Lic Of India — non-remittance of fund in money back-policy no[protected]

dear sir/madam,
With due respect & humble, i am to inform you that i have policy no[protected] in name of Dharashree Nayak.The money back amount has not been received in a/c no-[protected](in name of Dharashree Nayak, Ifsc code-ORBC0101386, Hassanpur)despite repeated telephone & fax to(Mandate also faxed) you & your Agent(Uttara Nayak) .Although More than 3 months has been passed.there is no response from your side as well as from your agent .Therefore, you are requested to remit the fund with interest at the earliest.
