that, i have been selected for the above referred card, based on my personal loan maintained in your bank.
one of your staff came to my residence, got my signature, photo and ID proof.
two days, before, i got an sms, that my application got declined.
I never asked for the card, your bank selected me and now they informed about the cancellation

Axis Bank customer support has been notified about the posted complaint.
It has been over a month i have opened my new salary account in axis bank and my debit card is not working yet. I have submitted my documents again to the bank agent as requested but still they haven't managed to get my card working. This is causing me a lot of inconvenience i would request you to solve the problem ASAP. I did not excepted this type of service form axis bank VERY DISAPPOINTED .
