total 5 transaction were made of Rs.79,777.04Rs at France through my credit card number [protected] on 28/12/2012 and i had immediately reported that to SBI helpline and blocked the card and even i tried to registered compliant to CYBER POLICE STATION BKC bandra. Mumbai. But BKC police has just taken the application and forwarded it to Samta Nagar Police Station. I have been following up with the SBI for the same but they are saying...

I forgot my online sbi banking username & password by mistake
there why i can't use my onlinesbi banking services.
If any way is there to recover my user name & password then plz help me,

I am using above SBI Credit Card from last nine years.
In this regard it is requested to kindly waive off the annual & extra card fee, which is already paid by me on 29.12.2012.
I had never paid the annual fee for the credit card in last nine years.
In case my request for waiving of the renewal fee is not agreed you may discontinue the services of the card.

I have used SBI Credit Card in year 2010 and cleared most of the due amount but Rs 3,713 was left unpaid because these were the late payment charges and Finance Charges levied on total outstanding due to the non payment of minimum amount for consecutive 2 months.
When I requested them to remove these extra charges from my card, they declined even though I paid the rest of the amount due.

On 31st december 2012 sbi charged me rs 1470 the description saying fee excess drs. I M 100 % sure I havent withdrawn any excess amount other than my balance anytime in the past.
