Hello,

I have the Optima Restore policy # [protected]-01. I am calling your customer care service people for last 6 months to add my new born baby to the policy. I have still not got anyone who can collect the money from me & renew the policy.

In last 4 months few people have called me & telling me that they will come to renew policy but after so many calls also noone came. My baby is already 7 months old and no one has still come to add my baby to the insurance policy, though after so many repetitive calling to the customer care people.

Is this the kind of service from Apollo Munich..??

If the baby details not added in next 10 days, i will cancel the policy & would like to get my money back.

I am Gaurav sharma holding medicalpolicy number [protected]. This policy is valid for today only.
Few days back on 5th september i hadraised request to add my wife's name Mamta rani and i was asked to pay premiumamount of rs 6848 and same amount was paid through cheque which has alreadybeen debited from my account.

Now the issue is at the time of fillingproposal form your agent mentioned Mamta's weight 4feet 1inch however actualweight is 4feet 11inches.when i checked with your agent he is saying that hehad mentioned 4feet 11inches but during mannual entry in system data entry boyread this figure as 4feet 1inch due to which Mamta's profile went in high BMIcategory.

Now i got a communication from apollostating that i have to pay 1007rs more as profile comes under high BMI

So i would request you to please get itrectified this issue on priority and update me on urgent

Moreover i have raised complaint at yourcustomercare number also

    i have taken a health policy from apollo munich my policy no is [protected], i am payng aprox INR10000 per year from last three years, this year i claimed inr14000, i have submitted all the documents to their claim settling co.(family health plan tpa co ltd) lucknow office nd while submitting documents i asked them plz check any thing elese you require but they told no every thing is o.k. And now after 15days they mailed that i have submitted corban copy of bills due to which they wont be able to procesd my claim.. I have submitted all the orignal bills which i got from hospital, how it became corban copy i dnt know... I request to every one dnt take a policy from apollo munich they are the cheater

      Why policy get begin on 12.03.2013 while drawn on 13.04.2013 (gap of 1 month, totally waste for me) ?

      Why origianl papers and card will receive after 2 months while payment is made on 14.03.2013 ?

      Why I sould pay for these 2 months ? (2 months are expected date as till now no final date received)


      I will really appreciate on response with extension of 2 months.

        Hi,

        I had filed one claim of my sister Kamini Aggarwal at your FHPL, office in Gurgaon. The details are as below:

        Name of Insured person: Kamini Aggarwal
        Name of Insured: Apollo Munich : 110102 : Noida : SO
        Claimed amount: 266944/-
        Claimed for: AVN Bilateral Hip
        Provider: SitaRam Bhartia Institute of science & research

        The said claim got rejected vide Claim ID - 55709 as pre existing disease stated by Apollo Munich health Insurance co.

        Facts:

        • The policy start date is 23rd Feb,11
        • The patient had come to know about this disease in September,2011 when she had acute pain in her left leg
        • Had we know earlier we would have not waited for six months to get her operated & let her bear this pain.
        • Upon consultation with the doctor we got it operated in October,2011
        • The doctor tried for cashless also but the same got rejected by Apollo saying that the patient can claim as reimbursement later.
        • We were suggested to pay in advance for the entire expense & later file for the claim
        • If that was the case then we should have got her treated in some govt. hospital as she is not that financially sound to bear such a huge expense.

        Than again I referred my case to their grievance cell & again got a rejection e mail on 12th Dec, 2011. Post that I got back to TPA at the hospital on 16th jan,2012 stated the reason of rejection but did not receive any reply from them. I even followed up with TPA till 18th Feb,2012 but went in vain.

        Than I referred my case to the Ombudsperson on 15th March,2012 detailing out the incident. Since then I have been following up with the Ombudsperson but did not receive any justice. Though exchanged couple of communication with them but fruitless.

        My submission here is that why do these companies harras the customers & make false promises. It seems that they are trying to escape of their responsibility. The plea that they are taking is that it is a pre disease existing for more than two years whereas I kept saying that how come a person afford to have pain for two years without any treatment. That is all.

        Moreover, we have paid the premium for two years now. Had we intended to any wilful act, we would not have paid the premium in the second year.

        I await justice & reimbursement of my claim of 266944/- along with compensation of this delay & mental harrasment.

            I recently took an Apollo Munich policy for
            my family bearing policy number 1401000/11121/[protected] dt.
            04-Mar-2013.

            Upon reviewing the policy schedule to my dismay, it was found that
            many details of the policy holder has been carelessly keyed in by
            Apollo Munich Health Insurance company. Its rather astonishing and
            shameful for me to apprehend that a leading private insurance company
            like Apollo Munich could make such insensible mistakes which could
            lead to lot of hanky-panky situation during the times when I may have
            to materialise my insurance during utmost necessity.

            I am not in India now and it may take for me months together to come
            back and pursue you all to get this rectified. I had personally written and given
            the field personnel all the name and age details to the accuracy
            as per our records. Also several govt. id proofs have been given to the field agent
            as a proof of my written statement with which she can enter the right details in the system database.

            The field agent, a woman, was all smiles and flowery words until she got the money for the policy.
            Later after that her responses were poor and we had to wait almost for a fortnight to get the reciept.

            I would like to say here that whoever entered my data must be very careless in his/her approach towards their job.
            AMHI needs to check and recruit deligent people so as to avoid such immature and demeaning incidents.

            Hope AMHI understands my problem and try to help me to get the policy
            with the right details in it. I anticipate a high level of it'sco-operation in this case.

              Hi,

              I purchased a policy (Policy# [protected]) from Apollo munich for my mother. We raised a claim for caseless and below this happened:

              We sent the request in that respone they asked for some document which we send after recieving there request.

              Again after getting those docment they asked for some other document when we sent that they asked for some other.

              and Most important thing after 48 Hrs (SLA is 6-24 hrs) they are rejecting stating that " There is possibility that this disease may be pre existing so we are rejecting your request"

                Apollo Munich Health Insurance Company has rejected my health insurance claim with out giving a valid explanation.
                I had a surgery for a fracture in my right wrist on December 2012 and they have refused to reimburse the claim on the grounds of a surgery I had 12 years ago which is completely unrelated and was on another body part. I have provided TWO DOCTORS' certificates confirming that both are UNRELATED and asked Apollo Munich to GIVE MEDICAL GROUNDS FOR REJECTION OF MY CLAIM. BUT THEY HAVEN'T DONE THIS.

                They have also kept all my original documents which I submitted while filing the claim and are not returning them in spite of repeated reminders.

                Appllo Munich have cheated me; they have been grossly irresponsible and are now unlawfully keeping my property (my documents).

                They caused me and my family great stress when they refused the cashless hospitalisation facility they had promised when I took the policy. If I did not have the economic standing to pay for the surgery, my right hand would have been damaged for life.

                They have cheated me and have caused great mental stress and trauma and should fulfil their legal responsibility.

                  Dear Sir,

                  I had taken Apollo Munich Health Insurance last year March-2012. My Policy No: 120300/11052/[protected] & its covered a sum insurance for my family ( Myself- RIAZ KHAN, My Wife TASNEEM AQSA & My Son REHAN KHAN) upto Rs 7.5 lac & also covered critical illness of each individual of Rs 7.5 lacs.

                  On Dec-12 I discovered a small (VASCULAR MALFORMATION-VENOUS) on my left thigh & went to visit a Doctor in Manipal Hospital & he advice for an MRI (report enclosed). As I been employed in UB Group, hence decided to go for an operation in Mallya Hospital only and contacted a Vascular Surgeon (Dr.Nagabhushan). He gave a date of 30th January-2013. (Dr Reports enclosed). Then Hospital Insurance representative (Ms.Neha) submitted document for Insurance claim to Apollo Munich & Claim reference No: 73298 on 28th January-13.

                  Things got worse from there on. The Apollo Munich Representative ( Dr.Yashwant-[protected]) kept rejecting the claim with various reasons. Mallya Hospital representative told that such things are very common these days with Insurance company. Better you get admitted & once we submit the admit reports & all, they will surely clear your claim.

                  Finally I got admitted on 30th Jan-13 (Thursday) & again hospital people submitted claims but Apollo Munich Representative ( Dr.Yashwant-[protected]) rejected the claim again by asking some more documents. Finally I got operated on 31st Jan-13 & while taking discharge on 1st Feb-13, Mallya Hospital has submitted all relevant documents (Copy Enclosed) in the morning. But Apollo Munich Representative ( Dr.Yashwant-[protected]) kept asking some documents & this exchange of mails continued for another 5 Hrs.

                  Finally at 4.00PM Mallya Hospital representative inform me about this which is a huge embarrassment for me & my family members. I immediately made a cash payment & took discharges before things get more obnoxious.

                  Between these exchange of mails I contacted below listed persons of Apollo Munich & ask their cooperation but rather helping me all tried to escape their hand.

                  3. Dr. Yashwant-[protected] (whom I explained many times over phone about my case but always tries to keep sending different queries)

                  Request you to look into the matter with deep concern & suggest further action in this regard. Also want a strict action against the representative mention above so that it avoid unnecessary harassment of customer & save your company image. While selling Mediclaim, these representatives knock every corner of doors & when it comes to claim (Especially from Individuals) they turn their backs.

                  1. USL Admin & HR Dept to inform all individual Insurance holder of Apollo Munich about this incidents

                    I had a health insurance with ICICI Lombard from 28.12.2009 to 27.12.2010.I changed the insurance to APOLLO MUNICH on 28.12.2010 & I was told that last year will be considered in this new policy & waiting period for many disease will be reduced by 1 year accordingly .I paid the premium cheque and due to some unavoidable reason the cheque bounced and I paid again by demand draft on 28.01.2011.My POLICY issued on 28.12.2010 was cancelled & new POLICY was issued on 28.01.2011.At that time i never thought that this policy is totally new for me & no 1 year reduction will be done in waiting period. When on December 14 2012 I applied for cashless facility for surgery of my wife the insurance company rejected the same by giving the reason that the surgery is under waiting period of 2 years but in reality almost 3 years are completed. APOLLO MUNICH is giving the reason that my policy is not a continuous policy from ICICI LOMBARD but a totally new one & it will complete 2 years on 27.01.2013.KINDLY SUGGEST WHAT TO DO as my Insurance claim is of around Rs.60000 (Sixty thousand only).

                    Hi There is one problem I got from ApolloMunich Health Insurance...ie Hospital submitted their required documents to TPA of ApolloMunich and they generated queries more than 5 times... where it can be asked once. In my experience its not actually hassle free in any case.

                      "Respected Sir,
                      This is Amar Krishna Kamath from Mumbai , who had applied for OPTIMA RESTORE –FAMILY FLOATER HEALTH INSURANCE PLAN for policy period of 2 years. I was conveyed all the features & benefits of OPTIMA RESTORE –FAMILY FLOATER HEALTH INSURANCE PLAN from the person from the office of Mr. Manish Kumar Thakur (Authorised Apollo Munich Agency Code holder: 80109153, Contact No. : [protected]). He has calculated the premium for 2 year period as Rs. 12667 after deducting 7.5% discount for advance payment.
                      I have made the payment through issue of Cheque No. 000021 drawn from Bank of Baroda A/c No. [protected] dated 11/08/2012 in the name of Apollo Munich Health Insurance Company Limited for the amount Rs.12, 667.00 towards OPTIMA RESTORE –FAMILY FLOATER HEALTH INSURANCE PLAN for 2 years. The same cheque for payment was honoured from my bank account on 25-Aug 2012 (payment reciept attached)
                      But to my surprise, on 28th August upon receiving, the policy document send by Apollo Munich Health Insurance to me through professional couriers today which mentions some another plan named EASY HEALTH FLOATER EXCLUSIVE -2 YEAR with POLICY NO.: 160800/11112/[protected] , policy inception date: 16 August 2012 issued for Insured Person Name: Amar Krishna Kamath (Member id [protected]) & Dr. Pooja Amar Kamath (Member Id [protected])
                      Also, there is a anomaly in the mention of the premium amount paid which shows the premium as Rs. 12726.80 (Gross Premium Amount)-(ie. Rupees Twelve Thousand Seven Hundred Twenty Six & Eighty Paisa) which is certainly not the premium for OPTIMA RESTORE POLICY for which I had issued a cheque to Apollo Munich.
                      The calculation shown, explained & calculated for my requirement of 3 lacs-OPTIMA RESTORE Plan1 –Age Group 18-35 years for 2 Adults is as follows:Basic Premium per year Rs. 6094 x 2 years x 12.36% tax = 13694.44 – 7.5% discount for Advance Payment Rs. 1027.083 = Rs. 12667.35(Rupees Twelve Thousand Six Hundred Sixty Seven & Thirty Five Paisa only)
                      After reading this, I am sure you have understood the issue & I eagerly look forward for the much needed correction on priority to ease out my tension.

                      NO RESPONSE OTHER THAN ELECTRONIC ACKNOWLEDGEMENT OF THE COMPLAINT ON SUBJECT: MEDICLAIM-WRONG POLICY DOCUMENT ISSUED

                        Dear Sirs,

                        I, Sanjay Ahuja, had Family Floater Mediclaim Policy No 354300/38/08/[protected] for a sum of Rs 4,00,000 from M/s National Insurance Co ltd since 2007. I had this policy for 3 years and as you can see no pre-existing diseases were mentioned as well as that no claim was received by them in a period of 3 years. I paid my last premium on 29/11/2010 am amount of Rs 13808/- vide cheque no 605520 drawn of State Bank of India, Inderpuri Branch. This cheque I had sent vide courier and the company said that it did not receive the cheque so my policy was not renewed.

                        In October 2011, I approached Apollo Munich Healthcare Limited for issuing me a fresh Mediclaim Policy for Rs 5,00,000/- each for myself and my wife separately. At that time the company had conducted several extensive checks to check if my health was perfect or not. I paid a sum of Rs 1350/- to the doctor who conducted the tests and was reimbursed a sum of Rs 425/- after issuance of the policy. My wife who was 55 years at that time also paid Rs 950/- for the tests and was reimbursed Rs 350/- after issuance of her policy.

                        On 25th April 2012 I experienced heaviness in my chest whilst walking. I took some Gelusil and Eno to relieve my symptoms which continue to persist.

                        On 1st May 2012 I went to the Local doctor, who prescribed me medicines for acidity.
                        His prescription was enclosed in my Claim form.

                        On 4th May I went back to my local doctor since I was not getting any relief, who suggested that I should get a Stress ECHO Test done.

                        On 5th may I went for the above test to Dr Dhoda’s lab on Pusa Road. My BP recorded before the test was 110/70. While the echo was OK but there was a possibility of Ischemic Heart Diseases.
                        The report in enclosed in original in the claim form.

                        I went on 5th Evening to Dr Moshin Wali, HOD Cardiology, Ram Manohar Lohia Hospital. at 4.00pm and upon testing my BP found it to be 164/95. Also he took a random Blood Sugar test using a strip and found my Blood Sugar to be 170. He prescribed Blood Pressure Reducing medicines and Insulin Producing Medicines and Cholesterol reducing medicines apart from Blood Thinning medicines and Nitro Glycerin.

                        His prescription has been enclosed in original along with my Claim forms.
                        This is the first time I had been recommended medicines for HYPERTENSION and Diabetes.

                        He also recommended me to undergo a myocardial per fusion Scan which was done on 10th of May at Delhi Institute of Functional Imaging. This test indicated a 90% blockage in my main artery of my heart. Also a weak heart valve. The report is included in original in my claim form.

                        I took 2-3 days to consult various doctors and weigh the options available before me before deciding to get an angiography done from Medicity.

                        I went to Medicity on 15th May at 10.00 am and was examined by Dr Ahmed ,Junior for Dr. R.R. Kasliwal. His prescription is as given below:-


                        I was admitted on 15th May for Angiography at Medicity and they preformed the angioplasty the same time as I was already in the Cath lab. I had deposited Rs 11000 for Angiography and they asked me to deposit another 115000 for the procedure. I had to deposit another 125000 before the discharge could take place.

                        When I was discharged on 17th they gave me a copy for the TPA desk which should contain all the relevant documents. This along with other originals was deposited by me in a couple of days to the TPA along with my claim forms duly filled.

                        The TPA M/s FHPL informed me to get the Certified Case Sheet Reports from the Hospital. I had sent my representative on 11th June to Medicity but he was sent around in circles without any result. I had written to the Doctor Dr R R Kasliwal and he has written back that I can get the concerned documents from the TPA desk.

                        I then sent the following letter to the TPA authorizing them and the hospital to hand over the case sheets to the T P A. The TPA was able to get the Certified Internal case Sheet reports from the hospital and on 5th July 2012 rejected my claim informing me that “CASE IS REJECTED DUE TO NON DICSLOSURE OF MEDICAL FACTS”

                        I sought from them the report on which they had based their points and they send me the following document:-


                        Now the points to be noted here are as under:-

                        a) I was extensively tested for medical problems by the Previous Healthcare Insurance company as well as by your company prescribed doctor. His ECG and other tests came out perfect and my policy was issued. In case I had been suffering from CAD since the last 10 years, should not it have been detected in my ECG which would have shown siginifant changes?
                        b) If my intention was to defraud the Insurance company and had I been suffering from CAD since the last 10 years, I would have sought the claim from M/s National Insurance Co ltd and not Apollo Munich Healthcare.
                        c) I would have filed for the claim within the period of 3 months of obtaining the policy from M/s National Insurance co ltd and not waited till I had actually suffered.
                        d) The Report of Dr Ahmed, the only doctor who examined me says *HTN (Newly Discovered) and *DM (Newly Discovered) where as the above report on which basis the claim was rejected says “Patient is a know case of HTN and DM and that patient had ? ACS (10 years back)”. Now which prescription or medical history is correct? Even the doctor who wrote this prescription is unsure about his diagnosis as he has put a question mark in his report.
                        e) The Report on which my claim has been rejected does not carry my name of Patient ID no of Medicity.

                            We have filed a claim with Apoolo Munich thru agent FAMILY HEALTH PLAN (TPA) LIMITED. (www.fhpl.net). The claim amount is approved and they mentioned in the renewal letter asking for premium for 2012 year. And once we paid the amount for premium, they have cloased the claim, askuing for id proof. I (C Santosh Kumar), went to FHPL office with id proof and spoke with the agent, and they told they will reopen the case soon, as. but it is more than a month. we haven't received the claim open message or claim amount. My brother has signed the register there.

                              As per requirements of Apollo Munich health Insurance company, my consulting doctor already send
                              five times the supporting letter to Apollo team regarding my Treatment in Apollo Hospital at Sarita Vihar. Now, he denies to write
                              more letters as he writes the same response again and again.

                              If they want any more investigation, then they can contact Apollo
                              Hospital.At the time of buying the policy,i am totally fit and totally
                              unaware that such kind of complications created from Apollo Munich health Insurance company in such
                              huge way.
                              please, it is enough ,that every time they are raising unwanted
                              queries again and again,please take a concrete action for the same. If
                              any one is on the bed and Apollo Munich Health Insurance Company Limited are arising unwanted queries ,it
                              is very shame full that they are increasing the complexities to the
                              patient to a great extend.

                              I also make a request ti IRDA Authority, that please look into the

                                  I, CHANDRIKA SARKAR, had taken a health Insurance policy no.130100/11001/[protected]-02, card no. [protected] for the year 2011-12 and it so happened that I had to be hospitalized from 01.02.12 to 03.02.12 due to a minor health complaint. As per claim requirements I had submitted all documents related to hospitalization, diagnosis & treatment to the Family Health Plan Ltd(FHPL) office on rd no 2, Banjara hillswell within the claims date.. FHPL is a body that processes claims for various health insurance companies.

                                  They have rejected my claims on the flimsy ground that did not submit to them my previous hospitalization details from an appendix operationa that happened in June, 2009. I explained to them my inability to produce the same since I had moved house twice after that and the related documents had gotten lost. More so this happened before I had taken a policy.They told me it would be okay if I gave a written undertaking saying the same, which I did.

                                  I request the concerned authorities to please ensure that justice is delivered in this case or else salaried people like me will have continue to be cheated by such unscruplous bodies like the FHPL and Apollo Munich health Insurance Co.

                                    I wanted to get portability from new india assurance company to appolomunich jus with the only condition as if I am getting all continuity benefits, no claim bonus etc then and then only I will get transferred if any of it is lacking I am not interested getting transferred to appolo munich….One of their agent mr rohit tondon (mob [protected]) assured me for all this will be done and got my sign on application form saying that he will fill up all necessary details…he asked for pervious policy copies I said it is not preserved so he said you give declaration saying so…I said if I declare so then will I get my continuation benefits and no claim bonuses…he said yes….Because in my policy it self it is mentioned in the bottom line that “policy in continuation since 2002” so he said it will be done…

                                    relaying on his assurance I DID not renewd my new india assurance policy because I was to get portability benefits from appolomunich…but after that appolo DID not gave me such benefits saying that I had not submitted backdated policy….

                                    So I got the copies of my back dated policies from my chartered accountant and submitted to mr sunil(manager) in ahmedabad office…he accelerated the matter and said now I will get all my continuity benefits but as of date I am not given justice,

                                    I want to take actions against appolomunich for giving me mental harassment ….And get me justice too…..

                                    I am not happy with the no claim bonus benefits passed to me and continuation benefits...
                                    both are considered as new policy ,
                                    but I am a portability customer and having my policies with new india assurance company since more than 20 years without break (I migrated to ahmedabad from mumbai in 2002 so 2002 onwards I have my medical policy with new india assurance company ) so I must get long term continuation benefits for no claim bonus as well as no disease continuation benefits to...which not given to me...I have submitted all back dated policy copies to mr sunil at your ahmedabad branch in person which proves my continuation.....
