This is to bring to your kind attention that that my late husband mr. Yogesh jain was an account holder of saving accounts with axis bank, road no 36, jubilee hills, hyderabad (Details mentioned below).

He was also the debit card holder ([protected]) with the bank. He had a sudden demise due to accidental fall from terrace on 11th april, 2017. The respective branch officials mrs. Sandhya rani and mr. Phani were intimated of the same via an application dated 20th april, 2017.
They were also requested to initiate the personal accidental (Death cover only) of rs. Five lakhs as promised on the debit card which he was actively using. I had furnished all the formalities as desired by the bank officials from time to time.
I have been enquiring from the branch about any update or progress on the death claim but they have not been able to furnish any satisfactory reply till date. I have visited the branch also multiple times and enquired about any update or progress about the death claim on the said debit card but no satisfactory reply. The branch head says that from their end they have done what they could, more than this they have no information to provide to me.
At last i raised this claim issue with the cluster head and the nodal officer appointed by axis bank on october 25th and also with bank ombudsman on 18th dec. To which i received a reply that the update on the same claim will be shared with me within three days time (Which i never received as on date). I sent a reminder to them on november 3rd and december 1st also and still i did not receive any reply from them till date.
Now i am absolutely disappointed and tired with the bank officials as to at what level i can get to know the status of the claim. I really do not know whose office door should i knock to get any resolution at all.
I request you to kindly look into the expedition of claim request as its been already more than eight months for me to keep inquiring about the claim settlement. This amount will help me and my son (12 years old) to have a respectful survival.
Your support and acknowledgement of this mail will rekindle the small hope in me that what bank promises while building a customer relationship, they also fulfil those promises.

(client id[protected] i was not getting sms alerts of the same on enquiry regarding this the banking staff yet could not give me satisfactory answer and i am not yet getting the alerts so please look into this matter also.
