Our vehicle gj-03-bt-6703 with policy no.302000/31/15/[protected]
Had been meet with an accident. whose claim was filled and final r. i. was done on[protected] and bills of works were also submitted. that process was done before 3 months ago yet no response has been given by the company. market is down now a days and we are facing a financial crisis. the money which we borrowed from the market for repairing the vehicle are to be returned back, so we have a high financial crisis.

O sir it is our humble request that you take required actions and do what is needful.

    I took a car insurance from your company for my honda city
    Reg no up93v5151 i am not very sure about date 3 months back i put an accident claim in your jhansi branch i did not get any positive response from your company side 4 or 5 times i have visited to your branch but i could not meet your officer who handle claim not even a single time i found him on his work station. And other employee refuse to entertain claim.
    I am quite worry about your services working style and behave of your employee with clients..
    Request you please let me know what is status of my claim..

      Sir,

      It is humbly submitted as under:-

      1. that i man mohan jerath, resident of 129, puda colony, green avenue, amritsar, panjab, is an old policy holder of national insurance company ltd. bo 4, mall road, amritsar (branch code 401208) and for the last several years, purchasing mediclaim policies from them. similarly, i purchased cashless mediclaim policy no.401208/48/13/[protected] from them, on 11.8.2013 for
      Rs. 1, 00, 000/-, towards general medical and rs. 200, 000/- towards critical illness under varishtha medi claim policy. which was valid for the period till 10.8.2014.
      2. that during the validity of the policy, ito get admitted in fortis escorts hospital, amritsar, for a day and claimed rs. 12, 482/- leaving behind insured amount of rs., 87, 518/- towards general medical and rs.200, 000/- towards critical illness.
      3. that i again suffered from heart problem and was to be admitted in fortis escorts hospital, amritsar on 15.10.13 for my heart bypass surgery. after the surgery, i was discharged from hospital on 23.10.13. the hospital raised an inpatient bill for rs. 2, 01, 200/- dated 23.10.2013 ’ and claimed the amount directly from park mediclaim tpa (admn.), new delhi, but they sanctioned only rs. 87, 518/- and disallowed rs. 113682/-.
      4. further on the date of discharge i. e 23.10.2013 and again on 2.11.2013, i purchased certain follow up medicines for total amount of
      Rs. 2650/-. immediately after discharge from fortis escorts hospital, i on 19.11.2013 filed a claim with your bo 4, mall road, amritsar, enclosing therewith all the documents as listed therein, which includes:-
      A. claim form
      B. original park mediclaim card no. 91310715
      C. inpatient bill for rs. 201200/- issued by fortis escorts hospital
      D. grant letter dated 23/10/13 for rs. 87518/- from tpa, new delhi..
      E. receipt dated 28/10/13 for rs. 113682/- issued by fortis escorts hospital, of their having received the balance payable amount from me.
      F. three medicines bills for rs. 2650/-.
      5. that at the time of issuing the said policy, i was told that the policy will cover rs. 1, 00, 000/- against all kinds of mediclaim and
      Rs. 2, 00, 000/- against all kinds of critical illness. despite repeated reminders for the terms and conditions of the policy in detail, your bo no.4 mall road, amritsar failed to supply any terms or conditions except whatever was mentioned in the insurance policy, supplied to me.
      6. that in the mail sent by tpa, new delhi to fortis escorts hospital (escorts heart and super speciality institute ltd., amritsar), it is clearly mentioned by tpa that only rs. 87518/- has been granted and hospital must collect the excess amount over and above the authorisation amount from the concerned member prior to discharge from the hospital as per rules and regulations of the hospital and advised the hospital to arrange further authorization for the balance amount from me, for which the i was forced to make the payment directly to the hospital vide my cheque no. 008049 dated 28.10.13 for rs. 1, 13, 682/- of bank of india, mall road branch, amritsar, duly acknowledged by fortis escorts hospital, vide their receipt no. ipd 89644 dated 15.10.2013.
      7. that when the matter was taken up by me, with your afore mentioned branch and tpa the tpa informed vide their communication no. nicchi/18465 dated 19.3.14 that further amount of rs. 9724/- has been sanctioned by them against the balance claim amount of rs. 116332/-and they disallowed rs. 106608/-. i again took up the matter with your amritsar branch no.4 and tpa, requesting them to make the balance payment of rs. 106608/- also and vide my mail dated 28.3.14, requested tpa to send the break up of amount allowed and disallowed.
      8. that tpa vide their mail dated 8.4.14 intimated me that claim has been approved for rs. 40, 000/- and reason for heavy deductions of rs. 76332/- is the compensation for cabg will be limited to 20% of s. i of critical illness under section-ii.
      9. that i again took up the matter, with your branch no.4 and tpa to make the balance payment to me as their disallowing of
      Rs. 76332/- is highly unjustified and unwarranted. your branch no.4 vide their mail dated 24.5.14 ‘ requested tpa to clarify, under which section of the policy 20% (critical illness) amount approved, so that they can satisfy the insured, regarding the claim otherwise please send break up to insured directly.
      10. that the tpa vide their letter dated 5.8.14 intimated me that they have closed the claim file.
      11. that the tpa also vide their mail dated 18.6.15 and dated 19.6.15 intimated me that my request is being forwarded to the concerned department for processing. they also mentioned that in case the complainant is not satisfied with their settlement, he may take up the matter with the policy issuing office, as the complete claim file has been sent to them on 12.6.15 for their final decision.
      12. that i further vide my regd. letter dated 22.8.15 again requested your bo no.4, under intimation to tpa, to make the full payment of rs. 116332/-, failing he shall be compelled to file a complaint with the district consumer dispute redressal forum.
      13. that tpa vide their mail dated 12.3.15 again intimated me of their having sanctioned rs. 40000/- against pending claim of
      Rs. 116332/-. the tpa have been asking me to send them receipt for full and final settlement of the amount to enable them to send payment of
      Rs. 40000/- and further threatening to close the file, in case i do not send them my approval for the same.
      14. that since then i have been following up the matter with your bo no.4, but with no result. your bo no.4 has been playing, delaying tactics with one or the other excuse and are unnecessarily delaying to make the genuine payment of pending claim.
      15. that the refusal to admit my genuine claim by your bo no4 and tpa is most unjustified, unwarranted and unfair and has caused harassment and mental agony to me your bo no.4 and tpa are not only deficient in service, but has also played unfair trade practice with me.
      18. that from the perusal of all the facts stated here before, your goodself will come to the conclusion that the action taken by your bo no.4 and tpa in not admitting my genuine claim is highly unwarranted, illegal and highly unjustified and against the principles of law and natural justice.
      It is therefore, prayed that my grievances kindly be examined on merits and your bo no.4 and tpa please be directed to make payment of my very genuine claim..

        Dear sir,

        I have submitted my all documents at your ambadker circle office last month and they say u will get the money in 7-10 days but still m not receiving money. When m calling in your office the same answer received wait 7-10 days what is this??? My claim already to late first hyundai agency misguiding me more days after nic agent misguiding me and now... Tell me one thing when u r doing our vechile insurance then u taking money immediate and when we claimed u r delaying why??? So now tell me when i will get the money pls do asap... Its my second complaints to u.

          I renewed my above mentioned mediclaim policy on 28/4/16, but as of today i have not received cashless cards for me and any of my family members. i brought this matter to the notice of the manager of the issuing branch who seems to be in a helpless position. i shall highly appreciate if anyone looks into the matter and do the needful in this regard.

            Sir
            screen of mobile was damaged as it was in pocket of my daughter due to slip of scooty in first week of july 2015. The service center of micromax mobile charged Rs.5250 for change the damage screen after 20 days i was able to get the mobile.from that day i am unable to get the claim of my mobile.
            as I have submitted all the documents required by them mention below -:

            1) Original Repair bill (with PAID Stamp and Seal by the repairer OR proof of payment) - parts name, labour charge and IMEI no / Serial Number or Jobsheet number should be mentioned; spare parts

            2) Copy of Estimate/ Job-sheet – IMEI no/ Serial Number should be mentioned- Damage should also be mentioned

            3) Copy of Insurance certificate AND Purchase Invoice (Stamp, sealed and signed by the Dealer)

            4) Original claim form

            5) Letter of description of the Complete Incident with date and time self written by the Insured
            Description Letter -
            For authorizing and admitting the captioned claim under Accidental Damage policy - Please write the entire description ( step –by – step of the occurrence) - with date/ time/ place and the entire incident how the product was damaged. Where was the phone kept during accident.


            6) Cancelled chq photocopy for reference to fund transfer by the Insurance co’

            7) Photo ID card

            Claim documents mailing address is mention below I have courier all the documents need by them for four times kabhi kuch kabhi kuch do somthing sir its my humble request .

              My health insurance policy no. is 361400/48/15/[protected] with your company gurgaon branch. i had a total knee replacement operation at primus hospital new delhi on march 22, 2016. the claim was submitted to vipul medicorp on 6th april 2016. the claim amount is about 4 lakhs. despite answering all their queries to their satisfaction, vipul medicos are not making payment of the claim. they are giving vague answers when contacted on their helpline[protected]. i have sent them e mail reminders also but no satisfactory reply is coming from them. they donot allow access to any senior manager of theirs. my cliaim file no with them is 17rb01naa0329. i have complained to the national insurance company gurgaon divisional office also as they have issued my policy. i donot understand why they are harassing your customers. they should be held accountable for such actions of theirs.
              Please let me know when i can get the payment of my claim and why it has been delayed by your tpa.

                My claim id is[protected] of september - 2015 for amount rs 21, 134/- till today as approx 10 months passed i have not received my payment for this claim. As per e-meditake this claim was approved for payment on 13th may 2016. I can not understand why national insurance is taking so much time to pay me the amount which already approved? When i talk to concern person every time he give me different reasons for delay in payment. There is no satisfy reply i have received from national hub, kandivali.

                  Baroda health policy (Expiring on 29.09.16) not received as yet while health cards by raksha tpa against policy no 361701/48/15/[protected] membership id [protected] are received on time. Several contacts in bank of baroda surya nagar ghaziabad-201011 has been made. They also contacted to vishwas nagar bob new delhi and laxmi nagar district centre new delhi. To add further i want to inform you that at the initial stage some person in bank my policy register in new delhi instead of ghaziabad (Up).

                    I m uma shankar
                    From raju park khan pur new delhi.
                    I have a bike insurance policy (Cirt no[protected]) (Mob no [protected]) through ur company.
                    After 3 months got a accendent with other vechil. Then i m going the authorised workshop and set a claim. After that my vehicle has been completed after 20 days.. I receive my bike and ur serviour told i forward ur report to comp for claim.
                    Finally serivour send the report in concerned branch. One and half month is gone ur sopore branch is not disburse my claim payment from last 2 months m still wating my payment..

                      My mother Mrs. K.Rajeswari was admitted in St.John's Hospital, Bangalore on 9-1-2016 for Exacerbation of Asthma, Severe OSA, HTN, Type 2 DM, Hypothyroidism, Osteoporosis. She was discharged on[protected]. I submitted a claim for Rs.34987/- under company’s group mediclaim insurance policy which was denied by the TPA United Health Care Parekh under the below mentioned exclusion clause.
                      Clause Description: Treatment for obesity or condition arising therefrom ( including morbid obesity ) and any other weight control program services / supplies

                      While the admission was done to undergo treatment for Acute Exacerbation of Asthma, the exclusion clause which mentions treatment for obesity was applied for the rejection of the claim. Please note it is Hospitalization policy and she was there in the hospital for more than 24 hours.

                      Request to take necessary action.

                      I got a call from the number +[protected] from a person Ravi who claims to be a manager from the claim department within minutes after this submitting the complaint. He asked for my debit/credit card number saying the claim amount will be settled within 1-2 minutes if I gave this detail. While I was surprised how they could refund the claim immediately without even going through the details, I said I am not comfortable to give the card details and suggested to refund into my registered account with the TPA or I could provide the IFSC code and account number. The call was disconnected abruptly.

                        After 2 months got a accendent with other vechil. Then i m going the authorised workshop and set a claim. After that my vehicle has been completed after 20 days.. I receive my car and ur serviour told in phone i forward ur report after a week...
                        Finally serivour send the report in concerned branch. One month iz gone ur sopore branch is not disburse my claim payment they given me dates there uh com tom uh com thiz date that date.
                        No displine thiz there ur whole sttap is irrespective irresponsible.... From last 2 months m still wating my payment..

                            Reference to letter no. 360702/713 dt 15/04/2016 received on 27/04/2016 from national insurance ashok vihar branch regarding rejection the claim of policy no.360702/48/14/[protected]

                            In this connection i want to share you that i have submitted original document of claim of rs.5400/- only at national insurance wazirpur office which is now shifted at ashok vihar.

                            After some time i have received a letterfrom fro park mediclaim for submitting original document.
                            When i have submitted original document at wazirpur branch and if it has been lost by your side than, what is my fault in this case. When my agent has met the concerning officer at ashok vihar, he unofficial admitted that some document has been missed at the time of shifting of office from wazirpur to ashok vihar. I again submitted photocopy of all documents at
                            Park office but no result has come out.

                            I have send several mail to park mediclaim and also request them to tell me the other options (Document attested by concerning hospital or other) but always i have received letter to submit orignal documents and last time when i call them they said we have send your file to ashok vihar and now i have received this letter from n.Insurance ashok vihar.

                            How can you reject my claim?

                            Do you give me any pre-information for rejection.

                            Why did you not inform me about the other options for getting cliam.
                            I have seen in claim file that orignal document of rs.1200 of ct scan
                            Is atttched in the file. Why you have rejected that amount also?

                            Last 11 months i and my agent are regularly follow up with you and park mediclaim,
                            I am wondering here n there due to your fault and you have even not advising me about other options to solve the issue, after one year you are just sending a letter of closing my file.

                            I again request you to take this matter seriously, if you lost my document than tell me the other option to resolve this issue because i have submitted all the document on time and on right place.
                            I always submitted my policy premium for this harasment. If i do not get any resoltion from your side than other option are open with me to take the
                            Claim amount and compensation of harsment.

                              My another query, i want to change my son & daughter name in policy as per actual name. I have submitted all document like adhar card, school i card of both to my agent. But till now 25 day gone no feedback come from nis & no reply from ur agent. He is not picking my phone. What i can do.

                              If no solution get, i will go to consumer court regarding both issue. & will complaint against nis & nic agent. I have all proof for this.

                                Would like to bring in your kind notice that i have swift d-zire vdi model aligned with national insurance company limited - under (Private policy zero depth) which had met with an accident on 16th apr’16 got minor dent on front & rear bumper and left rear gate.

                                While course of accidental car survey, the representative had given a statement that company will be able to reimbursement only for one side damage as per policy. However i gone through to the policy terms & conditions and found there is no such description appended stating that only one sided damage is covered under policy.

                                And i clearly mention in below mail (On dated 22nd apr’16 and same on 26th apr’16) how that accident occurs and why i claimed my items but am still suffer from that problem, for your reference also highlighted in below trail mail & again attaching that mail (26th apr’16) which i send to surveyor.

                                Reason behind taken delivery of vehicle was there for one month in same condition and you haven’t take any action on those point (Only left handed gate works done) and regularly received call from dealer for delivery..

                                Remarks:- please share your final decision now because am suffering from 2nd week of april (So i can decide to file case or not).

                                In addition of below mail would like to inform you that 3 minor damages which are not impaneled to repairing history as such as boot space, front bumper & left rear gate.

                                • about front bumper which got damaged due to dislocate burnt fog lamp bulb that make hole in bumper cause to buffalo fighting
                                • about boot space is also not in repairing history
                                • about rear left gate which got damaged on road by buffalos those were fighting on road

                                All above damages occurred due to collapsed by buffalos those are manhandling on road

                                I gone through to the policy again where no such remark found that old damages i can’t claim. Please clarify?

                                Even i clearly mentioned in claim form that these damages occurred due to buffalo fighting on road which collapsed suddenly with my car and had not verbally admitted that there was a mistake in mentioning correct side of impact

                                Also apart of all above statement can you please do let me know what kind of written communication you required from the insured mr. Ashok shekhawat and how much time you will take to get it resolve as i already stated in trail mail from last week my family are getting suffered.

                                As far as it is concerned to customer satisfaction hoping for gear up the case, so that car get back to my home sooner.

                                The matter was taken up with the surveyor who has informed that the damages to both the bumpers are old as confirmed by the vehicle history provided by repairer hence can not be considered.

                                As regards the damages to the left rear door, the same also do not match with the cause of accident mentioned in claim form for which you had admitted verbally that there was a mistake in mentioning the correct side of impact. You were requested by surveyor to provide a written clarification on this, so that these damages may be considered. You are therefore requested to obtain the same through insured shri ashok shekhawat and arrange to submit the same to surveyor/this office at an early date.

                                On saturday morning 16th apr’16 i left from office to home in morning around 4:30 am on service lane bhankrota to kamla nehru nagar red light there 2 buffalos were fighting in left street, suddenly came on service lane & collapsed with my car which got damaged from left rear gate. Later sudden have applied brakes due to front fog bulb come out from holder resulted burnt bumper made hole in it. And another vehicle which was coming from back after applied sudden brake they hit my car from rear resulted boot space got damaged.

                                As per above mentioned description please un-hold the repairing work & do let me know when it will be done.

                                    Dear sir today i visit ur bahadurgarh branch for deputing final surveyor, but smt. Kamal gupta (Dm bahadurgarh) refused to mark surveyor by saying that she is not liable for marking surveyor, also said plz. Go to urs policy branch (Narwana) for marking surveyor. I was very surprised to see how much irresponsiblity of a senior officer. Plz. Do the needfull and tak necessary action against this irresponsiblity.

                                      Claim No. :DHS16Z038614. NI Company's third party DHS has closed my Mediclaim Reimbursement and refused to reimburse the claim amount. DHS is the Third Party. They had said that my claim was claimed after a long delay. During my visit to Mumbai I fell ill. My illness commenced on January 3rd and was hospitalized for a day and I was on drip, on January 10 - 11, 2016. My treatment for the illness continued, firstly at Mumbai and later at Kochi, Kerala, upto February 18, 2016. My claim was submitted on February 24, 2016. DHS sent a query regarding some missing Hospital reports` from Hinduja Hospital, Mumbai. At that time they did not mention that my claim was delayed and required an explanation, this happened later.
                                      As my residence is in Kerala, it took some time to obtain the reports from Mumbai and submit the same. Finally I was told that National Insurance did not reply to the query raised by DHS on accepting my explanation for the so -called delay in lodging claim. And DHS closed the Claim by refusing to reimburse.
                                      I am a Senior Citizen (74 Years) with Varistha Mediclaim Policy. My wife, also a Senior Citizen is an invalid due to spinal disc deterioration and unable to walk on her own. I have to personally attend to all the daily needs of my wife including supporting her for walking, bathing her, taking her to toilet, feeding her etc. In the midst of all this I had to submit the Claim Reimbursement Form.
                                      Kindly ask the Company to reimburse the amount claimed, and condone any delay under the above circumstances

                                        In this connection, kindly note that at the inception of the policy itself 7.5% additional commission was granted on the said policy by the competent authority at ho since as advised from ho, such an amount was loaded in the premium and that the norms stipulated were adhered to. The complete set of communications in this regard is annexed hereto.

                                        Under the circumstances, i fail to understand the purpose of your said letter which, in effect, reduces my commission from total 15% to 11% which, you will agree amounts to a breach of commitment by none other than a public sector insurance company and therefore, unacceptable.

                                        The undersigned has been following up for the balance commission of 7.5% due to me since last two years and i am being unnecessarily engaged in unwanted communications through e-mails and meetings in person at your do as well as ro. All this while, the company has remained in possession of such a hefty amount to the extent of rs.17.0 lacs whereas, the undersigned is legitimately entitled to it immediately on collection and deposit of the premium to the company.

                                        The undersigned find it quite unusual that officers of a public sector insurance company could withhold commission committed to the agent without giving any reason and after 24 months of expiry of the policy could come up with such a ridiculous reduced offer of 3.5% instead of 7.5%. I fail to understand under which statute of the country; such arbitrary powers have come to be vested in officials who are mandated to perform the duties in terms of the law of the land and rules of the public sector company.

                                        I have been an agent with the company and it would be a sad day if i have to bring proceedings against my company for the arbitrariness of the officers who want to withhold my legitimate remuneration in the form of agency commission, committed to me, in writing, following which, the business was canvassed, secured and place with our company.

                                        In the said backdrop, i would expect that the whole amount due to me is disbursed in its entirety without any further delay.
