The applicant most humbly submits as under.

That, the father-in-law by name late Sidram S/o late Manikappa and mother-in-law Smt. Late Leelavathi w/o late Sidram of my daughter late Archana, son-in-law late Santosh Kumar s/o Late Sidram of the applicant, daughter late Archana w/o late Santosh Kumar of applicant and maternal granddaughter late Lakshmi d/o late Santosh Kumar of applicant, have expired in the motor vehicle accident at near Bhagyanagar Camp, Koluru Dist. Bellary (Total 5 survival members of the maternal grandson of applicant), only the maternal grandson of the applicant i.e. Taneshkumar is alive in the said accident. The applicant as a maternal grandfather of the said Taneshkumar age about 3 ½ years under care, protection and under the guardianship of the applicant and he is residing with the applicant residential at Village Mudbi, Tq.Basavakalyan, Dist. Bidar-585437.At the time of accident the above said deceased persons was resided at DRDO Township Phase-1, near CGHS, C.V.Raman Nagar, Bangalore as per their Aadhar card as the deceased Sidram was worked & got retired at DRDO Bangalore.
That, there is a LIC policy no[protected] in the name of deceased Sidram & nominee in the name of his wife Smt. late Leelavathi and the said Taneshkumar the maternal grandson of the applicant is entitle to transfer the said LIC policy accident death claim settlement amount in his name through the applicant as guardian, for which applicant as a guardian of the said Taneshkumar who is minor under care, protection and guardianship of the applicant and he is only the legal heir and successor of his grandfather deceased Sidram. Therefore it is requested that accident death settlement of deceased Sidram in the name of said Taneshkumar under guardianship of the applicant may kindly be approve as grandson of deceased Sidram & maternal grandson of applicant is under minor. I badly need of money for my maternal grandson (Taneshkumar) education & other expenses.
For your kind information I had written a letter twice on[protected] &[protected] along with all required original documents and also as per your letter dated[protected] I had submitted all required documents in original but still we have not yet received the accident death settlement of deceased Sidrams. Kindly look into the matter so that it will helps us for my maternal grandson education purpose as badly need of money for my maternal grandson Taneshkumar. And applicant undertake that he will held responsible for any litigation or any objection.

I am applying applications for home loans though Lic person Rajeev kumar Vyas cheated on the pretext of getting a home loan and Earn 1025 rupee through check, and process fee by phone pay 9000 RS, 6 months is complete but my home loan not process, i have a check no.                 LIC Housing Finance not returning your Original property papers

                We had transferred our home loan from LIC Housing Finance to SBI on 18th Feb 2020. We were asked by LICHFL to collect the documents after 20 days. In March 2020 we were told that they will call us back once the documents are dispatched from their Mhape office to the front office of Kharghar from where we have taken the Loan.

                With due respect I beg to state that I am a policy holder (jeevan Saral policy) of LIC, belonged to servicing Branch: Berhampur, Murshidabad, Pin: 742101, W. B. My policy number:[protected], Date of Commencement: 28.03.2010, Date of Maturity: 28.03.2025, proposal number: 48685 date: 28.03.2010, Table and Term : 165
                15. These are true and printed on the policy Bond.

                The policy has been continuing regularly from 2010 (Two thousand ten). But after some years it came to my notice that there was a mistake in typing my Date of Birth on the Policy Bond. My Date of Birth is 20.06.1966. But it had been printed as 26.06.1966. Then I went to Branch Office and made it correct by Branch Manager (I /C) on 24.05.2019. Unconsciousnessly at that time I did not come to know that the Date of Commencement and Date of Maturity had been changed.

                Later a letter printed on 24.08.2020 by P. Sr. Divisional manager (Ref:CRM PS/2020-21/D[protected] came to my hand. On that letter he wrote, “This refers to the above mentioned jeevan Saral policy issued by us. It has come to our notice that in your above policy, there has been a typographical error while printing the schedule in policy Document. Accordingly, we give correct schedule in respect of above mentioned policy .”

                In this way the schedule has been changed and the Date of Commencement has become 28.03.2018, Date of Maturity has become 28.03.2033., Sir/Madam, kindly note that: - -
                (i ) Possibly at the time of correction of Date of Birth the above mistake may be.

                (ii) You are requested to see the premium payment voucher on 26.03.2019 (Twenty six days of March, Two thousand nineteen) in which it is mentioned clearly the Date of Commencement: 28.03.2010.

                (iii) It is mentionable that I have taken a loan of Rupees 27000.00 (Twenty seven thousand ) on 11.01.2014 (Two thousand fourteen) on this Policy and that loan had been repaid properly within 2015 (Two thousand fifteen). All payment vouchers in my hand. So the policy be started at 2018, ie Date of Commencement: 28.03.2018, how is it possible?

                (iv) Moreover the next time on 2018 again I decided to take a loan on this policy[protected] and took a printed statement from the Branch Office. On that statement there is Gross loan payable amount of Rs. 76729.00 (Seventy six thousand seven hundred twenty nine ). Though I did not take the loan due to need comes to end. But the statement copy is in my hand. If I am wrong, how is it possible?

                (v) All loan repaid vouchers from 2014 and premium payment vouchers from 2010 are in my hand. If I am wrong, how is it possible?

                (*)In spite of these without checking how did The Divisional Manager change the schedule?

                (*) Probably the “Table:165” has been done closed from 2013. Notwithstanding if I suppose myself wrong, is it possible to be the Commencement Date: 28.03.32018 ?

                (*) Meanwhile about this a complaint has been done in “consumer forum”. And also a complain has been done in “grievance, L I C, Mumbai Head Office” on 27.10.2020, Complaint Number: [protected]. About this they have informed Divisional Manager, kolkata - 64. Now again a complain is done for solution.

                (**) It is very very important that Branch Office of Berhampur, Murshidabad, W. B. And Branch Manager of Berhampur, Murshidabad, (W. B) personally have given all informations (datas) to Divisional Manager again and again.But The Divisional Manager has been maintaining silent role. Why?

                Sir/Madam, you are requested to check all about my Policy Schedule unkindly bring it back in correct condition.

                              I am one of the consumers of LIC India. Please see the Attached ECS confirmation letters from LIC, these letters are issued at the time of issuing the LIC policies and I wanted to make sure that both the policies (Policy no[protected] &[protected] are having the Auto debit ECS facility to deduct the annual premium. I had given details of my savings salary account to make sure, I am maintaining proper balance for any ECS deductions, even now it has enough balance to deduct the ECS altogether.

                              However, I have noticed that the premium was not debited by the LIC and No Annual premium deduction was made from my account. The policy is on Hold/Inactive - now.

                              Therefore, I requested to Reinstate/Recommence my policies without any penalties. It was clear negligence from the LIC department itself - either it is the fault of the policy issuing sales representative or the ECS department/Administration department for not taking follow up on the same.
                              Just for your information, I issued one more policy from LIC in Jan-2020, and medical was also conducted - the dept. could have taken the Medical report as a reference from Policy no[protected] to reinstate, but after reporting the LIC branch - the first reply was to sent the bank statement to check enough balance was maintained or not!

                              And after submission of the Statement - there was no reply at all till December-2020 due to their own mistake. It is more than 6 months and the LIC branch is asking to submit forms or new medical certificate even after knowing people can't travel due to corona.

                              I can understand if it is one month or two month's delay for not addressing the issue, but it is almost two years and nobody has observed/inform regarding such a problem of ECS payment from the LIC department.

                              Kindly do the needful to reinstate the policies.
