I am a retired State Bank Employee and member of Retired Employees Medical Benefit Trust under Retired Employees Medical Benefit Scheme and having Insurance Policy No - [protected]P[protected] Policy B ) for sum insured Rs.3.00 lacs with UHID No-UIIC [protected] for Sri Manik Lal Ghosh (myself) and [protected] for Sm. Bani Ghosh (wife)

I have submitted 2(two) Medical Bills, one for claim amount of Rs.5672.50 for myself and other one for claim amount of Rs.2991.50 for Bani Ghosh ( wife ) on 08.11.2017 over the counter of person, name, Oindrila at the office of Insurance Company at State Bank of India, Administrative Office, Bidhannagar, Kankurgachi, Kolkata after approving the Bills from my pension paying Branch, Calcutta Airport Branch of State Bank of India.

Among these 2(two) Bills, only Bills for Claim amount Rs.5672.50 for myself has been paid and after deduction Rs.3947.00 credited to my account on 16.11.2017 but other Bill for my wife has not been paid. Both the Bills for Rs.5672.50 & Rs.2991.50 have been submitted at a time with one bunch, keeping my Bills at top & wife's Bill at the bottom.

I have enquired at Toll Free No, they said only one Bill for claim amount of Rs. 5672.50 has been found & that has been paid and they have not received any other Bill.

I have contacted at the office of Insurance Company on 02.12.2017 and advise the matter to Madam. Oindrila to whom I have submitted my both the Bills that I have not received the payment of second Bill of my wife. She checked only the records & told me you have submitted only one Bill the other Bill not has been deposited here and told me to make a phone on 06.12.2017, in the mean time she will search the records to see the Bills copies alongwith other papers. I have made phone on 06.12.2017 and asked him about my Bills, she again repeat the same talkings we have searched but found only one Bill which was already paid but nothing about my other Bills.

So, I request, please look into matter & arrange for the lost Bills & payment theirof.

The matter is extremely urgent.

    This is in regard to my insurance claim made with united india insurance and tpa 'dedicated healthcare services'
    -that i am an employee of bank of baroda
    -that we are covered under medical insurance scheme by united india insurance
    -that i was blessed with a baby girl on 2 october for which i had sent the reimbursement forms and other documents to the above mentioned company. On 28 october, their office had received my papers which is as per their condition of sending claim of reimbursement within one month. But till date i have not received the payment which is rs 34167. My claim id is 15660143. On telephonic conversation on their toll free number, i have been informed that 'insurer payment is in progress'. Even after 15 days of this status, i haven't received the payment.
    -this undue delay in making payment is just not acceptable and the company is liable to pay interest.

    Kindly look into the matter and guide me accordingly.

    I have made claim of my Vehicle bearing reg no HP575600 before 1 year and still the claimed amount have not been made from last 1 year, we are asking the offices in Himachal Pradesh and we are not getting the proper response from any where. Please do the needfull

      I insured my car under policy no [protected]p[protected] from your most preferred insurance company. My vehicle was stolen dated 10/06/2017. I immediate inform the police control room on 100 mumber and lodged fir on very same day 10/06/2017. I lodged the claim in your company but my theft claim was rejected/deny the liability due to delay intimation and not parked in parking. I wants to explain about delay and parking.
      This day was saturday and next day was sunday. On monday 12 june i was informed on telephone to branch office bagru where i insured my car on 11th june about stolen of my car. Branch office bagru asked me to inform to service hub jaipur i was not aware about to inform to service hub. Then i was tried to inform on telephone on 12th 13th and 14 june to service hub but no body picked the phone and i again tried continuously on 16 june picked the phone at hub and asked to inform in written. On that time i was suffering from tuberculosis and taking treatment from doctor and the treatment is still going.in mean time i was not in position to came my self due to illness although there was 12 june to 16 june was working day and 17 and 18 june again holyday.in spite of illness i personally came to jaipur on 19/06/2017 from my village which is far about 65 kilometre from jaipur and inform to service hub jaipur in written.
      On the date of accident i went to dulbhji hospital to visit/met my relative on 10th june. My relative was very serious so i was hurry to went. At that time the gaurd asked that there is no space in parking and wait to half an hours. I was not in position to wait as my relative was very serious so i parked the vehicle in street in front of hospital. There was sufficiant parking and about 20 to 25 vehicles already parked.
      I humbly request to consider/settle my theft claim and oblige me. As there is mare delay intimation and no parking was available.

          Hi sir,

          This is regarding insurance claim for my hyundai verna vehicle ka53 ma 7841 registered on name of sap labs india ltd.
          Car lease policy with sap labs got over on mar-2017 and sap labs has issued noc to us.
          I had taken an insurance policy with united india insurance policy number[protected] dated 10-jul-2017.

          Unfortunately we met with an accident on 30-sep-2017. We have provided all the required documents including noc issued by sap labs and leaseplan india on name of my wife (Deepika chary chilumula). We have also provided cancelled cheque of my wife's bank account.

          However united india insurance company is not processing the claim citing the reason that car is not transferred on our name and they can only transfer the money to sap labs.

          We have checked with our company (Sap labs) hr and they have confirmed that money cannot be accepted by sap labs since lease with company is already over. Money should be paid directly to costumer (I. E deepika chary chilumula).

          Request concerning authorities to help in this matter.

            I admitted my wife in yashoda hospital due to chest pain on&off and discomfort and senior doctor declared as blocks in arteries and suggested to go for stent installation after diagnosis. I paid some earnest money for procedures to complete and informed hospital insurance desk about insurance policy i carried and submitted the required documents. Policy of my wife sum insured was rs.2, 00, 000/- but i was paid only rs.70, 000/- for which i was shocked at the time of paying final bill.in such difficult situation there is no insurance concerned to negotiate reasons about this less amount assured.in such contingencies i was helpless being senior citizen aged 73 years and took a friendly loan from my son-in - law to pay balance & discharged my wife. Such a barbaric treatment by united/vidal insurance on insured is unforgivable and un ethical. If such situation comes for themselves they would have sanctioned claim amounts beyond limits. Funny clauses are never applicable for themselves being employees.

Company is not paying me rs. 50000/- (International tour) against 8 star agent incentive of financial year 2015-16. Since then i am continuously sending mails to company but till date no reply is received. Plz look into the matter and resolve it

                Dear sir, i am n r natarajasivam. I am a bank employee in indian overseas bank. I have the insurance from united india insurance company (Mdindia) for cash less treatment. I had an surgery (Vestibular scwannoma) on head on 2016 september. And made a claim for the same in united india insurance company. My claim was 2 part 1 is from sree chithirai thirunal hospital, thiruvanathapuram. 2 is from kmch, coimbatore. The 2nd part of my claim was settled on october 2017, but from the 1st my claim on sree chithirai thirunal hospital thiruvanathapuram was on problem. The insurance company was saying that i didn't submit the 2nd page of the bill. They are saying that, they received only page 1 and 3. But the 2nd page is on backside of the page 1 of the same paper. I have told this to the insurance company. And also sent a xerox copy of the same, along with my scan report. (The insurance company asked for those papers). Mdindia also have the original bill copy. Now they ve closed my claim saying that they didn't receive the 2 page of my bill. It makes me feel worse.

                I had submitted my Domicilary Treatment Bills for Rs 11347/-dated on 1/8/17 the claim settled on 4/11/17 but payment has not yet been received Please look into the matter and take necessary steps to credit the sanctioned amount Rs7158/- on receive this Mail.

                  After exhausting all sources, I am now approaching you with high hopes and as a last resort. Therefore, kindly spare few of your precious minutes from your busy schedule for me. There may be many retirees who must be suffering, suffocating slogging like me helplessly .
                  I am suffering from number of ailments SUCH AS DIMENTIA, ALZAMER, HIGH BLOOD PRESSRE, CHOLESTROL, SLEEP APNEA, DEPRESSION, HANDSHAKING .ETC.
                  I give below brief history of my grievances.
                  I, after few clinical/pathological tests prescribed by a Pulmonologist of Zainab Memorial Hospital –a trust run hospital for poor strongly advised by– DR. MADHVI GONDHA –MD, A PULMONOLOGIST TO USE CONTINUEOUSLY/ DAILY CPAP AUTOMATIC MACHINE FOR UNINTERPTED and ADEQUATE SUPPLY OF OXYGEN as there is FREQUENT BREAK DOWN OF BREATH . Consequently, ALL THE VITAL ORGANS OF THE BODY NAMELY BRAIN, MEMORY, EYES, LUNGS, LEVER, HEART, KIDNEY ETC..
                  You will please agree and appreciate that WITHOUT CONTINUOUS AND ADEQUATE SUPPLY of OXYGEN TO BODY, healthy survival cannot be imagined.
                  Upon doctors advice, cost of the CPAP machine was inquired. The cost was ranging between Rs. 65/90000.
                  I sent an e mail to DHS for pre authorization seeking permission to buy the CPAP machine. NO RSPONSE CAME FOR SOME DAYS.
                  The policy documents of Group Medical Scheme for retirees clause no 13.5 which reads as under “


                  QUOTE clause 13.5 of policy document of Group medial scheme for retirees of the bank.
                  “Rental Charges for External and or Durable Medical equipment of any kind used for diagnosis and or treatment including CPAP, CPAD, Bi-PAP, Infusion pump etc. will be covered under the scheme. However purchase of the above equipment to be subsequently used at home in exceptional cases on medical advice shall be covered " UNQUOTE

                  In view o[censored]rgency, I purchased a CPAP MACHINE relying on the terms and conditions of the policy document as stated herein above.

                  I submitted my claim for reimbursement of cost of CPAP MACHINE OF RS.72754/ AND AND PATHOLOGICAT/CLINACAL TESTS COSTING RS.5500/ amounting to Rs.78254/A-- ALONG WITH DOMICILARY CLAIM OF RS. 12219/ - on 26 th September 2017.
                  A query was raised by DHS on 3/10/2017 for submission CD OF BMI REPORT.
                  BMI is nothing but Body Mass Index which is an arithmetical calculation of Height and weight. The Pulmonologist certified that so long there is no CD made in India for BMI . The original certificate is with DHS. An email along with Zainab Memorial Hospital and doctor’s certificate, hospital case papers, BMI GRAPH and other papers were emailed on 6 th October and the hard copies thereof were couriered which have been received by DHS on 9th Octber 2017.
                  After trail of correspondence by me to DHS, when no response came I contacted the following authorities

                  Last fortnight I saw on web of Medibuddy the FINAL REMINDER- INFORMATION AWAITED.”
                  In fact the information /papers required were sent there and then on 6th October 2017 followed by hard copies received by DHS on 9thoctober 2017.
                  Last week it was displayed “MEDICINES ARE NOT RELATED WITH AILMENTS” . I sent an email to the authorities

                  At 5.30 P.M YESTERDAY I.E 22/11/17, It was displayed on Medi- buddy site -web that Rs . 12219/ - considered for my claim no 15414733 DATED 26/9/17 the DHS has considered only DOMICILARY claim of Rs 12 239/

                  NEITHER THERE IS ANY MENTION OF PART PAYMENT NOR MENTION OF REASON OF DECLINING CLAIM OF COST OF CPAP MACHINE AND CLINICAL/PATHOLOGOGICAL CHARGES. NOR GIVEN ANY COGENT REASON FOR NOTCONSIDERING
                  REIMBURSEMEN OF CPAP MACHINE ALTHOUGH THE POLICY DOCUMENT IS CRISTAL CLEAR.IIT MAY BE REITERATED THAT IT IS MACHINE AND NOT MEDICINES AS UNDERSTOOD /TREATED BY DHS MUMBAI.

                  MY AILMENT IS GENUINE, MY TREATMENT IS FROM REPUTED PULMONOLOGIST, NEED OF CPAP MACHINE IS UNAVOIDABLE AND TO TOP IT ALL IT IS WELL SPELT OUT IN THE POLICY DOCUMENT IN CLAUSE 13.5 AS STATED HEREIN ABOVE
                  I LOOK FORWARD TO YOU
                  Today morning while talking to H.o. I.R dept and help desk, I was informed there helplessness. . Hence this mail to you.
                  For the present I donot enclose copies of all the documents and e mails/ trailing correspondence, leave aside several telephonic talks with various authorities. However, If need be I will send a all the papers to you.ORIGINALS ARE LYING WITH DHS MUMBAI.

                  Sir, The DHS attitude and is certainly agonizing, humiliating and scarring to the senior citizens/retirees.
                  I with folded hand seek your whole hearted co operation and intervention for my genuine ailment, genuine claim.

                    On dated 23/10/17, i was admitted in emergency ward in mittal hospital, ajmer. They refused me for cashless treatment, my son prashant ganwani was informed to raksha tpa, jaipur. But they didn't guide proply and told that "mittal hospital, ajmer is not in our list but as per circular of punjab national bank dated 11/3/2016, hrmd circular no.300, we can take cashless facility if hospital is not listed in emergency. But due to hospital norms, we deposited cash in advance before the treatment started. I discharged from the hospital on 25/10/17. And i submitted the mediclaim bill by proper way of rs.13733 through our circle office punjab national bank, jodhpur on 2/11/2017. They sent the bill on 7/11/2017 to raksha tpa, jaipur but i am very sorry to say that raksha tpa has not taken the step for processing and i checked the status of my claim on web-site. The result seems that"no claim is pending. My policy coverage is with domiciliary treatment and paid higher premium. This is my first hospitalisation claim of the year. Due to very poor service of raksha tpa, i am very much worried that how i will recieve the cliam in future from raksha tpa.

                      I like to bring united india's attention that the customers o[censored]nited india is facing problems with md india health insurance tpa pvt ltd., regarding reimbursement of medical expenses;

                      Please go thru following details and take further necessary steps to ensure they apply their mind in resolving this simple calculations., and ensure reimbursement of rs 1600 asap., thanks

                      Day 3 1 1200+500=1700
                      Day 4 1 1200+500+1700
                      Day 5 (0.5) 600+250=850
                      Total 4.5 3900+1950=5850

                      Out of above, excess is only rs 500 (Day3-200 + day4-200 + day5-100).

                      But they deducted rs.2100., no responsible answer received from md india thus for reimbursement of rs. 1600 since 4 oct 2017.

                      I request they must use good quality calculator and must apply their mind to arrive the correct result of the above claim.

                      They must reimburse rs 1600.00 asap

                        Myself is surajit misra. I have purchased a family health insurance policy from united india insurance company ltd. On 18/01/2017. My policy number is[protected]. Myself, my spouse and my daughter (One year old) are the part of this policy. On september my daughter was admitted in a hospital in karad, maharashtra. After that i am desperately contacting with the customer care o[censored]nited india insurance company ltd. I have send several mails to the available mail id's of this company, but, i am very disappointing about their attitude and response against my mail.in-fact they did not send any reply and response against my inquiries!!!. I have purchased this policy through online and the amount i have paid that is rs. 10902/- for the one year. This is not a nominal amount for a policy, but, the situation was created during my daughter's hospitalization was horrible. I am getting nervous about this policy and i would like to take strong action against this policy maker company. I have attached all. Pdf files of my mails which were send to the customer care of this company. Please take a look at this and i will be very happy if this company can be ceased for ever. Please take a look at this.

                          They are making fake commitments, they given fake information they cheated i want to take hard action against them.
                          They informed me something else and provide me something else.
                          They charging me 25% of cancellation.

                          They said that i can go to any hospital for full body check up
                          I get 25% discount on all medicine
                          I can get 50% off on every lab test.
                          I can get 25% off on every radiology test.

                          But i get nothing i received a full body check voucher but i a can not go any where for body check up they must send some one who will collect the samples
                          To avail medicine discount they are given me 10% with gst only.
                          Cheaters
                          On radiology and pathology test i have go with there listed lab only either it's 10km far your home you paid 250rps convenience to avail 25% discount.

                          They never informed me about this they lie to me and making fake commitments.
                          I paid 6000 thousand to them to make me fool it hurts when some one cheats you on the name of fact.

                            Being retired employee of punjab sind bank, corporate policy no 500100/48/00/00/00000007 taken which expired on 31.10.2017. My pf code g 01490. Md i'd md-15-[protected]. Md id tpa was processing claims. Bill for rs 1901/ sent on 6.3.17 not passed in spite of sending duplicate bill on 3.8.17. Three bills for rs 2101/, 3648/ and 3246/ sent on 6.5.17, 6.6.17 and 31.8.17 passed for 1901/, 2648/ and 2546/ after deducting consultation fee. There is no response in spite of repeated reminders and discussing with concerned officials ms tejinder kaur and sh. Vishal tiwari. They assured of needful every time but the issue is still unresolved although policy has expired.
                            This spoils the reputation of insurance company.

                              My vehicle no. Up-78-bu4467 met an accident on 25th august-2017 at madhubani, bihar. I lodged the insurance claim in united india insurance company limited, micro office-madhubhani, main road, tilak chowk, madhubani, bihar - 847211.
                              After verification of surveyor, i also submitted the bills of car repairing & other relevant documents on 26th septemebr-2017. Thereafter, i used to regularly followup in both offices of kanpur & madhubani, but unable to receive any claim amount till date. Even i lodged the complaint in grievance redressal forum o[censored]iicl on 17th october-2017 but unable to receive any reply. Plz resolve the issue to maintain customer satisfaction.

                                  Hi,
                                  Posting this as a information for others - might be helpful.
                                  Our commercial vehicle/car got stuck in mumbai flood on 28-aug-2017, which toed to workshop on 29 aug and then we got insurance surveyor visit on 31-aug.
                                  In our case surveyor and workshop people said we will do document formalities with in us (Like estimate, go ahead, final bill checks etc). But you need to be careful, otherwise after even month of delivery of your vehicle you would require to collect documents for claim process. (And if your home branch and workshop is in different cites then it would be nightmare)
                                  So below are the steps/documents you need to follow/keep in mind. Also be patient claim process might require 1 or 2 months

                                  1. Before delivery of your vehicle, even though they (Insurance surveyor and workshop) said we had, get copy of estimate with sign and rubber stamp of workshop.
                                  2. Final bill of workshop again with sign and rubber stamp
                                  3. If bill has been paid through net banking/card then statement of bank showing that transaction.
                                  Below docs - which usually you will have
                                  3. Physical copy of claim form filled - even though you have submitted scan copy earlier
                                  4. Driver licence copy and original to verify
                                  5. All vehicle docs rc, insurance, permit copies and originals to verify
                                  6. Pan card copy of owner signed and with rubber stamp of your tourist company on that page.

                                  You have to follow for every step on your own, otherwise process might go long.

                                    I have paid for my policy on 12th of october with cheque an amount of rs.8155 to the following divisional office:

                                    But i have not yet received the copy of my insurance from your representatives. I am unable to use my car since the date as i do not have the insurance copy and previous insurance has expired. I am bound to travel to my workplace via taxi/cab due to this negligence from your side.

                                    This is my first experience with united insurance co. And unfortunately i am suffering this delay because of no good reason.

                                    Can you please take this matter seriously and help me with the copy of the insurance asap? I have been regularly following up with your executives and have been given fake promises every time.

                                    Attached is the receipt i got from the representative.

                                      I have a medi assist id no. - [protected]. My cesarean will be done on 5th november from the sunflower nurshing home. The package for the same is rs. 40, 000 whereas i only get the authorization amount for rs. 22, 500. So, it means i have to bear rs. 17, 500 from my own pocket. But i do note have that enough money to bear the same, so kingly it is requested to enhance the authorized amount so that i can only have to bear the co-pay portion.

                                        With reference to above subject a clerfication is required as my father was operated on 4.7.2017 at narayana hrudayalaya banglore. The computation sheet received from your end which gives the break up of the amount reimbursed is not satisfactory. Major expense of the surgery whcich was of around four lakhs you are just giving 25% of that. Kindlu explain us on that.

                                        This mail was written on 16th october to vidal health who is the tpa o[censored]nited india insurance. Vidal health has reimbursed the claim amount. We had requested for an explaination from them but till date and time their is no response from their end. When i spoke to one of the employee in vidal who was taking care of this case he just told me that in the policy its written to reimburse 25% only. Last year same operation was conducted in mumbai and at that time vidal had reimbursed the entire amount.
