    Dear sir,
    I had undergone a angio-plasti on 11th march 2020; on 10thmarch202, care hospital submitted a claim reference no. 21850672; i have submitted all the three years policy document copies; for which they said i need to submit 4 years of policy document copies which i am not carrying or retaining; i am a customer of bank of baroda and thru them this insurance was done.
    Their leave-let says it is 3 years continuous policy required to claim. Although i have four years of continuous policy without any break and requested them to please do something as i a regular customer with national health insurance and have been paying my yearly health insurance premium. My son approached national health insurance after the medical procedure was done; they could not help;
    Now i have bank statement showing the debit for the insurance premium was done on[protected] via neft for rs.7246.
    Ithis will cover the 4 years policy requirements and mediassist have said they are unable to process the claim.

    Please help me as i have spend almost rs.2 lakhs and 5 thousand to hospital and about 20, 000 for other test; altogether i spend around rs.225, 000. I have been paying for my insurance for this day but have resulted in great inconvenience to me and my family.

    All documents will be submitted to the tpa-mediassist after the lockdown period as the offices and banks are working on skeloton staff and not entertaining the customers for any copies of statement and the insurance contract copy.

    Please help sir, i am retired person and your kind help is requested.

        Operation : removed blockage applying stent at ramkrishna seva pratisthan on 12 september 2019.

        Payment : made by credit card as there was no cashless facilities, hence paying interest rate of

        3.49% per month.

        Delay : 5 months since past, nothing has been received till date i e 26th february 2020.

        This letter : please consider this letter as a formal one as advised by ombudsman and also the

        Consumer affair ministry of govt of west bengal for immediate settlement. And also i lik to

        Ask you to reimburse the unnecessary huge interest is causing along with the claim money.

        Situation : it is unbearable physical, mental and financial torture to a 78 year old senior citizen who

        Paid regular premium which is amounting to more than 5 lakhs till date (Last premium of

        Rs 26, 864/- paid on 23/3/2019, receipt no[protected]

          Regarding the claim number mdi4906766i have submitted the “delay submission letter” alongwith the documents submitted to md india's office.

          The claim was related to post hospitalisation of my mother sukla saha, date of discharge was 16th may 2019 & 60 days was supposed to be over on 15th july 2019. My main claim was settled through cashless so i have intimated the same to my agent mousumi mukherjee that i will submit my post hospitalisation claim after the expiry of 60days, accordingly i have asked her (His father tirtho mukherjee was working on behalf of her) to collect & submit all claim related documents for processing. During july 2019 to oct 2019 i was not in town due to my crop insurance duty in orrisa. I have asked my agent to coordinate the same & submit the documents to your good office but to my utter surprise he / she didn’t bothered to do the same. So i have left with no other option than to renew my policy with another agent mr. Ujjawal mallick & submit the claim documents through him.

          It is also surprising that the concerned divisional office was trying to safe guard the agent mousami mukherjee & almost forced me to change my initial letter by removing the actual in supporting part of their agent.

          Now my claim got stuck due to delay submission. It is humbel request that please re-consider my claim & make the payment or decline it with the same with proper reason so that i can move ahead accrdingly, because i have the call records & other proof to go ahead a proof against my previous agent about her services, she had enjoyed the commission against my premium paid to national insurance in return of which he / she supposed provide me the service as per requirement. So if national insurance can pay the commission to their agent from the premium received from the client then it is the duty of the organisation to take step against the agent based on the complaint received from the client.

            Ref: policy no.320904/03/0000/00044 dated 09/01/2004.

            I have surrendered the above stated policy to the branch manager, national insurance co. Ltd., power house, bhilai (Chattisgarh) on 28/11/2019 alongwith original policy document, cancelled cheque as well as aadhar copy. I was assured that the claim will be settled at the earliest.

            However smt santhi who promised to settle the claim as early as possible, now says that she has forwarded the claim to head office for settlement.

            I had similar policies issued by united india insurance company and oriental insurance company and both of them have settled the claim. It is therefore requested to do the needful from your end please.

                  Taken a policy for i phone on 29.10.2019 from xcellins and paid premium of 4236.75. I understand that they are your sub broker.
                  Phone got damaged and claim lodged on 06.01.2020, but it has been disallowed because i could not prove that i was injured.
                  Please check the genuinity of the claim and why it was disallowed
                  I have not received the insurance policy also. Looks like a fraud company.

                      And there damage is 73500 rs total estimated. Painting charge is not aplied my car. (Ambulance)
                      But pls give the labour charge. In bill of worshop labour charge is 15000 rs
                      But insurance company is provideded only 9000 rs what is the reason. This is not possible and satiesfied.

                          Respected sir,
                          My wife was admitted in astha multispecialty hospital at karjan for bacterial meningitis with saptimia on 21st april, 2019 at 11.25 pm.
                          Then she was referred by astha multispecialty hospital for m. R. I. Brain stroke protocol to vadodara. Mri brain stroke protocol was done by makarpura diagnostic vadodara on 22nd april, 2019 early morning at 2:45 am.
                          All the bills related to the treatment were handed over to national insurance ltd company. Now, i have received a letter from national insurance co. Ltd. Baroda do-i office, tilak road, vadodara-390001 stating that my claim is rejected under clause no. 5.1 (Misrepresentation, misdiscprition or non discloser of any material facts).
                          I have taken insurance with niicl for last 4 years from year 2014 to till the date. As my wife was admitted on 21st april, 2019 at 11:25 pm and immediately referred for mri and as soon as mri procedure completed she was again readmitted in the same hospital for further treatment. So there is no break even of time as it was the part of the treatment. How 5.1 there is misrepresentation, misdiscprition or non discloser of any material facts? There is no any past treatment as the dieses was occurred all of sudden on 21st april, 2019 at 11:25 pm.
                          So my case is absolute genuine and i am prejudice for rejection of mediclaim amount.in this situation where is the question of misrepresentation, misdiscprition or non discloser of any material facts? And for any query, any of our representatives may approach for clarification if required.
                          Please look into the matter and arrange to settle the claim at the earliest to avoid me to go through the consumer court.

Since last four month the officer of said branch assuring me that your claim will be settle in few days but i cannot get the claim amount.                                   Policy holder name - suryaben d shah and claim number[protected].

                                  As per above mentioned claim number total billed amount of rs.88, 968/- and out of that only rs.41, 680/- approved by hitpa. (Health insurance of tpa india ltd) and balance amount shown under deduction and reason mentioned as as per reasonable customary.

                                  If you can see in that major amount deducted under professional fees.

                                  We have asked to provide your base how they have cross verify the same on 14th aug, 19

                                  After that so many time we have send reminder for same and every time they reply same pls refer clause 3.12, reasonable and customary charges : reasonable and customary charges means the charges for services or supplies, which are the standard charges for the specific provider and consistent with the prevailing charges in the geographical area for identical or similar services, taking into account the nature of the illness/injury involved.

                                  After that on 25th aug they send settlement letter and mentioned they refer nova hospital chembur

                                  Also i have send the other hospital mine quotation and also i asked so many time the next step and also asked for grievance steps but they never reply on same.

                                  Then on 5th sep jagdish bobade sr. Executive reply as patient has undergone surgery in non-network hospital, hence claim is approved as per policy terms under reasonable and customary charges clause. Claim approved in comparison to tariff rates in network hospital in vicinity area. Note : charges are checked with apex hospital-borivali also.​ - now he mentioned apex hospital but in settlement letter mentioned something else.

                                  Also why we can go for their network hospital. We can go by where treatment going on. We have taken 2 years back surgery with same doctor and treatment going on continuously. Then why i can change doctor and hospital. It's obvious if doctor have more experience then he charged more.

                                  After that also i have send so many reminder but no reply.

                                  Then on 09th sep jagdish bobade sr. Executive reply and said now they ready to pay additional amount payable - 22188 but my total claim of rs.88, 968/- and previously approved rs.41, 68/- so what abt balance amount.

                                  Now he send me email to approve the same. But i don't know the reason why i can approve the same. If same is reasonable then they have to send the same to our account. But after that as i not received the same so finally i reply on 12 th oct but as date still there is no revert from jagdish.

                                  Also customer care also not support for same. Also in their website they have not update the same. Even when i call on mobile then still they not pickedup the call.

                                  Custome care send the call on hold and then deduct without our permission.

                                  Please help us to resolve this asap.

                                    2 Aug 2019 I am going NIC for bike insurance. And I am choosing payment mathed swip machine . My money deducted from my account but .nic manegar is asking payment is not done and I am again payment cash manager ask your payment refund minimum 7 days till time 2 and half month complete but my money is not refund

                                      Humbly and respectfully, it is to submit that despite of honourable permanent court hoshiarpur's orders the hoshiarpur office is neither affording credit to the account requested nor attending us and even not replying and paying heed to our emails. Thus, today i. E. 07th oct.2019 at about 12.12pm again i had a telephonic talk with (Through and on a mobile phone of another staff at hoshiarpur office) mr. Rakesh bhatia, asstt. Manager, who was too rude to one's imagination. He did not even listen to my request and uttered defamatory and unparliamentory language about h'able court and me. Rather assuaging the agrieved customer the offensive behavior of mr. Rakesh bhatia demands befitted punishment. The person who has no honour to the courts/ judgements of courts and that of a customer that too a senior citizen is liable for stringent action to save the image of the esteemed institution i. E. Nic.
                                      You are, sanguinely, requested to take action against mr. Rakesh bhatia and inquire the matter thoroughly and in the meantime order the hoshiarpur office to afford credit to the account of the beneficiary/claimant the amount ordered to pay by honourable permanent lok adalat hoshiarpur at the earliest and without further delay alongwith delayed period [protected]@10% i. E. W. E. F. 1st july 2019 to date.

                                      An early action and response shall be highly appreciated.
