unknown credit card reported on my cibil

Recently i applied for an od on my existing house loan in obc bank. The bank reported back informing me with low cibil.
On checking my cibil report i came to know that an unknown credit card is reported on my account.

Attaching here is the details i can see on cibil.
Also, i can see that someone is regularly making payments of 181rs to keep this card alive.

I have taken few loans in the period between 2015 and 2018 and every time the banker have referred my cibil score it has been above 750.

Sudeenly when i approached a bank last week for some financial assitance i came to understand that my cibil score is low. When i personally went to cibil website i saw that the score is 672. I was surprise as i has personally checked my cibil score in may and it was 765....

I had subscribed to cibil report in the month of jul2018 and i am surprised to see one personal loan information stated in my account which does not belongs to me.

The information reads as personal loan from shaha and opened on[protected] and status as written off. The amount over due is shown as rs 1, 95, 987/- i had never taken a credit card from shaha bank and had it written off/defaulted.

I had a standard chartered cc with card no – [protected], we have settled the account on 15th april 2002 ref – scb/hyd/2k2/04/72 receipts already sent to them in mail. As per that agreement we have paid the amount (Receipts available) also attaching the cumulative receipt.
