elector's name and father name is wrong mention in card

My name is Ajeesh Nair, and date of birth is[protected], as i filled the form same, but infront of that that i received the wrong election card, as in the card i got following detail

Name mentioned on card # Nair Azees
Actual name is # Ajeesh Nair

Date of Birth mentioned on card # 01/12/1981
Actual Date of Birth is # 05/12/1981

my sister election card receive but my document are not complit this say your officer please check my paper and give my election card beacuase both paper is same so why rejected my card
