Dear sir,
We the surveyors from nagaon are highly aggrieved for non payment of fees from last several months. We have requested you verbally several times, but nothing has been done till now. The surveyor fees are pending whereas concerned dealer or insured payment done. Further we have come to know that claims are registered at the time of claim payment in all the department. We all surveyor once again request you to make payment of all pending fees in any department within 23/06/2018 failing which you are forcing us to recover through legal suit & in that case you & concerned department personnel will be personally liable for payment of mental agony, due interest for delay along with survey fees payment. We once again request you to make payment of all pending fees as per pending list of individual surveyor attached. We further request you to stop this type of claim payment where surveyor fees are non paid. If you are not willing to pay then please reply that fees will not be paid. Payment of fees in a decent away are awaited by following undersigned surveyor till 23/06/2018.

My claim was approved in the month of March but I still haven't received the amount. When I contacted MediAssist, I was informed that they have duly forwarded the required documents including a debit note to the insurance company but the insurance company is delaying the payment or not responding to the same.

    I start my mediclaim insurance policy on 5th oct 2016. I am hospitalized on april 2018 for some pain. After 2 days stay at hospital. I submit my claim. But they are told that claim not acceptable. I have all report and all bills during hospitalized. Please advise accordingly. What to do to get the reimbursement, i want to lodge my compain against national insurance

      I pay 20000 premium a year for my car.. The procedures of this company takes more than 30 days to have my claim done. And the surveyor mr. Manik reddy from bangalore kormangala branch.. Is a kind of person who never talks in a good manner.. By the way he talks seems he is above 50. This man lack manners to customers. And the kormangala branch peoples are not even concerened about these things. They wont even reply for mail. And never provide the liability worksheet. If we call and ask they just cut the call.. Or else they will tell they dont have time for this.

I have submitted all the claim related document in the concerned bank branch on 11.04.2018 and according to bank bank has submitt all document to concerned national insurance company for settlement of claim till date claim is not settled. At this stage neither bank nor national insurance company provide status of claim

          Dear sir

          I am the former director general of the corps of army air defence in the indian army having had the honour to serve the nation for 44 long years in uniform.

          I wish to bring to your kind notice the case of my brother, shri pradeep saxena who is a cancer patient and whose genuine and just claim, duly supported by high-end medical advice has been unethically rejected by an insurance company.

          The unfortunate outcome of this rejection has been that a cancer patient is still awaiting proper treatment.

          I had initially taken up this case on 13 apr 18 with the office of the insurance ombudsman located at jeevan bhawan phase 26th floor naval kishore road hazarat ganj lucknow226001.

          The above office has advised me that the case is to be first presented to your office.in case i am not satisfied by your decision or no response is received from your office within one month of this letter (E-mail), then i can approach the office of the ombudsman as quoted above. (A photocopy of the reply received from the office of the ombudsman is enclosed please)

          As advised, i am therefore presenting the case to your good office

          The salient points of the case are briefly enumerated below

          • shri pradeep saxena is a case of hurthle cell carcinoma thyroid who has got the left half portion of the thyroid surgically removed on 09/02/17 to avoid spread of cancer (Post hemithyroidectomy).

          • shri pradeep has been under the insurance cover of _ for the past _ years. He has regularly paid all premiums year on year without a single default. His policy details are as under

          • towards the treatment of the above medical condition, a doctor of the stature of senior director nuclear medicine services at max super speciality hospital saket, advised on12 mar 18 that a 100 mci high dose 1-131 (Radioactive iodine) is required to be administered to the patient since the previous dose of 30 mci of radio iodine was insufficient since the scan shows a functioning thyroid tissue in the neck. (Copy of the letter fro the doctor is enclosed)

          • since the persons administered the above doses emit radioactive radiations, the atomic energy regulatory board (Aerb) has laid down in its guidelines that patients being administered doses higher than 30 mci will have to be admitted and will only be discharged when the radiation levels come down to permissible limit (50 usv/hr at 1 meter distance) this is to avoid exposing other persons with harmful radiations.

          • since in this case a high dose of 100mci was to be administered, the doctor advised admission for 1-2 days

          The claim prepared by the patient has been denied by the tpa.

          And since the patient does not have enough funds to pay up front, he is suffering without treatment

          Please consider the following facts
          • a dose of 100 mci has been considered inescapable by no less than the senior director nuclear medicine services max hospital.

          • 1-2 days of admission is a must to avoid exposing other people from harmful radiations. It is not a luxury. It is a must.

          • since the patient does not have the funds to spend upfront he is without treatment.

          In the light of the above, may i request you to please consider this genuine claim and direct the insurance company to allow the same up front.

            I had purchased insurance policy from national insurance company for my

            House hold goods. My tv was not working and require to replace the parts.

            Surveyor from the company had visited my residence along with tv

            Mechanic the estimate repairing cost was said about 2500 to 3000. The

            Final bill for rs 2750 paid by me to mechanic in dec/jan 2018 the

            Insurance company has after several reminders with branch and their

            Superior i rs 1500 credited in my a/c on 28/05/2018 i had requested

            Branch to clarify for less amount of claim but did not reply satisfactorily

            Let me know the calculation for less amount of payment to me

              On 21.4.2018, myself the primary policy holder of the health insurance policy no. Gini o[protected], was operated upon at apollo gleneagles hospital, kolkata to remove the implants (Two titanium plates and fourteen numbers of 3"& 2" sizes screws) from my right leg as i was not feeling well with those foreign bodies within me. Those implants were inserted during bone grafting operation at my right leg in the year '2016 to repair a comminuted fracture after an accident. This time, before going for operation, the treating surgeon informed the genins tpa by e mail that the implant removal operation would be a complex one which can not not be considered as a mere package deal and cost incurred should be taken into account accordingly. But, it is a matter of regret, despair and a feeling of being cheated that the genins tpa has made my well documented and valid reimbursement claim of rs. 139000/- into a final settlement of rs. 37000/- only as per a package of rs. 30000/-. Although, i have written at genins grievance with a protest against such irrational and whimsical claim settlement, nothing has been informed to me till now.in view of this hereby i am requesting you to please intervene into this matter so that justice can prevail in favour of a genuine patient and policy holder.

              3) BEFORE GOING FOR OPERATION THE TREATING SURGEON SENT AN ESTIMATION OF Rs 188000/- TO GENINS TPA THROUGH APOLLO GLENEAGLES HOSPITAL BUT GENINS REPLIED NOTHING AND WHICH WAS CONSIDERED AS AN AGREED GESTURE.

                I Ram Murthy ([protected]) was driving and accidentally My iPhone fell on the busy road a car accidentally drive on the iPhone and the display got damaged on 27/04/2018 around 9.00 PM at outer ring road Balladur and when I went to claim the accidental insurance I was declined stating as willful neglect, My CRN No: 260200/29458/17/[protected]. How can some one willfully trough the phone below a car that too when I have paid 70K on a phone and additional 3k on the insurance. I tried to talk to the ample or the icare customer care they repeatedly declined my claim stating willful neglect.

                  I am holiding the above policy for more rhan a decade. Recently i renewed the policy no[protected] and bank of baroda mylapore branch has issued a banker's cheque no no287208 dated 03 03 2018 for rs7434/_towards premium for the year 2018_2019.in spite of continuous followup with mr purushothkumar tswho is taking care of health policies in chennai i have not received my policy so far. I also send an email to 500200 and mr purushothkumar ts requesting atleast send a letter acknowledging renewal details to enable me to claim benefit under 80 (D) and send me id card if they couldn't send the policy. But no response. I thnk he is not inyterested in giving customer service. Kindly arrange to send the renewal policy. At an early date.

                      This is to bring to your notice about a insurance company of which i was a customer and i used to pay a premium as well from national insurance company branch pollachi,

                      Now i had applied for a claim for a shop insurance in 2006 for a cover of 3 lakh rs
                      Of which i got a approval of only 1, 20, 000rs.
                      Now after paying all my premiums on time and being a good customer
                      Firstly my claim was delayed by nearly 2 years.
                      And the amount that i was promised of 3 lakhs was nearly half the amount that was being given to me and then i had only 15 days notice to claim that
                      Of which those 15 days were finished in contacting the customer service itself who were denying me the entire amount that i was promised which is 3 lakh rs.
                      Now these sales people from the company are either giving false promises for a lead in their work targets or the company is telling them to lie to its customers at that they can attract false leads and dupe the customers of money or this could be a scam or else a fraud that is running in this company.

                      Now it's been more than a decade me fighting alone for my insurance and i am really tired fighting fory own money
                      As from 2006 and now it's 2018.
                      I even went to the court and wasted so much time and money then as my shop got burnt for which i applied for insurance claim i could not start my business again.
                      Have been unemployed for so many years as well.
                      Now i really need some help as these people are taking it for granted
                      So my request to you is to please firstly provide me the insurance amount back and then the interest fit do many years and also an alimony for these people harassing me over the phone as they use to directly cut the line or keep me on hold for a long time that i must get frustrated and hang up.

                        Fire broke out in our factory on[protected]
                        Fire broke out in our factory m/s fortune pharmaceuticals (India) pvt. Ltd. Causing huge loss of building machinery & stock. The information regarding the incident was reported to the national insurance company and their surveyor mr. R. K. Makkar has conducted the survey job, and as per their requirement we have submitted almost all the documents which includes, estimate of repair of building, paint, electricity etc., recent balance sheets, estimate of machine repair etc.

                        In spite of the fact that we have cooperated with the insurance company in all possible means, but even after one and half year after the incident no claim is given. And we are forced to even keep the damaged goods till date, although the same has been checked and surveyed by the surveyor.

                        We have incurred a loss of more than rs. 1 cr and as on now we are in short of fund to run our factory smoothly. There are 20 -25 employees which are working with us and any further delay may lead to termination of their services due to non functioning of the factory.
                        We had even requested the insurance company to release some interim payment so that normal operations in the factory can be resumed, but the insurance never responded. We are incurring daily/monthly loss due the salary payments and daily expenses.

                        We are in great financial stress right now and losing confidence in the functioning of the public sector companies for their delay tactic approach. We hereby request you to kindly intervene in the matter so that our trust in the govt. Regulatory authority.

                          My truck was stolen on 6th march'18 from bansgora under khejuri p. S, east midnapur. I then & then lodg g. D at khejuri p. S and informed nic contai office after that all paper works has been completed.in the basis of my personal search i found the vehicle on jalpaiguri and local kotali p. S took the vehicle under their care. After that all realsing process has completed and found all tyres, rim, battery were missing. The investigating officer mention this in his report.in spite of all there the b. M of contai does not care to release the claim. He demand police report but i. C of khejuri p. S tell us he already submitted the report on contai court if nic contai branch need it they collect from court otherwise if nic contai branch write a letter to khejuri p. S they will give a letter with missing items but b. M of contai branch does not agree to write such letter.in the circumstances my claim is left unsettled yet.

                            Hello sir,

                            My father met with a road accident and died last year in the month of august. We filed a claim application with all the necessary documents like fir copy, death certificate, post mortem report, final police reports, identification proof and address proof. Now the insurance company is asking for forensic pathological reports but, any such reports were not provided to us along with other reports. We submitted an application requesting for this report to the police station saying that the insurance company demanded this for settling this claim. They denied to reopen the case saying it is a crystal clear hit and run case and on spot death happened.

                            Please help us as we are helpless in this matter.

                                I mayank kumar having corporate policy no-100600/50/16/[protected]protected) of national insurance company regrat to infrom you that i have summitted all original document to Paramount TPA and claim process has been completed and approved by TPA, but still the payment is pending more than 5 month and TPA is saying that they have sent my claim file to national insurance company before 5 month and national insurance is making delay my payment posting, while i tried to approach both UIB broker and TPA last 5 month they are responding my call and email, i am waiting for my two payment rs.16408, and rs.21621

                                  I dipak moy banerjee having a policy no - 154601/48/16/[protected] of national insurance company regret to inform you that on 5th march 2018 cheque no 6 of rs 9273 debited from my a/c xxxx5635 of bank of india garia branch. On 26th march 2018, an amount of 9293 was debited from my a/c.
                                  Till date, i have not received my policy. God knows when i will get my policy. If this is the state of affairs in your company i am afraid i would think twice to continue with your company.
                                  My wife fell down at my residence on 13th april 2018 and she had to undergo a surgery. But in spite of paying the premium, she was deprived of the benefit of mediclaim from your company.

                                      Sir/madam
                                      Staff are not issuing polices on time because of that i losing my customer. They coming by 10.30 and having there food for long time. We are waiting for long time if we they will tell we cannot issue it fastly you should wait. Like that if customer came with policy for renewal to office they are sending them to other insurance company. Thare are not putting policy. It is mainly happening in tambaram office chennai region.

As per checking with TPA, claim process has been completed and approved from TPA side, but still the payment is pending for more than 3 months. While i tried to call both TPA and national insurance for the past 3 months but no one is picking up the call and responding.                                         Good Evening, My name is Sathyamurthy. My claim no is NICMRO-3/13853 - PARK Mediclaim TPA
                                        I have submitted all original documents to Park Mediclaim TPA insurance comp.. And they submitted to you 3 months back. As per checking with TPA, claim process has been completed and approved from TPA side, but still the payment is pending for more than 3 months. TPA is stating that they have sent my claim file to National insurance before 3 months and national insurance is making delay in payment posting. While i tried to call both TPA and national insurance for the past 3 months but no one is picking up the call and responding. Actual claim intimation date for the said claim was on july 2017 and it is pending for almost 10 months.
                                        i still had not received the amount yet I try to escalate this with several emails but no one is bothered about it and it's me who have to suffer without money Company's want the payments on time but when it comes to customer to claim and get money back they have 1000 of reason why there is a delay and why they can not pay but if you miss one of their instalment they don't know you at the time of claim.
