Dear sir, for your information sivasagar dist (Assam-785640) is one of the newly covering area under esic. My esic insirance no is: [protected]
My problem is we have not any esic dr or dispensary in our location.in first few month we got a dr for consult. But now he is decline to treat under esic. As he said he is not paid by esic & even have not any commitment/ agreement with esic. Now we have to run jorhat dist (About 75 km which needs journey of 2.3 hr by bus) to avail the esic facility, though we are paid for esic facility from our salary.
We are employee in a private company and could not avail frequent leave only to get the esic facility.
Kindly look into the matter and hopefully waiting for your positive responce.

Esi hospital basi dara delhi all patients toilet is very bad conditions but i was surprised when i see the esi's staff toilet room is good conditions and he dos't go any patients in his toilet why are doing this in esi hospital tell me... Government is make in clean india all is fake... Commitment... If you want to do treatment in esi doesn't go plz very bad conditions here.

I had to immediately admit my father in private hospital, so i thought will make use of esi benefits. I took my ill father to esi hospital to ask about the esi procedure, but the sisters at helpdesk turned out to be very rude and started screaming at top of their voice instead of guiding us. First of all the money is getting deducted from our hard earned salary and esi employees turn are behaving like this. After explaining my father's condition, one of the nurse agreed to give live list and asked me to consult a doctor sitting in room1 of esi hospital who was one more headache to deal with. Being a doctor he forcefully started examining my father, inspite of telling him that we already have a family doctor in aj hosital. He dint let us go easily, my father was in a very bad state. When we told him not to force us to get admitted here, he said i cant sign on ur esi form. We walked off from esi hospital, my dad's condition was worsening, they dragged for more than 2 hours. We straight away went to aj hospital, somehow managed to pay advance amount and got him admitted. After one week he was discharged form hospital, now i thought i will atleast make a esi claim, but again esi people made me run around from esi hospital to esi dispensary, karangalpady. Lack of responsibility, officials are not helping us. This is the 6th time am running to esi dispensary, the officials are mean, help desk is of no use. The esi sisters and doctor kept on saying straight away that my claim request will rejected. Because you have taken the patient to private hospital instead of government hospital. If esi officials behave this way, noone will come forward to utilize esi benefits. Every month esic amount is getting deducted from my salary, then why i should not get the reimbusement of my bills?
Why cant you make it optional, instead of compulsarily deducting esi amount from our salary. I have spent a lot on travel expense because of these irresponsible esi employees. One thing they should keep in mind is, we are claiming for our earned money not begging for theirs.

3. Reduce the no. Of documents, when we are technically forward now. Why cant we make an option to submit documents online.
4. They require rs 20/- stamp affidavit, again they make us spend on notary.
5. I can see lot of corruption
6. We being in it field work from (9 am to 7pm, with half an hour lunch break) but the esi officials have a flexible time zone, still they act so rude.
7. Please display the rules and other details in english language as well. The prints are not clear and helpdesk is for namesake.
8. Last request make esi optional, waste of time and money. Deducting from our salary and making our life miserable is just no good.

मेरे father ig esi हॉस्पिटल में admit हैं काफ़ी साल के बाद esi हॉस्पिटल jhilmil shahdra delhi से वास्ता पड़ा डॉक्टर और नर्सिंग स्टाफ़ का स्वभाव बहुत बदला हुआ यानी अच्छा था
साफ़ सफ़ाई बहुत अच्छी थी मगर काफ़ी कमियाँ ऐसी हैं जिनको देखकर हैरान होने वाली बात हे इन कमी की वजह से साफ़ पता चलता हे की भ्रष्टाचार अभी भी बहुत बाक़ी हे और ये लापरवाही इतनी बड़ी हे जो साबित करती हे की ऊपर तक चोर बैठे हैं
१. सबसे पहले मैन एंटेरेंस की सड़क जो के वार्ड पार्किंग तक जाती हैउसमें बहुत ज़्यादा गड्ढे हैं जिस मरीज़ के चोट लगी होगी या हड्डी टूटी होगी उसका क्या हाल होता हे ये तो मरीज़ या उसके रिश्तेदारही जानते हैं
२. एमेरजेंसी वार्ड के गार्ड बार बार (एक मरीज़ के साथ एक जन ही रुकेगा बाक़ी बाहर) इतनी ज़ोर से चिल्लाते हैं कि दिल के मरीज़ बच्चे और महिलाएँ तो बुरी तरह घबरा जातीहैं हॉस्पिटल ना हो कर जेसे कोई police स्टेशन हे ।
३. टॉलेट साफ़ होते हैं मगर सिर्फ़ सुबह और कोई भी फ़्लश काम करता हुआ नहीं मिलेगा ना ही कोई पेशाब करने का पॉट में पानी चल रहा होगा और आधे से ज़्यादा तो पॉट हे ही नहीं
४. मरीज़ों के लिए एक पैंट्री बनाई हे जिसमें मात्र एक सिंक नाम की सुविधा हे मगर उस सिंक को सफ़ाई कर्मचारी भी पोंछा धोने के लिए इस्तेमाल करते हैं और कचरा उठाने की गाड़ी भी उसी पैंट्री में खड़ी करते हैं
४. बेड की चादर रोज़ाना धुल कर आती हे मगर एसी होती हे की आप में से कोई भी अपने घर में इस्तेमाल नहीं कर सकता कम्बल तो शायद ही कभी धुलते होंगे । और पिल्लो यानी तकिया नाम की तो कोई चीज़ ही नहीं होती।
५. सुबह चाय आती है जो के railway station से भी बुरी होती हे उसके बाद नाश्ता आता है पानी वाला दूध ब्रेड ५ ग्राम मक्खन जो के एक स्लाइस पर भी नहीं लग पाता फिर दो टाइम खाना उसके बारे में क्या बताऊँ क्योंकि वो तो पहले ही मरीज़ों का खाना होता हे जिसे मरीज़ भी ठीक से नहीं खा पाता । कहने वाली बात ये हे की खाना बाटने वाले का बर्ताव ऐसा होता हे जेसे अपने पैसे लगा कर उससे कोई बँटवा रहा हे ।
६. अगर आपका कोई टेस्ट बाहर होना हे तो समझ लो गई कई दिन के लिए बात करते रहो इंतेज़ार और लगते रहो चक्कर ।
७. अगर आपको किसी प्राईवेट अस्पताल में रेफ़र कर दिया तो पहले आप लम्बे बिल भरो और फिर लगाओ चक्कर और लाइनें पैसा वापस लेने के लिए
८. अगर आप अपना x-ray लेने जाते हैं तो आपको x-ray बिना लिफ़ाफ़े के ही पकड़ा दिया जयगा।
आपको आश्चर्य तब होगा जब आपकी ख़ून की जाँच की रिपोर्ट किसी और की रिपोर्ट के पीछे प्रिंट हुई मिलेगी शायद काग़ज़ की बचत।
...ये तो ८ दिन का experiance हे अभी बहुत कुछ बाक़ी हे ।वैसे तो मिनिस्ट्री के अधिकारियों का हॉस्पिटल में दौरा लगता रहता होगा मगर फिर भी अगर कोई अधिकारी इस तरफ़ ज़रा सा ध्यान दे दें तो श्याद ये हालत सुधार जायें।

Worst behaviour from staff.. Worst advice from doctor.. He doesn't hear our voice... He doing his personal work.. Waiting time more than one hour.. Worst treatment.. My mother have heave throat pain and headache.. But he gave paracetamol tablet only.. It's available at medical 2rs.. Why we are waiting in the hospital more than one hour.. Doctor's not give correct advice.. Useless.. Please take an immediate action for this despensery.. Thanking you

I am nirbhay from yamuna nagar. Recently my relative name vinod goel from panipat met with an accident on 22/3/2018 at gohana highway and he has been shifted to park hospital, panipat.
But sir, doctor does not provide correct guidance of patient to the family members.
We thought that the patient has been dead from last 3 days but the hospital may make misuse of esic money.
I also mention that i am not sure but one internal person tell this to me secretly not mention his/her details.
So i want that one of your professional team and staff should visit in that hospital for a survey to check that patient as he is alive or dead and the treatment given to him also the use and misuse of esic money so as to provide us correct information.

One of our school (The gaudium international school - kollur, telangana) drivers was bitten by a snake today morning on the campus. He was rushed to the nearby esi hospital in patanchru since he is a covered employee. After doing first aid, doctors said the condition is serious and advised us to take him to a specialty hospital on our own since the ip is covered under esi from last may only. They said it is as per latest guidelines and they will not give anything in writing. We have admitted the ip to a nearby specialty hospital on our own. Doctors said the condition is serious and only after 6 to 8 hours, they can confirm about anything.
Snakebite is an accident and what sin we have done by covering our employees sincerely under esi as per law? What are we supposed to do now? Who has to bear the treatment expenses? God forbid if something untoward happens, what happens to his family?

I admitted my father in private hospital due to urgency, now i want reimbursement of my bills from esic department of nagpur, local dispensary doctor stright away saying that i will reject your file once it comes to me. Because you have taken the patient in too far distance private hospital and not taken to government hospital. Even the distance of government hospital from my home is too far then the private hospital. I have major that also. Every month esic amount is getting deducted from my salary, then why i should not get the reimbusement of my bills.

Dear sir/madam i bring to your notice that the staff of rama chandra puram esi hospital is doing so late to issue the referral form and other forms and certificates there is a good process of work but the staff is delaying the work carring their work leisurely as they like they should think about the requirements and the need of the patients...
Concern authorities please look into the matter and try to solve the small issues due to that so many patients are suffering...

1.My mother facing LS SPINE problem(L4 and L5) at last one year. She cannot walk as well.
2.Suggestion was taken from Dr soumyajit Basu. He is a spine specialist and one of the best doctor inproblema. He suggest of my mother for surgery.

4. We visit at maniktala ESIC hospital of twice time but they are not taken seriously about this matter. Only medicine suggest.
5. Last one year lot of medicine adhere by my mother but nothing was happened.
6. We cannot travels day by day with my mother. She facing more pain and cannot walk as well.
7.so I request to you to solve my problem. The matter is serious.

Good morning, i am dinakaran form tambaram, last monday i am going to west tambaram esic hospital branch for a treatment for me (Heavy fever & clod) at first time. One old man (He is esic employee) asking me are you coming first time in this hospital ok please come give me your esic card and please pay 500 cash and please buy one 300 page note book. I am going to buying the book and handover to that old man. The old writing a book for my personal details.
I am going to meet doctor and doctor ask me what a problem i am telling my illnesses and suddenly asked doctor why your mention one day treatment only, now i am talk to madam i am not mention word. Suddenly i go to ask that old man why your writing the note book for one day treatment, that old man ok ask me your coming first time and also you can't received hr sign form 2 so i mention the note book for one day medical only and please pay 500 cash to him. Then i paid rs: 300/- only to him. I think this amount for bribes. So please take correct action to that old man and hospital employees.

My father is enrolled to esi and every time he goes to a esi center, whichever medicine he asks is said to be outof stock and is they ask him buy from store outside.

Basic medicine is out of stock for months.. Kindly consider this

He usually gets medicine from esi center at baikampady, if medicine is out of stock he is sent to esi center at kadri mangalore... Each and every time he goes to both of center has same reason to give 'out of stock'

My father has applied all the documents but his salary is still pending from esic end.
Firstly esic was saying that his documents don't match the name so he reapplied with the proper name but still my father is facing the issue as the salary is not being forwarded from esic end.
What is the use of your services if it doesn't work on time. Please do have a look into this case.

I'm a resident of balamrai located in secunderabad, telangana. I would like to get your notice that regimental bazar dispensary is shifted from regimental bazar to balamrai locality in a residence in between all the residences and it's become a problem to all people residing in that area as there are many small children. Many ill and sick patients come daily spoil or surroundings by spitting, vomit and many more like these.
The owner of that house rented to esic dispensary for sake of money. It's not advisable to run a dispensary in a residential area. So on behalf of residents in that locality i request you to kindly shift the dispensary to another place from there at the earliest.

My name nisha.. My delivery date was on 3rd october 2017.. After two and half month of delivery i received salary from esi.. I hv submitted all the forms as given by esi office people in time.. Now its two more month passed still salary is pending.. What's the. Use of giving maternity benefit if it is not given in time as the expenditure will be more in postnatal period..
