      Hello, My Name is Manik Lal karnani, My Customer ID is [protected], I have a joint Policy with My Wife Lata Devi Karnani. She Has Expired on 13th of July'2020. After that i have submitted all the documents to the officer name Mr Abhishek Sen.. he again and again asking for something or the other.
      i have submitted Discharge summary and death certificate copy which we have received from the hospital but he is not persuing our claim and his behaviour is also not goof and professional.
      i request you to please look after this

        And also find below expenses for both applicant.

        Hospitalization for Vasantbhai Thakkar

        For RHYTHM HEART INSTITUTE received cash less claim from Religare health insurance company Ltd of Rs 131707 other amount paid by me of Rs 233810.

        Hence Total claim amount for Vasantbhai Thakkar is Rs 271059.22 out of which claim settlement done only Rs 55000 as confirm by VIPUL TPA which is not acceptable.

        Hence Total claim amount for Champaben V Thakkar is Rs 102253.10out of which claim settlement done only Rs 55000 as confirm by VIPUL TPA which is not acceptable.

        I am strange that in both files received same amount ...

          Hi,
          My name is kanwar ajit singh gill, we have been through dr kapur’s the healing touch eye centre for the eye surgery of my daughter arshpreet kaur as on 04 august and her eye problem is not older than six months.
          Find alongside attached email communication and associated documents for your support and just we had received a whatsapp communication from one of your executive harish kumar that “preauthorization cannot be extended at this point as surgery is not payable as per policy t&c as the same falls under cosmetic”.
          Afterwards, we have had a words with mohit arora who is from dr kapur’s the healing touch eye centre and he has suggested us to contact you back to inform you that this is a not plastic surgery and surgery is about to have strabismus surgery which has already been planned to be happening on 17 august.
          Hope this would suffice to clear out the doubts and take measurable steps accordingly in order to have surgery timely and not to linger on since it is required on top most priority.
          Your reply is awaited and if you would be requiring any additional document or clarification then do let us know at any point of time.

            My old policy of year[protected] expired on 30/7/2019. I renew it online on 30/7/2019. I got msg transaction has been successfully completed. But on 1/8/2019 i was refunded. I didn't know about that. But when i am going to renew policy for year 2020-21 i found that there is no previous year 2019-20 policy. Now what can i do for continue that policy?

              This informs you that i am an employee of randstad india pvt ltd, having policy no[protected] of my spouse name babita from the mediassist (Insurer name - national insurance). I was submitted my spouse maternity claim no[protected]Insurer claim no :[protected] before more than two month, they inform me you claim denied due to
              On scrutiny of documents it is observed that the hospital is not registered and does not full fill the criteria. Also treating dr was bams md ayurvedha who has performed c-section. Based on these confirmations provided by our internal investigation team the claim is being repudiated

              But i am not satisfied the from the investigation and reason for denial because in the document checklist the point no 8 clearly mentions if the hospital is not registered please get the letter from the hospital letterhead number of beds and doctors and nurses available round the clock. I already submitted this letter to the mediassist, also submitted the treatment description which clearly mentions the four doctors involved in treatment in which dr neeru bala (M. D) (Gynecologist and obstetrician) and dr gautam chawla done the surgery. But still they investigate the claim on information based on sites of internet and nobody went to hospital for check their records.
              The four doctors with their regd number mentioned at the hospital letterhead already given.

                      This is diwakar vikram singh wants to lodge a complaint against the surveyor and the national insurance company which is located in gorakhpur, uttar pradesh. My vehicle toyota fortuner up 53 cl 5555 had an accident on 06 march 2020 which was sent to rajendra toyota gorakhpur and was surveyed by vijay pandeyand repaired there. Later on when i requested for the bill he asked for bribe and when i refused he made a false bill so that i cannot get the expected amount. I had to run to between divisional office and branch office from the past 20 days and they do not have even a clear excuse.

                                Dear sir,
                                I had undergone a angio-plasti on 11th march 2020; on 10thmarch202, care hospital submitted a claim reference no. 21850672; i have submitted all the three years policy document copies; for which they said i need to submit 4 years of policy document copies which i am not carrying or retaining; i am a customer of bank of baroda and thru them this insurance was done.
                                Their leave-let says it is 3 years continuous policy required to claim. Although i have four years of continuous policy without any break and requested them to please do something as i a regular customer with national health insurance and have been paying my yearly health insurance premium. My son approached national health insurance after the medical procedure was done; they could not help;
                                Now i have bank statement showing the debit for the insurance premium was done on[protected] via neft for rs.7246.
                                Ithis will cover the 4 years policy requirements and mediassist have said they are unable to process the claim.

                                Please help me as i have spend almost rs.2 lakhs and 5 thousand to hospital and about 20, 000 for other test; altogether i spend around rs.225, 000. I have been paying for my insurance for this day but have resulted in great inconvenience to me and my family.

                                All documents will be submitted to the tpa-mediassist after the lockdown period as the offices and banks are working on skeloton staff and not entertaining the customers for any copies of statement and the insurance contract copy.

                                Please help sir, i am retired person and your kind help is requested.

                                    Operation : removed blockage applying stent at ramkrishna seva pratisthan on 12 september 2019.

                                    Payment : made by credit card as there was no cashless facilities, hence paying interest rate of

                                    3.49% per month.

                                    Delay : 5 months since past, nothing has been received till date i e 26th february 2020.

                                    This letter : please consider this letter as a formal one as advised by ombudsman and also the

                                    Consumer affair ministry of govt of west bengal for immediate settlement. And also i lik to

                                    Ask you to reimburse the unnecessary huge interest is causing along with the claim money.

                                    Situation : it is unbearable physical, mental and financial torture to a 78 year old senior citizen who

                                      Regarding the claim number mdi4906766i have submitted the “delay submission letter” alongwith the documents submitted to md india's office.

                                      The claim was related to post hospitalisation of my mother sukla saha, date of discharge was 16th may 2019 & 60 days was supposed to be over on 15th july 2019. My main claim was settled through cashless so i have intimated the same to my agent mousumi mukherjee that i will submit my post hospitalisation claim after the expiry of 60days, accordingly i have asked her (His father tirtho mukherjee was working on behalf of her) to collect & submit all claim related documents for processing. During july 2019 to oct 2019 i was not in town due to my crop insurance duty in orrisa. I have asked my agent to coordinate the same & submit the documents to your good office but to my utter surprise he / she didn’t bothered to do the same. So i have left with no other option than to renew my policy with another agent mr. Ujjawal mallick & submit the claim documents through him.

                                      It is also surprising that the concerned divisional office was trying to safe guard the agent mousami mukherjee & almost forced me to change my initial letter by removing the actual in supporting part of their agent.

                                      Now my claim got stuck due to delay submission. It is humbel request that please re-consider my claim & make the payment or decline it with the same with proper reason so that i can move ahead accrdingly, because i have the call records & other proof to go ahead a proof against my previous agent about her services, she had enjoyed the commission against my premium paid to national insurance in return of which he / she supposed provide me the service as per requirement. So if national insurance can pay the commission to their agent from the premium received from the client then it is the duty of the organisation to take step against the agent based on the complaint received from the client.

                                        Ref: policy no.320904/03/0000/00044 dated 09/01/2004.

                                        I have surrendered the above stated policy to the branch manager, national insurance co. Ltd., power house, bhilai (Chattisgarh) on 28/11/2019 alongwith original policy document, cancelled cheque as well as aadhar copy. I was assured that the claim will be settled at the earliest.

                                        However smt santhi who promised to settle the claim as early as possible, now says that she has forwarded the claim to head office for settlement.

                                        I had similar policies issued by united india insurance company and oriental insurance company and both of them have settled the claim. It is therefore requested to do the needful from your end please.
