Hello,

I forgot my user id & password for policy no[protected], because of that I am unable to pay my premium online. My annual premium is 3596 and DOB - 02/01/1979. Kindly do the needful to get my user id & passwoord as earlier as possible.

I have a policy No.[protected] with LIC Kanpur office and have sent two reminders for change of address to
128, AVTAR ENCLAVE, PASCHIM VIHAR, NEW DELHI-110063
and send me the option form to get first pension instalment which is falling due on 28.04.2011 but there is no response till date. Kindly respond and do the needful. Thanking you- VINAY ARYA
