1. I am connected to your broad band since the last 5 years. My Phone number is :[protected] at my residence Address : 141-A Sainik Nagar Rae Bareli Road Lucknow PIN - 226029.

2. Now, due to financial reasons, I request to disconnect my broad band (Internet and Phone both) with immediate effect.

3. Final Bill may please be prepared upto this date and adjusted out of security deposit. I shall pay if anything outstanding. Any amout of refund from BSNL, may please be refunded to me by Cheque.
