Since 06/06/2012 I am trying to make a duplicate dairy for my PLI policy no MP118641. My bag was stolen on transit and i have also file a FIR in local police station. I have also given application on my local Post Office on 06/06/2012 at Runija . But till today I have not able to get my duplicate passbook and I am also unable to pay my policy premium which may lead to policy lapses. It also may break my life insurance. So I request you to please provide me a duplicate pass book to enabled me to deposit my premium at earlier as possible.

I have enrolled in Indian Air Force on 1992. There i have taken PLI for 20 yrs my proposal no 176215 dated 23 jun 92. My policy no is 492/ANM/9 dated jun 1992 That time my policy bearing Service No. 9215774-B which is now 746607-B SGT Vattem Anjaneya Prasad or V.A.Prasad. I had retired from the service on 31 january 2012 after completing my 20 yrs. Unfortunately, on Jun 2012 i lost my PLI maturity bond while travelling to my sisters house. I request your kind suggestion to how to apply for the duplicate copy of my maturity bond. As my PLI maturity date is on 05 Apr 2014. Kindly, guide me how to apply for the duplicate copy of PLI and where to apply. As, of now Iam staying at Hyderabad in my permanent house which address is mention below. we have sent all the documents (original policy documents, attested copy of death certificate, claim form, and two receipt books) to the ASST.DIRECTOR, PLI, OFFICE OF THE CHIEF POST MASTER GENERAL, Ahmedabad CIRCLE, Shahibaug,GUJARAT. On 17-May-2011 through Our Loacal development Officer Mr. G.D. Prajapat , Dist;Valsad,Navsari & Dang Post Office.

My mother In law Nilaben Anilbhai Chaudhari as the nominee for the above mentioned policiy.

I have to inform you that there is no action on these claim settlement process from the Department Of Post for the last 11 months & 21 days. And there is no information about the process of the claim settlement. So I request you to take immediate action to settle this case.

I shall be thankful if you kindly settle the above mentioned claims.

With due respect I state that I have submitted my claim for survival benefit of above cited policy document and pass book through proper channel on 26 March 2012 . It was due on 18 April 2012 but I have not received it till date. Even after making a number of telephonic calls to Worthy and other subordinates in your good office no progress has been achieved in this respect. Concern official at your office states that same has been despatched from there vide RH[protected] IN dated 18 April 2012 but same has not been delivered to me till date. Non delivered of same is causing directly a great financial loss to me.

I have taken your product PLI bearing No. HY-AEA-106977-CS Dated 20-Feb-2006. 20% amount of sum assured was due on 20-Feb-2012. I have applied for this amount through registered post from main post office rewari on[protected] and sent original Preium Receipt Book to Dy. Divisional Manager (PLI) Ambala Cantt., Haryana. but till date there is no information regarding this and now it is impossible to deposit pli-premiun without Original Premium Receipt Book and in due to failure in depositing premium your department will charge interest/penlity which is unjustice. so,please send my Original Premium Receipt Book after settelment of my money back amount to avoid unconvinced to me.

Dear Sir,
Till date after lapse of seven months i am waiting for my matured sum. My policy details are DL-22242-CC sum assured-50000.00, matured on July, 2011. I have submitted all my original documents to CHINSURAH PLI office, Dist.-HOOGHLY, W.B on dated 22.08.2011. Please advice me how I will get my money as early as possible. Further when I enqired what about my matured money, the PLI office, Chinsurah told me that the file sent head office KOLKATA for verification my policy from Master data (The file sent on dated 16.11.2011) but no positive result has come out when i approached to Chinsurah office. I therefore request you to solve my problem at the earliest possible. May I approach to RTI- then please send me the address please. ATANU DE.

I have surrendered my PLI policy after its maturity on 20.12.2011. I got the payment order dt. 25.1.2012 for an amout of Rs. 100700/- which was very much less than its maturity amount as per the calculations. My policy No. AP AEA 104808 B.So far I have sent two letters to them but no response so far. Please advise them to respond immediately without any further loss of time Policy amount face value is Rs.50000/- My contribution is 328 pm for 180 months
Rs.59040/- is my total contribution. This is a money back policy but no money has been sent by them during the currency of the policy. I am supposed to receive amount of my investment+ bonus+ maturity bonus + the amount which was not paid to be during the currency of the policy.(money back amount 3 times and interest there on.)+ interest on the above amount from the date of maturity till date of payment .
