Dear Sir/Madam,
Sub:-Non-updation of Policy premiums at your end, already deducted from Salary and
paid through Treasury.

I am thankful to receive the cheque no.30215 of Federal Bank Ltd, dated 17/03/09, for Rs.3288/- being the refund of premiums remitted through salary deduction for February 2008 to April 2008 in respect of my Policies bearing nos.[protected] &[protected].
It is very clear from the above refund that the premiums were received at your end and were not updated by you, in my policy account, in due course.
Due to this non-updation in time, I had to pay directly at your Vaikom Branch, an enhanced premium of Rs.575/- each for the above two policies for three months amounting to Rs.3450/- for keeping them live, as per the following details:

So, please not that I was bound to pay an additional amount of Rs.162/-, in total, for a mistake
committed by somebody at your end.
I was in service of Govt Dept up to 30/04/2008 and hence the premiums remitted through Salary Deduction should be taken as such till that date.
At this juncture, I would like to point out that you have committed a couple of mistakes and it is not fair on the part of LIC to refund the regular premiums instead of refunding the enhanced premiums remitted thereafter.
Hence, I would request you to either refund the excess amount of Rs.162/- or adjust the same in the future installments.

As desired by your goodself I have sent the existence certificate under annuity Policy No.[protected] through Speed post (Photocopy of receipt enclosed) on dated:[protected] but till date I have not received the cheques of next annuity i.e for the year[protected].
However I am again sending the existence certificate for your information & record.
Therefore your goodself is requested kindly to scrutinize the lapse & release the amount of my next annuity which is due for this financial year to avoid harassment to my self. Hoping that you may release the amount of my next annuity without any reminder in this regard please.
