    With due respect i would like to state that on 24.05.2019 my wife dolan mondal was suffering from severe headache and admitted to health world hospital, durgapur. I have a mediclaim policy under national insurance company. My policy no. Is [protected] and i was applied for a claim for the same. But on 25.05.2019 i was informed that my claim was rejected as per information that the patient could be treated at opd. As a result the hospital got a denial letter and forwarded to my email. But as my wife was in still pain and under the treatment of hospital, they informed me that her treatment is under process and they've to go through some more investigations. At last they took some fluid from lumber and got a result that there is some viral infection into the brain. As per doctor advice i came to know that lumber puncture could not be possible at opd. Hence i hospital was treated her in indoor basis.
    After i got the first denial letter on 25.05.2019, hospital once again process for the claim because of the lumber puncture which could be possible without getting indoor treatment. But it was denied again and when i mailed them and asked for the reason for the deny they did not furnished any kind of information to me.
    Now i just request to you that please take action in this regard that why national insurance company avoid to issue the 2nd denial letter and obliged.

      I have renew my parivar mediclaim policy in advance on 19/03/2019 which was going to expire on 27/03/2019. I did received receipt and policy copy same time and they assured to deliver my tpa cards withing 20 days after new policy effective date. Till from that time i am requesting them (Related nic official) to provide my tpa cards but i did not receive it till now. I contacted them via phone, whats app and even email them on their official email "[protected]@nic.co.in" but didn't heard back. More than three months are gone. Please do the needful.

        Greetings!!!

        Reference your letter no. 20rb01naa0686 dated 26 june, 2019

        Policy number :[protected]

        Claim ack number :ack[protected] dated 29.05.2019.

        With utter shock and disbelief i am writing this mail to you.

        I have just now received your company letter no. 20rb01naa0686 dated 26 june, 2019 intimating that my claim for policy number[protected] for rs. 31, 881/- has been approved for rs. 11, 493/- only. The letter also gives reference of some letter dated 15 june, 2019 purported be written by your company to me that i need to provide original cheque to enable your company to remit funds to my account.

        While i acknowledge part of your communication (S) to be true, i am shocked to receive above letter which contains whole bunch of lies probably with the intention to deny my lawful claim. The facts of the case are as under:

        1. I had submitted my claim on 29 may, 2019 through my insurance advisor as i was unable to move because of my immobility due to plastered leg. Ack of the same was provided by your office.

        2. As there was no communication from your office i requested my insurance advisor to follow up. At that stage i was verbally informed through her that following original documents were required by your office:

        A) echo report with film,
        B) ankle/leg report.
        C) ecg graph
        D) original prescription from doctor dated 07 & 08 may 2019.

        3. Please note that all these follow up were through the insurance advisor who had been taking all pains to be a bridge between insurer & tpa. My complements to her. There has not been any communication from your office as mentioned in your letter under reference.

        4 i was informed verbally through insurance advisor on 13 june, 2019 that some papers were not found in the claim submitted by me. I intimated her that all original documents were submitted along with claim documents. I sent a letter through insurance advisor intimating that all documents have been submitted to your office along with claim form. I was informed over telephone that some original documents amounting to about rs. 5, 000/- (Approx), were not available in records. I was then provided with your contact number and email id. On my call, you confirmed that some original reports were not traceable and i need to write you a mail that deductions towards documents not traceable may be effected. As this was to facilitate my faster claim settlement and my inability to move and check the records, i sent the confirmatory mail to you in good faith with the clear understanding that rs. 5, 000/- (Approx) will be deducted from my claim. The contents of my mail sent by me are as under:

        Quote "i had undergone surgery for ta repairs at fortis hospital. Bills for subsequent domiciliary treatment were submitted and acknowledged vide receipt number
        Ack[protected] dated 29.05.2019.

        I am informed that some of the bills/reports are not available on records submitted by me.

        As discussed, i am not in a position to move due to surgery and old age, i authorise you to deduct related bills and clear bills at the earliest. I urgently need funds to continue my treatment". Unquote

        In response to my above mail, i got the following mail from your office on 14 june, 2019.

        Quote "your case has been medically approved & is in further process."unquote

        While i was expecting credit of claim i was once again verbally communicated through my insurance advisor that i need to give an affidavit that capt. S. N. Suri and surendra nath suri are the one & same as there was a mismatch in my policy and bank account name. The same was provided by me on 21 june, 2019. After continuous follow up i was intimated by a message received from ax-nicltd that endorsement no. ………... Has been passed against your policy no...………. On 26 june, 2019.

        And today i have received your letter dated 26 june, 2019.

        Please note that deduction of a huge amount of rs. 20, 388/- which amount to rs 64% of bills is not acceptable to me. I, therefore, demand a recalculation of your claim settlement and pay me the lawful claim without any further delay. The bill in question is an interim bill and final bill on completion of treatment will be submitted in due course, which needs to be settled as & when submitted., please note that i am still under plaster and treatment under dr, pankaj walecha, the treating surgeon.

        Please note that i am holding above policy since my retirement from bank of india as chief manager since 2015 under tie up between bank of india & national insurance company. This was my first claim under domiciliary treatment in the last 4 years. The first and only bill in four years has been grossly mishandled by your company. Please be advised above policy has since been renewed vide policy number[protected] which is valid till 26 june, 2020.

        I am sure you/your office will move with all positivity in the matter. Hoping for an early revert.

          I am a owner of Mahendra Scorpio LMV Car bearing Registration No. TN 59 -BC - 7878. The registration certificate issued by the Government of Tamil Nadu is stands in my name. My vehicle was insured in National Insurance Company through Madurai Branch III ( office code 640103) on 21.04.2017 having effect from 00.00 hours on 22.04.2017 to 21.04.2018. My vehicle was met with an accident at 21.15 hours on 22.04.2017. The vehicle was repaired at TV Sundaram Iyyangar Sons Pvt. Ltd the authorized service center at Madurai. The Police complaint was registered for the accident and necessary certificate was also issued by the Inspector of Police, Traffic investigation wing Unit III Madurai, Tamil Nadu. The officers attached to Insurance Company made inspection and assessed the damages and recommended for settlement of claim. But the claim was rejected as if I sold the vehicle. Till today I am the owner. During accident the policy is coverage. Hence repudiation of my claim as if I sold the vehicle is wrong and against true fact. My claim number is[protected]. Hence the present complaint is made.

              I have done claim for my tata nano car GJ01-RS-9680 from national insurance claim. I am satisfied with the service which they are provided . I would request to complete the whole process from them by today only, so that I can take delivery of my car by tomorrow.
              I was informed from dealer that, there are some formalities of taking photos from company side which is pending, so that they can not deliver the car by tomorrow, so it is my humble request to do it as soon as possible, so that I can take delivery of my car by tomorrow end of the day. My car is in workshop since last one week.

                i am the customer of NIC, i have purchased online insurance from NIC, my Vehicle no is -HR51AM0924
                on 02/02/2019 my car is met with an accident, Spot is conducted by Mr. Mohan Lal Goyal, and DDR is also Logged, after That the Car Is going in workshop for repair and the estimate is submitted to mansa office, then the DO office bathinda assign the case to Surveyor Mr. PD Goyal but the till time claim report is not submitted by surveyor,
                the workshop manager says the car going to total loss but the surveyor misguide me and take me wrong information, i have already submitted a affidavit on the trust of surveyor but now the scrap purchaser denied to pick the scrap, because delayed in claim process

                  वर्तमान पालिसी संख्या[protected] एक्सीडेंट में कार का फ्रंट ग्लास दो बार टूट गया था। दोनो क्लेम Ms Shreeji Service Center, Bundi Rajasthan द्वारा स्थानीय ब्रांच को प्रस्तुत किये गए। बाद वाले क्लेम का भुगतान तो प्राप्त हो गया किन्तु 5-6 महीने बाद भी पहले वाले क्लेम का भुगतान अभी तक प्राप्त नही हुवा है, जो संभवतया वर्तमान पालिसी से पहले वाली पालिसी से संबंधित था। स्थानीय ब्रांच से 3 महीने पहले संपर्क किया था। उनके द्वारा शीघ्र भुगतान का आश्वासन भी दिया गया था पर अभी तक भुगतान नही किया है।

                          I am 83 years old super senior citizen. I have mediclaim policy since 1996 without any break & without any dispute. I gave an proposal to nic kashipur on 16.10.2018 with ch no 642842 dt 16.01.2018 for rs 14090.00. Cheque was debited to my a/c & credited to a/c of nic kashipur. But they did not issue my policy till now. They say that their system is not accepting proposal of person aged above 80 years. They were saying to their higher offices & it deptt. To give any solution. After recieving any answer, they will issue the policy. But till now they have not issued policy. I went to office on 30.01.2019 to know the status of my policy (18-19) and to renew the policy for 19-20. I submitted premium by bank draft no 135844 dt 30.01.2019 for rs 14200.00 payable at pnb kashipur. They recd the draft & returned it on 20.02.2019 saying that higher offices have not given any solution to renew policy for 18-19 & 19-20. I also complaint to nic d. O. Haldwani & nic r. O. Dehradun. Please arrange to issue my both policies.

                            I want a claim in the month of april and it has been since 1 and 1/2 month, my claim isn't been solved.
                            The employees in the office are only doing their duty when i am calling them to do so.
                            They are not doing out of their own.
                            This is not done and it is only harassment to a customer.

                                I am joint medical policy holder (Mediclaim policy of national insurance company no :[protected] with my wife for the last eighteen years. My policy was due for renewal on 27th march 2019 & so i issued a cheque of ₹55497/-on 19th of march 2019 & handed over to agent & cheque was presented to my bank on 27th of march 2019 & reportedly returned without any reason as intimated by the company. On the advise of my agent i purchased a pay order of the bank for ₹55647/- including cheque returning charges if ₹150/- & deposited with the company on 27th march itself. Company initially cancelled my policy but later on renewed when my agent deposited ₹2922/-additional which i paid to him as he has deposited the amount in order to keep the policy enforceable.
                                Later on agent came to me & told that there is no reason specified for return of the cheque to which i was surprised. I checked it with my bank & found that the cheque was properly in order & there was sufficient balance in the account to meet the demand of the cheque. It seems there has been some mistake between the collecting bank & paying bank.
                                My request for refund of ₹3072/ (₹150/ cheque return charges + ₹2922/ excess premium charges) is based on below mentioned considerations:
                                1. I am oldest customer of the company.
                                2. My cheque was properly drawn & sufficient balance was maintained in the account to meet the demand of cheque.
                                3. The cheque was given well in advance.
                                4. The policy stands renewed.
                                I trust that my request shall be looked into at the earliest as i am also a senior citizen.

I have been continuously following up with national insurance /vipul tpa for mediclaim cards but till now have not received. Please expedite the same.

                                    Regarding ccn: 97363993,
                                    As of now 8 mouths are over for processing my claim.

                                    As per the insurer request on 11 february 2019 regrading pending documents, i submitted tpa with icps on 18 february 2019 via medi assist india tpa pvt. Ltd.

                                    After that i did't received any update. After that i called assistant manager of nic, she is on maternity leave. So that these much delay happens.
                                    So i mailed to responsible people by informing this.

                                    After that they transfer this to action on april 03 2019.
                                    But still i did't get any update.

                                    Each week i am sending reminder on this but no response.
                                    Please do needfull

                                      My medical claim under national insurance medical policy no. 201000/50/18/10000053 and policy period 16.11.2018 to 15/11/2019, against my wife mrs kalpana choudhury as claimant, submitted on 25.01.2019 to n. I. C ltd, shillong main branch and 57 pages of all the required relevant documents along with duly filled up claimed form on 25.01.19. Subsequently the bunch of claim documents have been forwarded to the vipul medicorp insurance tpa (P) ltd against their letter reference no 20100/mediclaim claim/2018-19/4269 on 31.01.19 to their office at guwahati. By nic ltd, shillong head office. I regret to inform your office that till date my claim has not yet been settled. I therefore request your esteemed organization to kindly look into the matter and pursue the insurance company for immediate settlement of my above mentioned medical claim. Whatever the paper asked by the tpa time and again i have submitted to nic ltd without any delay.
                                      Furthermore, if my claim is not settled in another couple of weeks i shall be compelled to seek help from the hon'ble consumer court.

                                        Against a mediclaim (Mediassist claim number[protected] rased on 1st feb 19 and approved on 5 feb 19, national insurance has not settled the payment. Continuous follow up with third party administrator mediassist only results in one information, payment settlement pending with national insurance.

                                        There have been numerous follow up with mediassist and services by them are also pathetic. Escalation email to email addresses provided by them bounce back, escalation email address of national insurance as available on public website does not get a response. Hospital comments that there is a huge concern with settlement of claims by national insurance.

                                        No wonder the common man is put to inconvenience at all costs.
