4 months before i had personally submitted filled in signed format for the "change of mobile number" and issuance of a "cheque book" at the axis bank, lansdowne road, kolkata. Again recently 2 weeks before i have submitted similar filled in signed fprmat for the "change of mobile number" and issuance of "cheque book" at the axis bank, supela, bhilai. But the bank irresponsible staff & management is not attending it so far.
In both the above rather all cases every customer, the dealing staff teared off the acknowledgement strip from the format and throws down in the dustbin instead of filling & signing it properly and never bothered to hand it over to the customer.
Further, i have not been provided a atm debit card despite several reminders. The first card issued to me expired long back, but i didn't receive the new card upon its auto-renewal as yet. When enquired at the supela branch axis bank, they simply charged rs.200 for the card, and since it was just a renewal card, therefore i refused to pay. Since then i am left out without atm card since last 7 years.
They have deliberately charged and deducted rs.750 for missing quarterly balance by few hundreds just for few days, on my 11 years old salary/savings account with regular balances over one lakh. They are very careless and least bothered of the customers. Their own staff wrongly advice to cancel the existing account and to open a new through them.

I have submitted 2 applications (read TWO/TWICE) in your prescribed format, duly filled in & signed in, for my mobile number update as above to your Bank worthless staff appointed by you, after discussion with him/her personally handed it long back as directed there, and provided both the Active Account Number, Registered eMail as well as Mobile Number. Haraamkhors "Throw it in Dust Bin" and ask to submit again... Where is my application ??? See it and attend it immediately, the matter is pending since last 5 months at Kolkata, and since 1 month at Bhilai branches of your worthless Bank.
