Rathod, had opened saving a/c No.[protected] with your ICICI BANK, Tilak Nagar, Mumbai-89 Branch, 1 1/2 yrs ago & received a Credit Card from them having No. 4375xxxxxxxxxxxx2006 ( I am currently not having full 16 digit No. because it was destroyed/torn by your staff) & I have closed saving a/c also on 3/12/2014 & also my Credit Card was torn in to 4 pcs by your staff & also paid full outstanding amount at that time only....
