Madam,

In the month of February 2009 I placed a request for the issue of Citibank Credit Card. The executive has collected my signatures on the credit card application form but for my surprise he also managed to sign up for the issue of insurance policy on my name. Redumption Payment not received in my Bank account for last one month

I am investing in B51 Birla Sun life Equity Fund-Growth fund by SIP. and sir i was given partially redumption request to the company on 9th December 2009. but till today i have not got my payment in my bank account. after calling in customer care company representative said that payment has been made on other bank account by Direct Credit.and after receiving last statement i had found that company has given wrong account number....

redemption of MF

Dear Sebi,
My self Rakhi Agrawal. I have purchased one MF with folio number [protected] with Birla sunlife mutual fund.
I have given the redemption request for above mentioned folio number on 21/11/09 to BSL thane branch. i requested them to send me a DD and do not credit the amount in my account as it was liened due to some dispute with the bank which is going on.

Fraud with the Policy

The executive from BSLI named Rajiv Saxena from Andheri office offered me Saral Jeevan Policy (Policy no.[protected] for Rs 37000 annual premium and assured me a total deduction of 9% on this annual premium.
