Sir, I am an ex-serviceman retired from Indian Navy in 31.12.1992. I applied for ECHS Card on 09/07/2013 vide filer no. 058347/7/Hsp at Hoshiarpur, Punjab . After so many queries from ECHS Centre, every time I was told your card is awaited. I do not know whether my file is still lying in ECHS Centre at Hoshiarpur or struck at Station Headquarter Jalandhar.     Dear Sir I No.15350415 Ex-Nk G.Mohan Naik, Served in Indian Army from[protected] to[protected] after my Discharge i am living with my Pension, I am staying in Guntakal (PO/M) at Anantapuramu(Dist) A.P. ECHS has been established in Anantpauramu District since 7months Passed but till today there is no Lab equipment no Dental Equipment and No Ambulance .Recently I have sown the Dental Treatment to my wife in out side the cost is 8, 000/- it is very costly .Even i have ECHS Membership card there is no use.Hence you are requested to kindly look in to the matter personnel y and take necessary action may please be arrange the equipement immediately Thanking You Sir, Yours Sincerely, Sd/xxxx G.Mohan Naik Guntakal(A.P)

      Sir, I submitted duly completed and signed documents for re-fund of the amount, I spent for my wife's treatment held at MIMS of Calicut, to The ECHS Perinthalmanna of Malappuram District, unfortunately on refund has been received even after completing four months. Several reminder has been made the Col (Retd) Sridharan, the IOC of Pernthalmanna ECHS. All documents and original bills issued by the Hospital signed by specialized doctors have been attached with the original application, for necessary refund. It is therefore requested that the amount I spent for the treatment may please be credited to the Canara Bank - Kurukathani branch, of which an voided cheque had been attached with the above documents.         By-paas heart surgery at tongia hospital, jaipur dctors of tongia hospital did angiography and adviced me that now i can remain on medicines, subsequently i went to delhi and developed a serious heart -pain and then in extreme emergency i was admitted in apollo hospital, delhi this incident was reported to miliatary hospital delhi cantt within 24-hours of admition to apollo hospital, delhi on 13 th january 2010 after this on completion of by-paas on 14-jan-2010 surgery a bill of rupees 2.40 lacs was sumitted to echs authority at ajmer however my payment has been passed for only 1.30 lacks that is short payment by rupees 90, 000, since then i have been requesting for my 90, 000 to echs authorities at all levels but sir they haven't responded till yet.Now echs has not followed it's own laid out policy in this case therefore i request you as the welfare society to examine this short payment and deliberate denial of due payment to me as a member of echs. They have been maintaining that we have paid rs 1.30 lacks as per cghs rates . But as per echs scheme payment should have been of rs 2.40 lacks plus relief for all this delay.

          My father, ex serviceman, was admitted in FORTIS ESCORTS, AMRITSAR on21-10-13 for the treatment of BREATHLESSNESS/HTN as per hospital record under IPID NO 77326 &UHID 100/20794 . After keeping one night in OT RECOVERY and going through other preliminary investigation he was shifted to Ward on 22-10-13 at about 3p.m. On 23-10-13 at 3.30 a.m.My brother, who was attending my father, suddenly noticed problem of breathlessness to my father and immediately intimated to the person in the ward and consequently he was taken to the OT RECOVERY at 3.44a.m. My brother gave me missed call and on reverse call I was told about the problem of breathlessness to my father. I was the person who got my father admitted in the hospital and at the time of preliminary investigation, all details related to the disease and the history of previous treatment was explained to the doctor. On reaching the hospital, I requested doctor to give him the same treatment as it was given earlier at GOVT. HOSPITAL TANDA {H.P.} when he suffered the attack and than was taken there after covering the distance of one hour. However but I found that my father was on ventilator and he was not showing any signs of visible senses or response. The doctor, who was on duty at that time, told me that he has given injection to my father to give him rest and the doctor further told me that my father will be in senses after few hours and further action will be informed when SENIORS come and made us to leave the OT RECOVERY. In between nobody was allowed to visit the patient. At 12PM when the visiting hours time started, I visited the OT RECOVERY and found that nothing has been done yet and When I raised the question they called NEURO SURGEON who told that it is case of brain damage and he should have been called earlier. Sir, here I would like to bring one more thing in your notice that my father was suffering from BREATHLESSNESS problem from last three months and in these three months he had faced two same attack as he faced on 23rd oct.2013 morning. But the treatment which was given to my father was totally opposite as given earlier. In earlier treatment, the doctor suggested that during treatment patient should not be made to sleep as if he sleeps he will be silent forever .At that time doctor after taking all test pertaining to heart, brain and other vital partsput the patient on oxygen which lead to reviable. I have explained each and every detail of past decease and treatment to the doctors{ who handled the admission } at the time of admission. But this time doctor put him on ventilator and gave him injection which is used to bring the patient unconscious as required for ventilation . When I questioned the doctor for this treatment, he answered that that BP was very high at that time and to control the BP he have to put on ventilator . But the BP was noticed at that time was near about 200/120 and this was not a very big danger for my father because many time the BP of my father touched this level. And it was not so high which caused for damaged the 90 % brain. In my view, due to the injection {which was given to my father to put on ventilator } brain could not received the required oxygen and cause for brain damage. It is my humble request that entire treatment must be investigated. Including all above, many other type of carelessness I have noticed like :- 1. The communication gap between the concerned doctor and the attendant of the patient Means many time, I try to meet the doctor A.K.Chopra who was mention as doctor of my father in papers. But uptill 24/10/13 I failed to meet. All the time the front office staff said he will call you after 10 minutes. I don’t know the definition of 10 minute for doctor A.K CHOPRA and front office staff . At last I meet with MADAM PAYAL who is the member of management of Fortis Escort Hospital Amritsar and explained my all problem then she made arrangement. So 24 /10/13 at 12:30 pm I could met doctor after 3 days of admission or we can say one day before death of my father. 2. Doctor and nurses did not know even the blood group of my father after the 3rd day of admission As I have mention that my father was admitted on 21/10/13 but untill 23/10/13 when they had to infused blood the nurse asked me the blood group of my father and only than they took the sample of blood for checking the group which is a case of extream carelessness and negligence. It is very important point extreme carelessness When my father was unconscious and was in OTR and ICU I asked doctor for further action then they always replied that only when the body of my father respond, they will try to drain the clout or can suggest further treatment. Between 23 to 25 /10/13 me and my family noticed twice daily the movement in the body of my father during attendant visiting hours but doctor always refused and said it is our doubt .But on 25/10/13 in the presence of doctor A.K.Chopra and one doctor from ECHS AMRITSAR, who visited after my complaint with poly clinic echs AMRITSAR FOR MAY BE VISITED AT the instance of hospital authority, my father made two very big movements of his hand and leg. Only at time doctor agreed on the movement.however the staff attending my father said to be taking care for 24 hours did not notice any movement in my fathers body where as same was notice by us in few minutes during visiting hours . It is not end of the story .After seeing the movement in the body of my father my sister asked further treatment.On saying this Dr. A.k. Chopra said the the BP is very low so I can not do any thing. When my me and my sister requested the doctor to control the BP the doctor misbehaved and said suggest the way how to control.On this Than my sister requested that she is not a doctor and only he can find out the way. To sum up negligent, carelessness and casual approach of the hospital in attending to the patient and not taking appropriate steps for immediate proper remedial treatment during crucial hours of need has resulted in irreparable loss to the life of ex-serviceman. We tried to procure the record of treatment which they have given us reluctantly and it appears still some information is missing. The information which they gave to us is attached for reference. Our request you to investigate the details and record of the entire treatment so that such carelessness shall not repeated with other patients.We are always ready to be evidence to this investigation. On 25 /10/2013 immidiate after the visit of echs doctor from Amritsar, the doctors showed their callousness and inhuman behavior by announcing the death of the patient at about 8:40pm knowingly well that patient is from HP and body s to be taken at odd hours to his native place which is 250 km from AMRITSAR . At about 11 pm after completing all formalities body was taken to BAIJNATH .One can imagine the extent of meanness. Document of treatment from govt hospital tanda will also be send soon.             I am an paraplegically 100% disable ESM living at above mentioned address. I am supposed to get some items every month prescribed by the specialist doctor , among these one is catheter.Earlier catheters of different manufacturers were supplied then which ever was not suitable to me I was getting it exchanged from the medical store of Command Hospital.But now, for about 1 1/2 I am getting only catheters of NUFILE only which is allergic to my urine bladder,and I am also not getting these catheters exchanged also from the medical store of CH.& ECHS-medical store has only NUFILE catheters.I shall be grateful if you do some thing in my favour.

            ECHS — Free issue of Nebuliser

            First the local Medical specialist of ECHS was reluctant to sign the requirement slip. Now the medial store of 166 MH have declined saying they have no funds for such procurement. Please advise.

                Dear sir, i am echs member and my mother is undergoing cancer treatment from bl kapoor hospital. Since beginning i am facing problem in paper work submission to the hospital. We are called on the dates for chemotherapy and returned back due to discrepancy in documents submitted. Although later on we comes to know that we are getting ill treatment due to being echs case. The patients getting cash treatment are dealt properly, but in our case hardly it happened that we were treated polity. The hospital admission staff who is responsible for coordination with echs purposely misplaces the papers submitted by us, when we saw them the receiving of the documents, again they make excuses that they had sent the papers for approval from echs and it will take time. I request all concerns, please direct the hospital to care the patient, as nobody likes to visit the hospital for entertainment.

                B. L KAPOOR HOSPITAL — WRONG TREATMENT

                I can only say if some wants his or her from early death it is my advise not to admit any one in b.L kapoor hospital in delhi.
                Rest it your choice but it is alarming call for all citizens on delhi /india

                  I am an armed forces veteran. Service no.095302-y, rank: por(Tac), name: naga raju. Mudunuri, ex.Indian navy. I, along with my spouse have applied for membership under echs through army sub area, head quarters echs cell, secunderabad-10 andhra pradesh. I personally submitted the application duly filled with all necessary documents on[protected] in the office of echs hqrs.Cell,sec'bd and i hot the original receipt for receipt of application and documents.

                  It is 18 months since i applied for but till now i have not received my smart cards. I wrote 2 reminder letters to know the status but no reply from them. Whom else can i approach with my grievance ? Please show me some solution for my problem.

                  NON RECEIPT OF SMART CARD
                  Respected sir,
                  I am the widow of an Armed Service Veteran. Service No.44113, Rank ME 1, Name D.R.Gutterson, Ex Indian Navy. I have applied for membership under ECHS through the Master Warrant Officer, M WO IC ECHS, Station H Q ECHS, CHAF Bangalore - 560007, Karnataka.
                  I had personally submitted the application duly filled with all necessary documents in May 2011, with a demand draft of Rs.135/- and again in May 2012 with another demand draft for an amount of Rs.225/-.In the above mentioned office in ECHS HQ Bangalore, and I have the original receipt, for receipt of application and documents. Registration No. 15927/05/11/BLR. It has been two years now since I applied for a smart card and have not received it till date. I have been on a number of occasions to the ECHS office Bangalore, and keep calling them every month, but yet no reply about the status of the smart card.
                  Kindly let me know who else can I approach with my grievance to solve this problem.

                    I am an ECHS member dependent upon ECHS polyclinic Dehradun. My membership number is LK0001301. I was denied the investigation by Dexa Scan for the osteoporosis. I wonder whether this, my comment will reach the proper echelon of policy makers for ECHS. An anomaly that I came across regarding the ‘osteoporosis’ that effects mostly women but also elderly men and women both and those who have been given extensive dosage of steroids as treatment for other medical complexities . One of such a medical treatment is for acute asthma. I have this treatment given to me during service and having developed osteoporosis was prescribed massive dosage of Calcium with Osteophos etc for about 7 years . Iwas for a short duration atMumbai. I got a dexascan done in an empanelled Radiology Diagnostic Centre in Mumbai referred by the ECHS Poly clinic, Mumbai Upnagar. But now the ECHS at Dehradun refused me this diagnosis locally because they have not put any local private diagnostic Centre on their panel for this. I was first told by the medical officer there that men patient are not authorized Dexa Scan. But on showing them my last report they relented and referred me to the Osteopedician in the local MH today on 15Sept 2012. The specialist did advise repeat of Dexa Scan after these 5 years. The ECHS has now refered me to ECHS, Lodhi Road, Delhi and get them to refer me to either R & R or any other private diagnostic Centre in Delhi. I am an ECHS member dependent upon ECHS polyclinic Dehradun. Locally such a facility is available and those diagnostic centres are on ECHS panel for other kinds of radiological investigations. It is my request that the ECHS Polyclinic Dehradun be instructed to include this facility also. Travelling to Delhi and staying there for getting this investigation done all at my own expense is not justified.

                      I visited the Delhi Cantt Polyclinic today to get my tooth fitted which was accidently got broken. The doctor replies till all the teeth are in intact you will not be provided with artificial tooth. First you are to get removed all your teeth because the polyclinic is only providing full dentures, not one tooth facility. Can a patient expect such type of reply from a doctor ?. This reply from DS-3 Doctor from ECHS Poly Clinic, Delhi Cantt. Now I have to get treatment from private clinic by paying from my pocket. Then What is use of this Paid treatment which is compulsary for all ex-serviceman ?

                        1. Sir, I am complaining regarding the treatment being provided at ECHS Jind. The facts are given below :-

                        (a) I am living approximately 50 kms from ECHS centre Jind.
                        (b) One way bus fare is Rs 28/- + auto fare extra.

                        2. My wife is suffering from some urinary problems to whom I send to ECHS for treatment. ECHS reffered to Delhi Hospital at Jind and instruct not to take medicine from the hospital. Docter prescribed some medicines and she has to come back to ECHS again for medicines. There the medicines were not available hence she has to bought the medicines from the market but the reimbursemnent has not so far been received.

                        3. After getting treatment from Delhi Hospital my wife got no relief and again reported to ECHS Jind this time they reffered to NC Jindal hospital Hisar for treatment.

                        4. After examined , Dr Promila Sharma prescribed some medicines and I purchased the same from the Medical store at NC Jindal Hospital at Hisar.
                        5. When I asked OIC ECHS Jind for reimbursment for the medicines The OIC declined to accept the same.
                        I also discussed the matter on phone that Sir my village is 80 kms from Hisar an d further 50 kms ECHS is located and I am to expend time + money , which impact more than the medicine. OIC ECHS not replied in favour of me.
                        6. Kindly advise me what to do.

                        Ex-servicemen contributory health scheme — hassle to get add on card

                        sir,
                        Earlier my parent polyclinic was Panagarh. Due to my livelihood i am staying in Dehradun, Uttarakhand. In the mean time a girl child is born to my wife. I had applied for change of parent polyclinic and to include my daughter as a dependent. I was advised by the Dehradun ECHS HQ to apply for upgradation of Cards and a new card for my girlchild. I had applied WITH ALL DOCCUMENTS on 05 Feb 2011. Three cards except my daughter's were despatched. Now the application is returned and advised to apply for all the four cards again.

                        I am unable to understand why daughter's card have not/can not be made? Why these three upgraded cards are to be remade again?

                          my wife smt bakshish kaur refered to kidney hosp. through echs jallandhar. she was treated there[protected] to[protected].and discharged from hosp. on[protected].
                          dr.sps subramaniam advice that you take medicine from your echs and report after one week.here we reported oic echs mr hans raj.he state way refused to provide medicine and few medicine given on the request of my wife.due to lack of medicine of few days she become serious and again reffered to kidney hosp. on[protected] and disharged from[protected] when she reported Mr. Hansraj OIC ECHS Una. He declined to provide medicine causing that medicine are very costly we cant provide and till date my wife is laying on bed without medicine and injection. I also lodged complaint on complaint book of ECHS Una but in vain . Plz give your contact no.

                          One thing i want to ask that some of personals in the ECHS department should not be in this department, as we know that Indian army is the only force which is well disciplined as per to their rules and regulations, in any manner. So many soldiers died while saving the country, so many are getting bravery awards, under the command of very well educated, royal looking, passionate commandants, while in active duties.
                          Second thing i want to ask so many pensioners, widow pensioners are having all kind of facilities like monthly pensions, aerials, medical facilities especially ECHS where one can so much depend if talking to the man and woman who cant afford much expensive treatments, so Indian government has made the life of a pensioner easy by giving this facility.
                          Now third thing i want to ask is that i don't know how much it would be the truth, that when comes to the referral service to the ECHS Empanelled hospitals, the retired personals who were the in charge of ECHS poly clinics got confused or may be know that from where they would get the higher commission, they send the patients to the hospitals where the patients got treatment but not the satisfactory complete treatment, in some cases i have studied that in some hospitals doctors advice to carry on the further treatment to another hospitals, as the another hospital is not empanelled by ECHS so the private doctors make it confidential in between him and the patients, due to this the responsibility of the doctor is over, and he will take the incentive to send the patient to the hospital of his choice, money don't matters to the patients who want to be cured fully for those who are financially sound, but what about the patients who are only depend to their pensions, its a very major problem to think.
                          Now due to lack of some honest personals we have to suffer this problem because money is much more important whether it is coming through a commission or incentive or straight away through the financially sound patient as the personals may be not be satisfied with their pensions, so they need more money by putting the patient into the only actual possibility that is uncertain death.
                          I salute the INDIAN ARMY and the INDIAN GOVERNMENT which has provided this kind of valuable service, but didn't provide the honest personals to serve the country life long.
                          it mean's that they do not do your duty in the right manner.
                          In Every where office, hospital etc, have complaint box or complaint register.
                          but ECHS Una there is no complaint Register.
                          if any asking for complaint then they do-not do any thing for his/her complaint
                          ECHS una have more complaint but they do not give complaint register to patient to write your complaint's
                          or suggestions.
                          so, please change the staff in ECHS

                          ECHS send patients for medicines, after standing in queue for an hour or so, to the MH. There in the dispensary again stand in queue, then they send to central store, again same thing, he directs to go to Medical store, where after a long wait, the staff says, injection/medicine is not available (in case of Costly ones).
                          Is this the way to treat the veterans?
                          The patient asks when it would be available? Does not know the answer For name sake there will be a land line phone stuck to wall and none responds and they don't give their mobile No The treatment meted out to the veterans there, is a very shabby one.

                          My experience is out of 11 months, the patient got only three months medicines. Why non supply of medicines promptly? When CGHS is promptly giving the same medicine in the same city without asking their patients to make rounds and rounds.Is there any one to address this issue?, and set right the callous attitude of the staff in the Medical store of MH?.
                          The person responsible for ECHS Una is a very rude one. He needs immediate kick on butt. He does not respect even older ppl and has not sense of duty at all. I request the authorities to take action against this rude officer.

                            Medical officers don't make them available for patients at ECHS Polyclinics. They run their private hospitals due to which ECHS patients have to wait for hours n hours at respect ECHS Polyclinics. Even if they are available to patients, it takes only few seconds for patient diagnosis while in their private hospitals, they take 30-40 minutes for diagnosis. Unavailability of Medicine is also a common problem.

                              I ex mwo holding ECHS card face lot of problems as the nearest ECHS centere is aproximately 110km from my native and the nearest city Ajamgarh which is hardly 30km has good hospitals where i can have the benifits of medical services privately. But to avail the facilalty from ECHS Faizabad i have to spend rupees 150 as a bus fair and minmum Rs.100 for laging and boarding. Also, I have to waste two days for my normal treatments at the age of 65 this all becomes problem some to me. Hence, i am compelt to take the treatment from local doctors. I request you sir that my medical allowance may please be paid to me.

                                Hello Sir/Madam,
                                It is with very heavy heart and deep regret I’m lodging this complaint about the ECHS patient services of IAF. My Dad is an Ex serviceman(No. 257248) and is sick and needs constant hospitalization and this costs a lot of money. We really cannot afford to spend this kind of money and we are entirely dependent on ECHS facility. Initially things were going fine. Off late the hospital has refused ECHS services due to nonpayment of bills from ECHS. This is proving to be really hard on us.
                                When we approached the Officer he offered us to shift my dad to Mayo hospital in Chennai .This will only create more issues for us as we are constantly in touch with the Doctors at Sri Ramachandra Hospital, Chennai.
                                I do not have enough words to express what we as a family are going through at this point of time. There is so much stress already and at this point we are told that due to no payment of bills ECHS patients are not accepted in the hospital. And we certainly cannot see him leave us because we could not afford his treatment.
                                My dad has served the country for 30 years ,he was a part of 1971 war and IPKF operation as well. I've grown up seeing him sweat out for his country day in and day out with no specific time defined.
                                It is really sad to see the treatment we are receiving and this is not only about my DAD I’m sure a lot of other ECHS patients are facing similar issues and concerns.
                                I would really appreciate if you can do something about this and help us out.

                                  I applied for ECHS card in the month of November last year at Amritsar, Punjab. I have temporary slip with me. My medical allowance was also stopped from that time. I was not given any registeratio number at that time.
                                  I wrote a mail to jditechs1 @echs.gov.in to know what was happening to my card.
                                  First they wanted to know my application registeration number, which I never got . I brought this fact to their notice and now tyey want to know the pass word for my application which was never created as I had not applied on line but submitted all documents physically .
                                  I am a 73 year old veteran and have no strength to pursue this matter any more, please let me know
                                  a) How can I can start getting medical allowance again.
                                  b) How to get back the money already deducted for the last one year.

                                    The behaviour of ECHS staff and doctors are intolerable. There are 2 doctors in the ECHS hospital, Mysore. My father who is the ECHS member is treated by the lady doctor. I don't remember her name, but she is the only lady doctor in that hospital. My parents are being insulted by the doctor in Mysore every time he visits her for a check up..She shouts at him telling that my dad has no brain...She just does not speak politely...She does not offer a place to sit for her patients..I noticed that she is very uncivilized with the way she uses few words and the way she talks to her patients..

                                    Last time I took my father for a check up she treated me and my dad very inferior to herself.she was shouting at both o[censored]s for no reasons. After which she was on the phone talking to her relatives in Coorgi language and writing the prescriptions. As soon as she wrote the prescription she din't hand it over to use rather threw it on the table without even looking at our face and she carried on with her phone...We had several doubts to clarify, but she just ignored us as she was too much involved on her phone. I am just worried how should I trust on the prescription while written when she is over the phone...

                                    I kindly request you to look into the matter, so that patients do not get treated badly in future.

                                      This is not the first time that my card has been deactivated. Even the last time it was deactivated, no reason was given for the same. It was reactivated on my representation. So far as I am concerned, I had to run-around , that too when my wife was in urgent need of treatment.

                                      I want to know who authorises such arbitrary actions which lead to Harassment of the Senior citizens. Why should I be made to travel 90 Kilometres (both ways) for no fault of mine?

My card is damaged as its lamination got removed. Me presently in Chennai and need treatment from ECHS from Chennai.                                         Once an ECHS member holding smart cards, which are valid all over the country. Why there is need to get new smart cards on transfer of poly clinic? Uncessary wastage of money and effirts. Harrasment to the members. Please reconsider the procedure.

                                        It may for information that old smart card and software of sitl i.e. vender installed at polyclinic has feature in software where as soon as smart card is inserted in machine in outstation policliclic It will ask the option if you wish to change the polyclinic to present (new) polyclinic and if YES is selected The new policlinic will be updated in chip of card .Central org has issued the clarification if you want I .can post the auth . vijay

                                        I Ex serviceman from 131 army air defence i have taken a treatment for my mother in emergency in jun 2010 all documents as per requirement of ECHS Miraj has been submitted by me in Jul 2010 however amount of claim has not been given to me on asking to OIC ECHS Miraj Col Hargude say i send your claim to Pune office and not received from them till date. I not ask to higher Headquarter to early settle to claim. Hence you wait. Please see in to matter and settle the claim at the earliest.

                                        Sir, I am retired from Indian Army(Corps of Signal) Record Office/Jonal Center ECHS Jabalpur on 01 May 2011. I have sent my ECHS application to Signal Records during 18 Feb 2011 . My Regd. No. is[protected]/Sig. I have not received my ECHS card till date. Many problem being faced by me for treatment of my family. When I am come from Jabalpur, record office ref told me that card reached by post with in short time after many complaint record office not given satisfied answer pl write to Zonal center ECHS Jabalpur and signals record Jabalpur

                                        I, Ex-sgt Hrushikesh Das retired from Indian Air Force on 31 Mar 2011. I had sent my ECHS application from 11BRD, AF on Jan2011. But till today I have not received any letter from Record office to collect my card from Chilika.Orissa. Please help me for this .
