Meri gadi july 2011 me raipur mowa area se chori ho gai... Jiska f. I. R. Sambandhit thana me karaya aur uske baad b kuch result nahi nilalne par thana se antim pratiwedan ki copy hasil kiya. Us copy ko mai insurance claim karne k liye priti auto centre tapkara ko saup diya aur kaha k aap plz jaldi karwa dijiyega... Unhone kaha hum karwa denge aap tension mt lijiye. 2011 se lekar aaj tak jb b maine unse jankari leni chahi to unhone hamesha yahi kaha ki hum laga diye hain korba office me pending hai... Ho hi jayega... Paresan mt hoiye.
Aaj 6 saal ho gaye par kuch hasil nahi hua. Kripaya aawashyak karyawahi karen.

      I am an employee of bandhan bank ltd, i have claimed for my mother's cataract operation.
      I have completed all the formalities and documentation advised by mediassist.

      My claim has been approved since 16/11/2017 and i have got the approved letter too from mediassist but approved amount has not been credited into my account.

      Mean while in letter it is clearly mentioned that approved amount would be credited in 7 days..

      But still approved amount is not credited into my account.

        I believe you are the point of contact for national insurance company ltd. To brief you my name is ranjana and work at ernst & young company bangalore.

        National insurance company is the insurer for ernst & young and the third party was united health care upto march 2017. For this year 2017 i have 2 reimbursement claims policy number-602200/46/16/[protected] which was for my dad as he was diagnosed for cirrhosis of liver due to hepatitis b at st. Philomena’s hospital bangalore unfortunately he passed away in june 2017. The problem with the first claim id[protected] &[protected] for january 2017 (17th -19th jan 2017) is that uhc is expecting a document “treatment chart” and the hospital says that they don’t have any separate document called treatment chart instead hospital has indoor case papers and nursing sheet and asking uhc to refer to that which i have already submitted but uhc has denied my claim for this purpose.

        The second claim id[protected]th – 19th march 2017) the problem is a temporary treating doctor (Dr. Ranjith shetty) had done the distension of abdomen fluid for my dad and he has sent the patient history as alcoholic which is incorrect. Later this doctor (Dr. Ranjith shetty) has also written a letter saying the patient is non-alcoholic attached his letter written in march for your reference. This temporary doctor was unaware of the fact that the patient was a hepatitis b positive due to which it led to cirrhosis of liver and not because of alcohol. The doctor who had provided the letter stating the patient is non-alcoholic is the actual treating doctor (Dr. Praveen) who had treated my dad from the start i. E january till my dad’s death certificate was provided. Only the march treatment was done by dr. Ranjith shetty which has caused a chaos to this claim. Dr. Praveen the actual treating doctor has also written a final letter stating the cause for cirrhosis of liver is due to hepatitis b (See attached)

        Note that i have also reached out to the grievance cell o[censored]hc but no response from their end.

        Incase this has to be forwarded to any other relevant contact please do the needful as i want to approach the grievance cell of national insurance company to take this forward as there seems no end to this case. I need justice to this.

        Please find below the email chain and discussions so far with uhc. I have also attached all the doctor’s letter i have so far along with the uhc rejection document.         Sir, I have lodged complaint online grievance portal vide No. GRI[protected] but till date I could not find the status of payment of my claim. Claim was approved on[protected] but till date I do not receive the payment. Is it a prompt service of your company.

        It seems you are doing nothing for the customer but only come to the office for cup of tea and gossips. Please you should read or take care of your own commitments.

          I have been informed to contact you for complaining about our policy. We havent received full claim even after talking to dealer multiple times and waiting for 3 months. We choose government company because we thought it would be easy to get claim but happening quite opposite.

          So, we are escalating the matter to you.

            Venous malformation surgery of my daughter khushi garg in gangaram hospital on 22/5/2017 after adopting a due process of law. Mediacare tpa has been informed well in time. All requisite documents were submitted by us according to their satisfaction as and when demanded by medicare tpa. Pursuance for this mediclaim was continued upto september with medicare tpa but they did not respond me properly,. At last on my asking they have only replied our doctor has declined ur claim but did not send any doctor report or any other information officially to me till date. All sequence was apprized parallelly to grievances cell as well as senior officials of nicl. At last we appealed in bima lokpal. Only one telephonic reply was got from bimalokpal that ur case has been registered but we are still unable for hearing as presiding officer is not on the seat. After this no rcommunication recd to us. Kindly fix hearing in this matterasap

              I tried renewing my mother's (Sanjana's) health insurance policy (National mediclaim policy) having policy number[protected]

              I tied renewing the policy online through the healthinsuranceindia.com website and made the payment of rs 10, 019 via credit card. The payment got successfully made but the following message was displayed on the screen - 'your payment is successful but system could not generate the policy. The transaction number is easi[protected]

              Since the transaction has been successfully made, i request you to generate the renewed policy and send it at my postal address as well as via e-mail on this email address itself.

              I am attaching the screenshot of the page that got generated after the transaction was made.in it is clearly mentioned what i have stated above.

              Please let me know if something more has to be done at my end for successfully generating this policy.

                  National insurance company has given me above policy number 271200/48/16 valid from 19 march 2017 to 18 march 2018. I have been complaining to them that service provider medicare tpa services is not issuing any card or identity to me but company is not bothered. Every year they change service provider. Even they have not forwarded my details to service provider. Poor service is so much. Please ask insurance company to refund my premium or provide sensible id card, details etc

                        My father, radhey shyam gupta, is covered under national mediclaim policy with national insurance company ltd and family health plan (Tpa) ltd - kolkata. My father was admitted in fortis hospital & kidney institute (Rashbehari, kolkata) on 16th august, 2017 for kidney stone operation and was discharged on 20th august, 2017. As cashless facility was denied by the hospital so we submitted all the relevant documents for claim reimbursement to fhpl through agent on 1st september, 2017. The claim amount submitted for pre-hosp and hospitalisation was rs.1, 65, 315. It has been almost 3 months and we have still not received the claim money, neither have we been informed about the settled amount. We made so many complaints in fhpl but they just don't care to respond. They are unable to give any justification for delay. After constantly complaining my father received just one sms on 16th november, 2017 which states -
                        "dear sir/madam, your claim reimbursement for claim ref. Id 1621322 has been processed and amount will be credited to your bank account shortly."
                        After this again there is no response.
                        Now, as per policy we are supposed to be reimbursed the claim money within 30 days of document submission. The complete policy statement as per nicl :-
                        "on receipt of the final document (S) or investigation report (If any), as the case may be, the company shall within a period of 30 days offer a settlement of the claim to the insured person.
                        Upon acceptance of an offer of settlement as stated above by the insured person, the payment of the amount due shall be made within 7 days from the date of acceptance of the offer by the company.
                        In the cases of delay in the payment, the company shall pay interest at a rate which is 2% above the bank rate prevalent at the beginning of the financial year in which the claim is reviewed."

                        As per policy and the given delay situation, it is expected to provide as soon as possible not only the claimed amount but also the interest amount.

                        Also i submitted post hospitalization documents in 3rd week of october, 2017. There is no response about the post hospitalization claim money as well.

                        I am not getting any valid response from fhpl/nicl. I want to get my money back asap (Pre-hosp, hosp, and post hospitalisation money).

                            I had written an email to FAF.nic.co.in on 22-Nov-2017 with a photocopy attached.
                            1. My policy document shows incorrect policy number[protected].
                            I had attached a photocopy in my email. (The policy no is less than 20 digit)
                            2. Did not receive any photo ID card. Policy start date is 2-Feb-2017
                            3. All the email addresses printed in the policy document are wrong. Emails sent to these addresses bounces.
                            4. The 80D receipt shows wrong value.

                            No replies or even an acknowledgement received.

                                My patient mrs sathi banerjee policy number 100300/50/16/[protected]Dhs id:- wi[protected] reimbrushment claim submitted on (Claim #[protected] at 14:09. The total amount claimed was 247997. From you & as well as dhs massage was received that around rs.231610 was approved and it is on process.                                   I refer to my claim no.15495581 for rs.36, 000/- for cataract operation
                                  An amount of rs.3000/- being cost of instrument charges and phaco tip are dis allowed for unknown reasons. I was informed that the above amounts are not payable as per policy terms and when i insisted for specific terms of policy under which the said amounts are disallowed. I was informed that the said amounts should have been included in hospital expenditure instead claiming separately. This can not be a reason for disallowing the expenditure incurred by me and i insisted for reimbursement of the said amount, i have been directed to you.
                                  The operation can not be done without said instruments and they are necessary expenditure. Hence i once again appeal to you to consider the amount for reimbursement without further delay

                                    With this letter, I am filling a complaint against New India Assurance and its TPA E-Meditek for the non-reimbursement of my mediclaim amount. I am furnishing details herewith:

                                    I received email on October 10th that my claim is approved. As I did not receive the amount in two days I called on Customer care number to inquire about it and I was told that they have rejected my canceled cheque as cheque did not bear my complete (full) name. Cheque has initials and so I have to submit a cheque bear full name. I asked them to mail me this reason but I have not received any intimation of for this till date. When asked why I was not intimated by email or SMS, the lady on phone replied that we do not inform such things and you have to follow up personally. When the premium amount is accepted with the cheque bearing initials then why the same cheque cannot be considered as the proof? I somewhere feel this is the way of an insurance company and TPA to harass customers and deliberate move to either delay payment or not pay the dues at all. This is waste of my time and mental harassment.

                                    My agent personally went and submitted bank statement on October 12 to Mr Jignesh Panchal of E MEDITEK, AHMEDABAD but nothing happened. We inquired once again and we were given same reply so we re-submitted bank statement second time on 31/10/2017 to Mr. Neterang Shah of e-meditek (TPA). But still nothing happened and once again I inquired with him but he flatly denied that he has not received the bank statement. So I once again emailed him on 13/11/2017.

                                    Please note that I was not informed either through mail or SMS about rejection.

                                    On following up with Mr. Neterang Shah on phone he ensured that it will be done in a day or two. But till date, nothing has happened. Now he is not even picking up the phone call.

                                    I request you to take up this matter.

                                    Now please reimburse my claim amount along with the interest on the amount, Rs 20, 000 for the mental harassment and Rs 2000 for all the expenses incurred for the multiple follow-ups.

                                    I have all the copies of emails that I have mailed to E Meditek.

                                    I have already registered my complaint with IRDA and here is the IRDA Token Number :[protected].

                                      On 15.11.2017, i, sumit roy, holder of national insurance mediclaim policy no.[protected], paid a premium of rs.21, 176/- (Rupees twenty one thousand one hundred seventy six only) online [vide state bank of india transaction ref. No.[protected]igacjpknf2 and mediclaim transaction id: easi[protected]] towards renewal of the aforesaid mediclaim policy which is going to expire on 25.11.2017. After payment, though my sbi savings bank account was debited by rs.21, 176/- (Copy of bank statement is enclosed for your ready reference), neither any computerised receipt was generated nor any renewed policy document was sent to my e-mail address by national insurance co. Ltd. Even after a lapse of three working days.

                                      Please look into the matter so that the receipt alongwith the renewed mediclaim policy is sent to my e-mail address at the earliest.

                                        Its been more than a month i have claimed for a insurance of my home which have been damaged by a truck accident.
                                        Before 1 month a person came and served
                                        A damaged home and have said to wait for a week. But it's been more than month no documents i. E. Report have been send till day. I called the office manager of gangtok many times he says no report have been received from siliguri office. I am really dissatisfied with the work..
