I have mentioned all the things point wise below:
1. On 14th October 2008, Mrs. Punam Asthana took the Reliance Health Policy. Policy Number - [protected] by paying Premium of Rs. 2,115=00. Policy Plan – Silver. Policy coverage of Rs. 2 lacs only.
2. The last date for renewal of Policy was 13th October 2009. There was a 15 days grace period also.
3. Policy was taken in Gurgaon. Presently, policy holder is in Muzaffarpur, Bihar.
4. On 11th October 2009, Mr. Sitesh Rajan, S/o Mrs. Punam Asthana, made a call to Medi Assit India Pvt. Limited on its toll free number –[protected]. Customer care agent gave the toll free number of Reliance General Insurance Company Limited’s Customer Care. (Customer Care No.[protected]. A request for renewal of Policy was submitted, Request No. 25950.
5. Mr. Rajan received a call from Reliance and agent advised for renewal of policy through Credit card. As the customer didn’t have credit card, agent told that he will forward the request to concerned department for cheque pick-up.
6. Again on 13th October 2009, Mr. Rajan made a call to Reliance customer care to enquire about the development. Executive assured for a quick response in this regard. Agent also told that there is no office of Reliance General Insurance in Muzaffarpur, Bihar. On request, executive gave the address of Patna branch of Reliance General Insurance. Customer care executive told the customer that he is re-forwarding the request and customer will receive a call from Reliance’s side soon.
7. In the meantime, Mr. Rajan went to the office of Reliance Life Insurance Co. Limited, Near M.D.D.M. College, Muzaffarpur. Executive told that there is no office of Reliance General Insurance in Muzaffarpur.
8. Till 28th October 09, there was a no response from Reliance General Insurance. On 29th October 09, customer again made to call to Reliance customer care. Lodged complain regarding this issue. Complain number – 701967. Agent advised to wait for 72 hours. Agent gave the address of Reliance General Insurance, Gurgaon office as policy was issued by Gurgaon office.
9. No response till 3rd November 2009. On 4th November 2009 made a call to Gurgaon office of Reliance General Insurance. Unable to talk. Then, made to call to Customer care of Reliance. Executive told that he will re-forward complain and advised to wait.
10. After few days, received a call from Reliance. Executive confirmed address and told that executive will visit for cheque pick-up in two days.
11. In the meantime, on 11th November 2009, sent a letter to the Gurgaon office of Reliance General Insurance. In the letter mentioned all the details and requested for a quick response on this matter.
12. Again on 12th November 2009, made a call to Customer care of Reliance. Agent assured a call back from concerned department today itself.
13. On 12th November 2009, received a call from Mr. Brijendra Kumar, SM, Reliance General Insurance from [protected]. As per Mr. Kumar, he received a mail from Mumbai office today itself. Told him that we made a request on 11th October itself. Mr. Kumar told that he will talk to Mumbai office/customer care and enquire why it took 1 month to process the request. Mr. Kumar told he will call back later.
14. On 12th November 2009, received a call from Mumbai office of Reliance. (Number[protected]. Executive told that policy can not be renewed. Told him all details. In reply to this executive said that he will look out for possible ways and will get back in a day or two.
15. On 12th November 2009, in evening, made a call to Mr. Brijendra Kumar. Mr. Kumar in meeting, told to call in 1 hr.
16. On 12th November 2009, received a call from Mr. Vivek Jha (No.- [protected]). Agent confirmed address and came to home. Agent said we have to take a new policy. Previous policy can’t be renewed. Agent also told that there is a office of Reliance General Insurance at Bhagwanpur Chowk, Muzaffarpur.
These are the sequences. Kindly look in the matter and do the needful.

I BRING TO YOUR KIND NOTICE THE FOLLOWING FACTS FOR YOUR CONSIDERATION AND IMMEDIATE ACTION.I RECEIVED A PHONE CALL FROM YOUR ORGANISATION ANDROMEDA MARKETING PVT LTD GUINDY CHENNAI ON 26TH MARCH 2008 REGARDING RELIANCE GENERAL INSURANCE IN THE MONTH OF MARCH 2008. I WAS ASKED TO AVAIL MYSELF THE POLICY AMOUNT OF RS 15000 FROM RELIANCE GENERAL INSURANCE. THE HSBC BANK REMITTED AS RS-15000 IN THE MONTH OF MARCH 2008 TO RELIANCE GENERAL INSURANCE WHICH WAS MENTIONED IN HSBC BANK MARCH 2008 STATEMENT TO MARCH 2009 RS -1380 AS EMI DEBITTED IN MY HSBC BANK CREDIT CARD STATEMENT. I HAVE BEEN PAYING RS- 1380 AS EMI EVERY MONTH TO HSBC FROM MARCH 2008 TO MARCH 2009.
BUT SO FAR NEITHER I RECEIVED THE POLICY DOCUMENT NOR REMITTANCE RECEIPT NOR ANY OTHER INFORMATION FROM YOUR ORGANISATION. THIS CAUSED ME HEAVY FINANCIAL LOSSES LIKE NOT CLAIMING INCOME TAX BENEFIT AND MEDICAL BENEFIT, REIMBURSEMENT BENEFIT AND ALSO I SUFFERED MENTAL AGONY. HENCE I REQUEST YOU TO COMPENSATE ABOVE SAID FINANCIAL LOSSES AND ALSO SEND ME THE POLICY AMOUNT WITH INTEREST AT THE EARLIEST. ALREADY I HAVE SENT A REPRESENTATION REGARDING THIS MATTER.THERE WAS NO REPLY FROM AMDROMEDA CHENNAI. SO I REQUEST YOU TO TAKE IMMEDIATE ACTION.
