OK, Here is my story - on 19th, 16th and 22nd july I booked 3 confirmed tickets for myself and my family, the pnr number assigned to me as per irctc.co.in were - [protected], [protected] and [protected].
Usually I take a printout as soon as i know that my ticket is confirmed. I had my old mother travelling with me so I was extra cautious that tickets are infact CONFIRMED and no last minute waiting. We were supposed to leave from NZM to DDN on 5/Aug/09 at 11:55 pm and i went to take printout of these ticket on 5/aug/09 at 1:00 pm only to find out that the tickets have been cancelled, surprised..., I called up IRCTC [protected]@[protected] and here is the transcript of my conversation -
Customer - I am calling in regards to the PNR no. [protected], I have confirmed tickets and your website says that they are cancelled ???
IRCTC exec - let me check (after 5 minutes) yes they are cancelled
Customer - I want to know how ? I never placed a request to cancel them... is something going wrong with your system ??
IRCTC exec - let me check (after 5 minutes) my systems show that you cancelled the tickets
Customer - well I did not, I am going to board the train in next few hours... why the hell on earth will I cancel my tickets ???
IRCTC exec - no, actually the tickets were cancelled by your user id and password
Customer - I am absolutely sure that no one but I know the password of my irctc.co.in account
IRCTC exec - we cannot do anythong sir, your tickets are cancelled, how may I help you
Customer - you can only help me if you tell me WHO cancelled my tickets... give me the IP ADDRESS from where the tickets were cancelled you must be keeping the session IDs or some sort of information for tracking purpose ?
IRCTC exec - all I can tell you is that tickets were cancelled on 4/aug - yesterday
Customer - I want to know who cancelled it ? give me the session information, IP address something from where my tickets were cancelled
IRCTC exec - I cannot help you any further, please email us at [protected]@irctc.co.in and they will definitely provide you all the information
so, I called up again (about 3 times) but... it was a waste of time... and I got the same reply - "email us at [protected]@irctc.co.in"
I went to the nearby police station and launched an FIR under cyber crime that someone hacked my password and is misusing my account. Local police said that they will take this up but they were surprised why IRCTC guys are not helping me, they should atlease keep track on IP, session information.
So my request to [protected]@irctc.co.in was to kindly check what were the activities on 4/aug/2009, I did not even log into my account that day so something was wrong - I am very much sure that it was someone from IRCTC IT DEPARTMENT who did this, god only knows why ? may be he wanted his/relatives tickets to get confirmed and randomly picked my account and cancelled this.
As i was very much expexting, there was no response from IRCTC guys, they are just bunch of stupid people sitting there are eating governments money, well, our taxpayers money.
Look at the irony, we pay them and they don't work for us and we can't fire them. :)

I have booked a ticket through IRCTC on 09 Mar 2012 and the amount got debitted from my Account but PNR number not generated and ticket not issued. I checked in IRCTC site under failed transactions and the transaction is available and provided with information that instructions provided for the Bank to credit the amount on 03 Mar 2012. But still the amount not credited to my account when I enquired with my accounting branch, bank informed that no instructions has been provided to credit the amount. IRCTC Customer care requested to provide the screen shot of my bank trasanctions. But till now I have not received any reply and I followed with them for a couple of times. Please advice how I can get my money back.
