This is in reference to acknowledgement # [protected] dated 7th March 2008 wherein i have requested to get the last name changed to tiwari from kalra. I have submitted the following documents and i would like to some person to take ownership for this matter

1. Copy of Passport # B 2969544 with the name Shweta Tiwari
2. Copy of Driving licence # P-39301-GBN-2001 with the name Shweta Tiwari
3. A certificate from a Gazette officer, confirming the facts besides that “he knows me personally” and confirming that for official reasons my name got changed
4. Copy of divorce decree (proof of the official reasons)
I guess it is more than enough certificates and documents sent to get the name changed. In lieu of all the above documents it is requested to review the matter again otherwise i would be left with no option but to apply afresh for a new PAN card and then surrendering the present PAN card.
I really hope your employee start taking ownership and reduce hassles in the life of common people which is already under great stress. I have recently got divorced and am going through a difficult phase in my life already.
Request you to kindly take up the matter at personal level to help me

I have applied for Pan Card in 2005 & i got the card within 20 days from the date of applying with No.APUPK2641C.But i am unable to know my number in the database. I would like to know whether my card is Originally issued by the Income Tax department or not.Kindly Give me the information at the earliest.

I have still not received my income tax return for the year[protected] amount of Rs. 7000.

My name is Nagaraja N i had applied for PAN card on 2nd August, after 10days i find the status of my PAN details in UTI site in that i got my PAN number & Dispatch date,

Dispatch date: 13 th August

But still also i did not recive my PAN card to my mentioned address plz do the needful.
