You have to give a photo and id proof copy. And you have to write in the back of id proof copy something like

Sir please block my mobile no :[protected] as it was lost(stolen). by your name and give it to Airtel office

My number is [protected].My phone is stolen in Rohini at 7:30 pm on 29th dec. 2008.
Block my Number so as to avoid the misuse of my number.

I have lost my HTC P3400i with aitel connection. I had asked them to trace the mobile but they refused to do so. so please take action against the airtel company.
