Debit card is not working properly

I have a SB account in Andhra Bank - Balajinagar Branch, and I am using Debit card also. But recently I used the Debit card at South India Shopping Mall for Rs.170/- on 16/03/2017and Petrol bunk at Miyapur for Rs.300/- on 26/03/2017. Amount deducted from my account but not reached to the merchant ( Declined).

Please refund the amount wrongly debited in my account.

As I have done transaction with Paytm on 3rd March 2017 by tensfering money from my account to paytm, but the amount got deducted from my andhra bank account and nothing was added into my paytm wallet. And it's been a long duration that I didn't receive any response from of them neither bank nor Paytm. So please look into it and do the needful as soon as possible....

After 20 mts. there is an SMS alert to my mobile that Rs.2000/- is debited in my account due to transaction at POs. As seen from the account statement of the POS the next day the amount was not credited to their account....

RS 1720/- amount deducted from my account but there is no slip from swipe machine on 19 march 2017, around 8:30pm at NOOR AL ARABI MATAAM Arabian restaurant, ERRAKUNTA, HYDERABAD.
Then next time I have done a 1400/- rs swipe it is successful at same place only after 5-10 mins
